The incidence, aetiology, and adverse clinical consequences of less severe diarrhoeal episodes among infants and children residing in low-income and middle-income countries: a 12-month case-control study as a follow-on to the Global Enteric Multicenter Study (GEMS).

BACKGROUND: Diarrheal diseases remain a leading cause of illness and death among children younger than 5 years in low-income and middle-income countries. The Global Enteric Multicenter Study (GEMS) has described the incidence, aetiology, and sequelae of medically attended moderate-to-severe diarrhoea (MSD) among children aged 0-59 months residing in censused populations in sub-Saharan Africa and south Asia, where most child deaths occur. To further characterise this disease burden and guide interventions, we extended this study to include children with episodes of less-severe diarrhoea (LSD) s

Oral hydration therapy for children in Florida.

Eighty-six children in Florida died of complications associated with diarrhea between 1985 and 1990, deaths which constituted an important preventable fraction of infant mortality. The state will support health professionals in reducing the number of hospitalizations and deaths due to diarrheal complications, Governor Lawton Chiles announced in September 1991, and the Florida Department of Health and Rehabilitative Services is being awarded a $100,000 grant from the Centers for Disease Control for a three-year study on the effectiveness and utilization of oral rehydration therapy. Recent developments in the nutritional management of diarrhoea. 3. Practical approaches towards dietary management of acute diarrhoea in developing communities.

Diarrhoeal diseases are a major cause of malnutrition, partly due to poor dietary practices. Misconceptions among the general population and medical personnel lead to withholding of food or avoiding the use of nutritious, locally available and affordable foods. Breast-feeding should not be interrupted during diarrhoea. Many recent studies have shown that when cow's milk is used, full-strength milk should be fed throughout the disease and in convalescence. The concept of diluting milk with water should be altogether avoided. If necessary, full-strength milk should be mixed with equal amounts of

[Risk of high copper content in drinking water].

Copper occurs in small amounts in certain food items, but toxic exposures in Northern Europe have occurred only in connection with contaminated drinking water. Chronic exposure of small children can result in development of Indian Childhood Cirrhosis. This disease has recently been documented in Germany as a result of drinking water contaminated from corrosion of water pipes made of copper. Continued diarrhoea in small children can also be due to high copper exposure. Copper is not routinely determined in drinking water in Denmark. Further, no central registration is available concerning water

Causative agents of acute diarrhoea in the first 3 years of life: hospital-based study.

During the 2 years of the study, 402 patients and an equal number of age- and sex-matched controls were investigated for the presence of diarrhoeal pathogens. Pathogenic organisms were recovered from 277 (68.9%) patients and 97 (24.1%) controls. In the patient group, possible bacterial pathogens were found in 210 (52.2%) cases. Enteropathogenic Escherichia coli (EPEC) was the most frequently found potential pathogen, being recovered in 132 cases (32.8%) with serotypes 026, 086, 0111 and 0124 being the most frequently identified. Other bacterial pathogens identified were enterotoxigenic E. coli

Gastroenteritis associated with enteric type adenovirus in hospitalized infants.

Enteric types of adenovirus have recently been identified as a causative agent of infantile gastroenteritis. We utilized enzyme immunoassay and tissue culture techniques to evaluate prospectively the role of ET Ad in diarrhea occurring in hospitalized infants. We found that ET Ad was associated with 14 of 27 cases of diarrhea occurring during a 12-week study period in the late autumn and early winter months; ET Ad was found in the stool of only one of 72 children without diarrhea (P less than 0.001). Although adenoviruses other than ET Ad were found in the stools of two of the 27 children with

Diarrhoea amongst under three children in rural Haryana.

A community based study was conducted to assess the magnitude of the problem of diarrhoea and factors associated with it amongst children below three years of age. Two districts in Haryana state were selected by using multistage random sampling procedure. Eight hundred eighteen children in the age group of 0-35 months belonging to scheduled caste communities were studied. All children suffering from diarrhoea at the time of survey or within last 2 weeks were included for the detailed study. The occurrence of diarrhoea was highest in age group 6-11 months (28%) and was lowest in age group 0-5 m

Shigellosis in neonates and young infants.

To determine the clinical features and outcome of shigellosis in young infants, we reviewed the hospital records of 159 infants < or = 3 months of age (including 30 neonates) and 159 children 1 to 10 years of age with shigellosis who were admitted to the Diarrhoea Treatment Centre in Dacca, Bangladesh. Infants more commonly had a history of nonbloody diarrhea (82.8% vs 42.7%; p < 0.001), moderate or severe dehydration (59.9% vs 32.1%; p < 0.001), or bacteremia (12.0% vs 5.0%; p = 0.027) and less commonly had fever (32.7% vs 58.6%; p < 0.001), abdominal tenderness (1.9% vs 12.6%; p < 0.001), or

Approach to the pediatric patient with diarrhea.

Numerous viral, bacterial, and parasitic pathogens are known to cause diarrheal illnesses with increased frequency in children. Oral rehydration can be used to treat and prevent dehydration, the major sequela of diarrhea in children. The impact of diarrhea on nutrition may also be reduced through the rapid restoration of a normal, age-appropriate diet. Most diarrheal illnesses are acute and self-limited; however, increased knowledge of persistent diarrheal syndromes in children may lead to prompt recognition and diagnosis in children with diarrhea lasting more than 2 weeks.

Cryptosporidium, chronic diarrhoea and the proximal small intestinal mucosa.

The association between Cryptosporidium, chronic diarrhoea and a proximal small intestinal mucosal enteropathy was reviewed over a six and a half year period. One hundred and twenty three children with cryptosporidiosis and no clinical evidence of immune deficiency were identified. 50% of children excreting only Cryptosporidium had chronic diarrhoea. Most cases (63%) of chronic diarrhoea occurred in the first two years of life. A mild to moderate enteropathy was present in all nine children undergoing a small intestinal biopsy and seven showed the presence of Cryptosporidium adhering to villou

Oesophageal and severe gut involvement in the haemolytic uraemic syndrome.

Between 1982 and 1989, 78 children with diarrhoea-associated haemolytic uraemic syndrome (HUS) were referred to this hospital. Most presented with abdominal pain, bloody diarrhoea and vomiting. Seven had severe gastrointestinal involvement, four of whom required resection for bowel perforation or necrosis. One also developed an oesophageal stricture, a previously unreported complication of HUS. These seven children had a high incidence of other complications including hypertension, and cerebral and pancreatic involvement. One died from severe cerebral involvement, one has a residual neurologic

Safety and efficacy of three oral rehydration solutions for children with diarrhoea (Edinburgh 1984-85).

Of 357 children with acute diarrhoea admitted to the City Hospital, Edinburgh, over a 12-month period, only 5 (1.4%) required IV infusions. Three hundred and nineteen were treated with oral rehydration (OR). Of these 269 were studied in detail and 43% had signs of dehydration, but in none of them was it severe. There were no fatalities. Patients were randomly allocated to treatment with one of three OR solutions in a double blind trial. The solutions had sodium concentrations of 35, 50 and 90 mmol (mEq)/l, and dextrose of 200 (36 g/l), 111 (20 g/l) and 110 (19.8 g/l) mmol/l, respectively. Hype

Control of diarrhoeal disease in Tonga 1978-83.

Tonga, like many developing countries, suffers from a shortage of medical staff and a high morbidity and mortality from paediatric diarrhoeal disease. In 1980 a programme was started to train medical assistants and village administrators in the correct use of oral rehydration salt solution for rehydration. The effect on morbidity, mortality, and admission to hospital over the six years 1978-83 was assessed. After the introduction of the scheme the number of deaths due to diarrhoea fell considerably and the state of hydration in children admitted to hospital with diarrhoea greatly improved. It

ACUTE DIARRHOEAL DISEASE IN LESS DEVELOPED COUNTRIES. 3. METHODS FOR PREVENTION AND CONTROL.

The programme presented in this article for controlling the diarrhoeas and dysenteries of less developed countries is based on epidemiological principles applicable to acute undifferentiated diarrhoeal disease-its specific as well as its non-specific elements. The dominant importance of weanling diarrhoea requires a main emphasis on maternal and child health procedures, with nutrition singled out for attention, along with public health education and medical care of patients: this in addition to the established worth of means for promoting environmental sanitation. The several features of the s
