E-AHPBA innovation committee position paper on the future of hepato-pancreato and biliary surgery and artificial intelligence.

BACKGROUND: As artificial intelligence (AI) continues to influence surgical practice, its role in Hepato-Pancreato and Biliary (HPB) surgery is becoming increasingly significant. This paper proposes a structured framework for understanding surgical autonomy, with implications for training, departmental organization, and the future of HPB surgery. METHODS: An international survey was distributed to 1100 members of the European and African Hepatic Pancreatic and Biliary Association (E-AHPBA). The survey comprised 38 questions, designed by the Innovation Committee and the Artificial Intelligence

Robotic pylorus-preserving pancreaticoduodenectomy: Technical considerations.

Laparoscopy has revolutionized the way of thinking abdominal surgery, however, to date there are still limitations making it difficult to apply this technique to some types of surgical procedures considered technically demanding even when performed by open surgery, such as the pancreaticoduodenectomy. This technical note provides a complete description of the surgical procedure performed for the execution of a robotic pancreaticoduodenectomy through the use of the "Da Vinci Si" robotic system. Robotic systems represent a real evolution in minimally invasive surgery. We wish to emphasize this c

Thromboelastography-guided therapy improves patient blood management and certain clinical outcomes in elective cardiac and liver surgery and emergency resuscitation: A systematic review and analysis.

UNLABELLED: Essentials TEG-guided therapy has been shown to be valuable in a number of surgical settings. This systematic review and analysis specifically evaluated the effects of TEG-guided therapy. TEG-guided therapy can improve blood product utilization and enhance resource management. Use of TEG improved key patient outcomes, including bleed rate, length of stay and mortality. BACKGROUND: Thromboelastography (TEG 5000 and 6s Thrombelastograph Hemostasis Analyzer; Haemonetics) is a point-of-care system designed to monitor and analyze the entire coagulation process in real time. TEG-guided

[Modern diagnosis and treatment of duodenal injuries].

We analyzed data of Russian and foreign studies devoted to modern diagnosis and surgery for improving postoperative outcomes in patients with duodenal injuries. Duodenal trauma is relatively rare. These injuries present a clinical problem due to difficult diagnosis and severe consequences without timely treatment. There is no unified algorithm for diagnosis and management. Early diagnosis of duodenal injuries and early surgical intervention contribute to better outcomes.

[Statement of the surgical working group for liver, gall bladder and pancreatic diseases (CALGP) of the German Society for General and Visceral Surgery (DGAV) on rapid report V19-03: association between volume of cases and quality in complex pancreatic surgery].

The Institute for Quality and Efficiency in Health Care (IQWiG) was commissioned by the Federal Joint Committee (G‑BA) to carry out a systematic literature search and assessment of the evidence for the association between the hospital and surgeon volume of cases and the quality of the treatment results in complex pancreatic surgery. The results are presented in the rapid report V19-03. The report confirms an association between the number of cases and the quality of the treatment results in favor of hospitals and surgeons with high volume case numbers; however, a statement with respect to the

[Inter-fascial dissection in laparoscopic colorectal cancer surgery: principles of retraction and exposure and techniques in fascial separation].

Colorectal surgery for malignancies has evolved into an era of careful and precise dissection along mesorectal or mesocolic fascia to achieve the so-called total mesorectal excision or complete mesocolic excision. The wide use of laparoscopic technique prompted more anatomical, histological, and embryological studies. This leads to a deeper and more precise understanding of fascial anatomy concerning colorectal surgery, though controversies exist. The complicated anatomy of multilayer parietal fasciae and dense adhesion between fasciae at specific sites still represent a major hindrance to per

Anesthesia for Patients Undergoing Peroral Endoscopic Myotomy Procedures: A Review of the Literature.

Idiopathic achalasia is a motility disorder of the esophagus with important implications on anesthesia and periprocedural management. As new and more complex treatment options develop, anesthesiologists are increasingly involved with these patients. The cardinal symptoms of achalasia are as follows: dysphagia, regurgitation, chest pain, and weight loss. Achalasia is also associated with an increased risk of aspiration. Patients are frequently treated by endoscopic botulinum toxin injections, pneumatic dilation of the lower esophageal sphincter, laparoscopic Heller myotomy, or peroral endoscopi

The Relevance of Thiamine Evaluation in a Practical Setting.

Thiamine is a crucial cofactor involved in the maintenance of carbohydrate metabolism and participates in multiple cellular metabolic processes. Although thiamine can be obtained from various food sources, some common food groups are deficient in thiamine, and it can be denatured by high temperature and pH. Additionally, different drugs can alter thiamine metabolism. In addition, the half-life of thiamine in the body is between 1 and 3 weeks. All these factors could provide an explanation for the relatively short period needed to develop thiamine deficiency and observe the consequent clinical

Deflate to Extricate: A Technique for Rectal Foreign Body Removal of Inflatable Ball.

BACKGROUND: Rectal foreign bodies are commonly encountered in the emergency department (ED). Three techniques are well described in literature, including using a Foley catheter, "scooping" the object out, or grasping the object directly with ring forceps. We present a novel extraction method for an inflatable foreign body. CASE REPORT: A 27-year-old man presented to the ED 13 h after inserting a rubber inflatable child's ball into his rectum. After well-described extraction techniques failed to remove the ball, an 18-gauge needle at the end of a syringe was inserted into the rectum to punctur

Transanal pullthrough Soave and Swenson techniques for pediatric patients with Hirschsprung disease.

Both the Swenson and the Soave procedures have been adapted as transanal approaches. Our purpose is to compare the outcomes and complications between transanal Swenson and Soave procedures. This clinical analysis involved a retrospective series of 148 pediatric patients with HD from Dec, 2001, to Dec, 2015. Perioperative/operative characteristics, postoperative complications, and outcomes between the 2 groups were analyzed. Students' t-test and chi-squared analysis were performed. In total 148 patients (Soave 69, Swenson 79) were included in our study. Mean follow-up was 3.5 years. There are no

Laparoscopic pyloromyotomy and pyloroplasty in dogs.

This article describes clinical experiments involving laparoscopic pyloromyotomy and pyloroplasty in six dogs diagnosed with hypertrophy of the pyloric sphincter. Laparoscopic pyloromyotomy was performed in three dogs, and pyloroplasty was carried out in the remaining three animals. The patients were operated on based on the authors' previous experiences with experimental pyloromyotomy and pyloroplasty in pigs. Pyloromyotomy and pyloroplasty resulted in full recovery and complete subsidence of symptoms in all patients.
