Endovascular Approach to Severe Acute Type B Aortic Dissection.

Acute aortic dissection (AAD) is an important emergency that should be identified promptly. The classification of AAD follows two different systems: Stanford (which defines lesions as types A, on the ascending aorta, or B, on the descending aorta) and DeBakey, which also accounts for the extension of the aortic dissection. We present a notable case of a 63-year-old male who presented with a history of abrupt abdominal pain radiating to the dorsal region for endovascular treatment. He was oliguric with symmetric pulses in the superior limbs and reduction of pulses in the left lower limbs, with

A case of severe aortic valve regurgitation caused by an ascending aortic aneurysm in a young patient with autosomal dominant polycystic kidney disease and normal renal function.

Aortic aneurysm is one several well-known cardiovascular complications in patients with autosomal dominant polycystic kidney disease (ADPCKD). Commonly affected site of aortic aneurysm and its related dissection in ADPCKD is abdominal aorta. Long standing hypertension, haemodialysis, old age are closely related with discovering of aortic aneurysm and dissection in ADPCKD. However, thoracic aortic aneurysms and its related severe aortic regurgitations (ARs) are rare in younger patients suffering from ADPCKD, especially ones who have normal renal function. Here, we report a case involving a 27-y

Intimomedial mucoid degeneration resulting in a dissecting infrarenal abdominal aortic aneurysm in a young Middle Eastern male: a case report.

INTRODUCTION AND IMPORTANCE: Intimomedial mucoid degeneration is a rare vascular disorder first depicted in 1977. It involves mucin deposition in arterial layers. This will cause elastic tissue degeneration and aneurysm formation. This pathology predominantly affects the aorta. However, it could involve other smaller vessels. Surgical treatment could become complicated by a bleeding diathesis. Therefore, a precise surgical technique is necessary to avoid the ensuing complications. CASE PRESENTATION: We present the case of a previously healthy 27-year-old Middle Eastern male who presented to o

[Acute aortic syndromes].

Acute aortic syndromes include aortic dissections, intramural hematomas, penetrating ulcers, ruptured or contained ruptured aortic aneurysms. In presence of acute thoracic or dorsal pain, elevated D-Dimers and if acute coronary artery syndrome has been ruled out, additional imaging should be performed to detect acute aortic pathologies. Acute type A dissections involve the ascending aorta. Emergent open repair is the preferred treatment. Acute type B dissections involve the thoracic descending aorta. Endovascular treatment using thoracic stent grafts is indicated in complicated cases (malperfu

An investigation of the origin, location and variations of the renal arteries in human fetuses and their clinical relevance.

We investigated the origin, localizations and anatomic variations of the renal artery (RA) in human fetuses with the aim of determining the distribution of these variations according to lateralization and gender. In total, 90 fetuses of spontaneous abortion (45 males, 45 females) with no congenital malformations were included to the study. The abdominal aorta and its branches were dissected after latex solution colored with red ink had been injected into the vessels from the thoracic aorta. In all, 180 RA dissections were performed bilaterally in 90 cases and the anatomic variations were photo

Consequences of incomplete repair of acute type A aortic dissection.

During emergency repair of acute Stanford type A aortic dissections, surgical compromises in the form of incomplete arch replacement are made due to the unstable condition of the patient and safety issues of the performing team. We report a case of delayed reoperation after previous incomplete surgery for acute type A aortic dissection in a young patient with Marfan's syndrome. He presented again with repetitive chest pain five years after initial surgical treatment. Extensive aneurysmal dilatation of the aorta and remaining dissection led to the decision to replace the ascending aorta and the

Modified technique for recovering aortoiliac homografts.

BACKGROUND: Infection of the prosthetic aortic graft bypass remains one of the most serious complications of aortoiliac bypass grafts. Use of an aortoiliac allograft as a replacement for an infected prosthetic graft is an effective treatment for this problem. However, the availability of aortoiliac grafts is affected by the high rate of procurement errors experienced during recovery. This overall error rate nationally averaged 32.5% in 2006, of which nearly a third (9.6%) were related to tears of the intima at or below the bifurcation of the abdominal aorta. OBJECTIVE: To analyze the potentia

Aortic and iliac aneurysm.

Sonography is a highly accurate method for assessing the abdominal aorta and the iliac arteries for aneurysms and their complications. Proper technique is required, however, to ensure diagnostic accuracy. With respect to abdominal aneurysms, the principal duplex applications are diagnosis, measurement, and follow-up after surgery. Assessment of contained aortic rupture is best accomplished with CT, rather than ultrasound. Aortic dissection may be diagnosed with duplex sonography, but the extent of dissection and the circulatory ramifications are best evaluated arteriographically. This review a

[Extra-anatomic bypass operation of ruptured thoracoabdominal aortic aneurysm].

A successful surgical case of ruptured thoracoabdominal aortic aneurysm of Crawford type III was reported. The patient was a 40-year-old male suffering from cystic kidney, hypertension and dissecting aortic aneurysm. The operative procedure was implantation of a large Dacron graft between the ascending aorta and the common iliac arteries, with branches of small Dacron grafts anastomosed to the left common carotid, left subclavian, celiac, superior mesenteric and renal arteries, and exclusion of the aorta.

The fate of the abdominal aorta after endovascular treatment in chronic Debakey IIIb aneurysm.

OBJECTIVES: We sought to identify the risk factors for abdominal aortic remodeling after thoracic endovascular aortic repair in patients with chronic DeBakey IIIb aneurysm. METHODS: From 2012 to 2016, 70 patients underwent thoracic endovascular aortic repair for chronic DeBakey IIIb aneurysm. The abdominal aortic diameter was measured at 3 different levels (celiac trunk, renal artery, and infrarenal aorta). Abdominal aorta status was classified as expansion or stable. Expansion status was assigned when the abdominal aortic diameter was increased over 5 mm at least 1 level. Otherwise, it was c

LTBP3 Pathogenic Variants Predispose Individuals to Thoracic Aortic Aneurysms and Dissections.

The major diseases affecting the thoracic aorta are aneurysms and acute dissections, and pathogenic variants in 11 genes are confirmed to lead to heritable thoracic aortic disease. However, many families in which multiple members have thoracic aortic disease do not have alterations in the known aortopathy genes. Genes highly expressed in the aorta were assessed for rare variants in exome sequencing data from such families, and compound rare heterozygous variants (p.Pro45Argfs∗25 and p.Glu750∗) in LTBP3 were identified in affected members of one family. A homozygous variant (p.Asn678_Gly681deli

Contemporary Results of Endovascular Repair of Isolated Abdominal Aortic Dissection with Unibody Bifurcated Stent Grafts.

OBJECTIVES: To report the midterm safety and efficacy of the Aegis™-B (Microport, Shanghai, China) unibody bifurcated stent graft for endovascular treatment of isolated abdominal aortic dissection (IAAD). BACKGROUND: Isolated abdominal aorta dissection (IAAD) is a rare event. Endovascular stent grafts seem to offer an efficient therapeutic approach to treat IAAD. However, the relatively small diameter of the infrarenal aorta and aortic bifurcation remains the main anatomical limitation to endovascular repair. METHODS: Between 2008 and 2015, we retrospectively evaluated 32 IAAD patients (21 m

A case of acute abdominal aortic dissection caused by blunt trauma.

We present the case of an 18-year-old man involved in a fall with blunt abdominal trauma. The patient had hypovolemic shock and findings of an acute abdomen. Initial computed tomography (CT) showed pulmonary contusion, pneumohemothorax, hemoperitoneum, hepatic contusion, right kidney laceration and vascular avulsion, rupture of the mesenteric vein, rupture of the right rectus muscle with bowel hernia, and infrarenal aortic dissection. There were no signs of limb or medullar ischemia. After hemodynamic stabilization and surgical repair of the associated lesions, the dissection was successfully

Bilateral triple renal arteries.

Knowledge of the variations of the renal artery has grown in importance with increasing numbers of renal transplants, vascular reconstructions and various surgical and radiologic techniques being performed in recent years. We report the presence of bilateral triple renal arteries, discovered on routine dissection of a male cadaver. On the right side, one additional renal artery originated from the abdominal aorta (distributed to superior pole of the kidney) and one other originated from the right common iliac artery (distributed to lower pole of the kidney). On the left side, both additional r

A 52-year-old man with asymptomatic giant thoracic-abdominal aortic aneurysm.

A 52-year-old man was admitted to our hospital for a "CT-diagnosed thoracic-abdominal aortic aneurysm." One week ago, the patient had repeated dry coughs and went to the local hospital for treatment. A chest radiograph revealed a huge mass in the left thoracic cavity. A further chest computerized tomography examination revealed a thoracic-abdominal aortic aneurysm and was transferred to our hospital for surgical treatment. The patient is almost healthy, with no fever, no severe chest and abdomen pain, no dyspnea, no dysphagia, or other clinical symptoms. Ten years ago, the patient underwent "a

Color flow and conventional echocardiography of the Marfan syndrome.

Imaging and color flow Doppler echocardiography are an integral part of any evaluation of a patient with the Marfan syndrome. The major cardiovascular manifestations of this condition are aortic dilation, which may involve the proximal and distal aorta, aortic regurgitation, aortic dissection, mitral valve prolapse, and mitral regurgitation. Patients who have the Marfan syndrome should have serial echocardiograms to measure aortic root diameter carefully at the sinuses of Valsalva and subsequent levels (sinotubular junction, arch, descending and abdominal aorta). Additionally, color Doppler ec

Fenestration of the intima to facilitate successful endovascular repair of a dissecting abdominal aneurysm.

Aortic aneurysm is an enlargement of the aorta with a loss of the arterial wall parallelism. There are several possible causes concerning etiology, one of which is the postsurgical presence of a patent distal false lumen. An aortic aneurysm is mainly seen after a surgery for type A aortic dissection and it represents an important late complication. Even more, in some cases the abdominal aortic aneurysm could be complicated by a patent distal false lumen, which makes it challenging for further treatment. We present a case of a 44-year-old male patient with Marfan syndrome and the history of a s

Altered course of the right testicular artery.

An unusual course of the right testicular artery was observed during routine dissection of the posterior abdominal wall of a 60-year-old male cadaver. It arose from the abdominal aorta, inferior and posterior to the origin of the right renal artery, and passed posterior to the inferior vena cava and right renal vein; it then arched anterior to the inferior pole of the right kidney and descended anterior to the psoas major muscle, crossing anterior to the genitofemoral nerve, ureter and the proximal part of the external iliac artery. Finally, it passed to the deep inguinal ring and through the

One-stage surgery for type A aortic dissection and iliac artery aneurysm rupture.

We present a patient with Stanford type A aortic dissection with malperfusion of the left kidney and lower extremity, and rupture of a common iliac artery aneurysm. After establishing lower extremity reperfusion using a percutaneous femorofemoral shunt in the emergency room, simultaneous open aortic arch repair and open abdominal aorta repair were performed. Our approach allowed concomitant repair of proximal dissection, limb ischemia, and iliac artery aneurysm rupture, all of which are critical predictors of mortality.
