Aortic coarctation (AC) is a congenital aortic narrowing. We describe for the first time the findings obtained by unenhanced post mortem computed tomography (PMCT) in a case where the death was caused by cardiac tamponade from a ruptured aneurysmal dilatation of the ascending aorta and the aortic arch without dissection combined with aortic coarctation. The patient, a 46-year-old man, was found dead at home. PMCT showed haemopericardium and dilatation of the ascending aorta and the aortic arch. This appearance led to the mistaken interpretation that the images represented a dissecting aneurysm

Bicuspid aortic valve (BAV), the most common congenital cardiac anomaly, predisposes individuals to aortic stenosis and regurgitation due to valve degeneration. Abnormal hemodynamics, arterial wall characteristics, and genetic factors contribute to ascending aorta dilatation, potentially leading to severe complications like aortic dissection. Presently, the most recent guidelines propose that individuals with BAV requiring valve replacement due to valve dysfunction should undergo simultaneous replacement of the ascending aorta when the diameter of aortic dilatation exceeds 4.5 cm. A 60-year-ol

BACKGROUND: Acute aortic syndrome, including classic aortic dissection, intramural aortic hematoma, and penetrating atherosclerotic ulcer (PAU), is a term used to describe a group of conditions with similar clinical symptoms, but with different pathophysiological mechanisms. PAU is a lesion that penetrates the internal elastic lamina through the media. It is usually located in the descending aorta and rarely observed in the ascending aorta.
CASE PRESENTATION: A 76-year-old man with a history of essential hypertension was brought to the emergency department (ED) because of a sudden-onset chest

BACKGROUND AND AIM: Conventional mitral valve replacement (MVR) is carried out under cardioplegic arrest with cross-clamping of the ascending aorta during cardiopulmonary bypass. In this case, MVR was performed with on-pump beating heart technique without cross-clamping the aorta because of the diffuse adhesion around the ascending aorta, and tube graft presence between ascending and descending aortas.
METHODS: A 47-year-old female patient had aorto-aortic bypass graft from ascending aorta to descending aorta with median sternotomy and left thoracotomy in single stage because of aortic coarct

Traumatic aortic injury is potentially fatal. Although uncommon, involvement of the aortic arch and the ascending aorta can occur. This case shows concomitant dissection of the ascending and descending sections of the aorta after blunt chest trauma where the open surgical approach was successfully performed to treat both aortic injuries.

We report the case of a 74-year-old woman who underwent mitral valve plasty for mitral regurgitation. During the surgery, the ascending aorta was dilated and turned dark red after aortic cannulation. Intraoperative transesophageal echocardiography and direct epiaortic echography revealed type A aortic dissection. In addition to mitral valve plasty, replacement of the ascending aorta was performed under hypothermic circulatory arrest. The postoperative course was uneventful. Because intraoperative aortic dissection is a rare complication, its rapid identification and appropriate management is e

Intramural hematoma of the ascending aorta occurs after rupture of the vasa vasorum. Previously considered as a first step of acute aortic dissection, it was later defined as a separate entity that may or may not lead to arterial dissection. The debate about the most appropriate treatment for a 69-year-old patient with intramural hematoma of the ascending aorta, led to this extensive review of the literature demonstrating that intramural hematoma type A is a life-threatening condition requiring urgent surgical support.

BACKGROUND: Over the past 20 years, a series of procedures have been designed to reconstruct the aortic root of patients with aortic insufficiency, in whom the pathology and hence, the surgery, spares the valve leaflets. The objective of this current study was to evaluate our midterm results comparing the reimplantation technique with the remodeling technique in patients with aortic regurgitation due to aortic dissection, aortic root and ascending aortic aneurysms.
PATIENTS AND METHODS: During the years 1993 and 2006 we operated on 209 patients with aortic regurgitation secondary to dilatatio

BACKGROUND: This study was conducted to evaluate the efficacy of simple suture reduction of the ascending aorta (SRA) performed with aortic valve replacement (AVR) in patients with borderline ascending aortic dilatation (45-50 mm).
METHODS: Ninety-eight patients (ascending aortic diameter 47.7±3.4 mm) who underwent concomitant SRA with AVR were enrolled. Median follow-up duration was 83 (IQR 27-173) months. Computed tomographic angiography (CTA) follow-up was performed at 71 (47-149) months after surgery (N.=69). At least two CTA scans were performed in 34 patients (interval = 63 [46, 156] mo

Aneurysms and dissections of the aorta--depending on their location--are the cause for a variety of cardiovascular symptoms. In most cases they are the result of a generalised disease of the vessel wall. Consequently, echographic examinations should include the entire aorta from the root to the bifurcation, systematically utilising all accessibilities. This procedure often permits a complete evaluation of the aorta and its major branches thus enabling more selective use of catheterization. Morphologic and topographic information is gained mainly from two-dimensional images, whereas functional

Acute aortic dissection type A during cardiac catheterization has been reported as a rare but fatal complication. We present a case of acute aortic dissection type A occurring during catheter manipulation in the ascending aorta during mapping of ventricular premature contraction via the retrograde approach. In the present case, transthoracic echocardiography showed no pericardial effusion and no flap of the aorta, but intracardiac echo clearly showed the flap. Enhanced computed tomography revealed the aortic dissection, which extended from the ascending aorta to the bilateral common iliac arte

BACKGROUND: Aortic dissection involves a tear leading to a false lumen. According to the Stanford classification, there are 2 main types-type A, involving the ascending aorta, and type B, exclusively involving the descending aorta. It is typically characterized by chest pain. However, some patients are asymptomatic and incidentally found with dissection.
CASE SUMMARY: A 43-year-old patient presented to a primary care physician with palpitations. Transthoracic echocardiogram revealed thoracic aortic aneurysm with a possible type A aortic dissection. A computed tomography angiography of the aor

Acute myocardial ischemia and infarction due to retrograde coronary artery dissection are uncommon complications after ascending aortic dissection. A case of acute aortic dissection with massive right ventricular failure successfully treated by replacement of the ascending aorta and postoperative right ventricular mechanical support is presented. Although mechanical assistance may be hazardous after ascending aortic replacement, because of the need of anticoagulation, it represents a valid therapeutic option when severe right ventricular failure due to acute myocardial infarction ensues.

A surgically treated case of saccular true aneurysms in the ascending aorta caused by cystic medial necrosis is reported. A 59-year-old woman, who had no findings of Marfan syndrome, was admitted to our hospital because of chest discomfort and dyspnea on exertion. Echocardiography showed that massive pericardial effusion and the dilated ascending aorta without dissection. No aortic valve insufficiency nor dilatation of the Valsalva's sinuses were found out. Chest CT scan and aortography also revealed that focal dilatation of the ascending aorta without the intimal flap and the false lumen. Bec

Endovascular intervention has emerged as a less traumatic alternative treatment for several diseases of the thoracic aorta.(1,2) However, depending on the different aortic pathologies, procedure related complications have become increasingly evident: severe complications include type I endoleaks, migration, and endograft (EG) collapse, as well as those observed during conventional surgery (eg, stroke and paraplegia).(3,4) One of the emerging and most alarming complication of thoracic endografting is iatrogenic retrograde type A acute dissection (RTAAD).(5) Retrograde type A acute dissection is

We report a successful surgical treatment of acute dissecting aneurysm of the ascending aorta associated with annuloaortic ectasia. The patient was a 29-year-old male, who was admitted to the hospital 5 hours after the onset of anterior chest pain. Aortography revealed acute dissection of the ascending aorta (DeBakery type I) with AAE. The intimal tear surrounding the right coronary ostium was found. The procedure between the right coronary artery and aorta (replaced artificial graft) using saphenous vein graft was also required in addition to Bentall's operation. Postoperative studies reveale

BACKGROUND/AIM: Hypertension is a known predictor of proximal aortic dissection, but it is not commonly present in these patients on presentation. The associations between ascending aorta with left ventricular hypertrophy, cardiovascular risk factors and coronary atherosclerosis, and outcome of these patients are not fully elucidated.
METHODS: This retrospective study included 55 consecutive patients with acute type A aortic dissection treated surgically in our institution during the last 2 years. The diagnosis was based on imaging studies. Diameter of ascending aorta was measured with echoca
