INTRODUCTION: We report a case of a diving accident associating both cerebral symptoms and signs of respiratory impairment after two dives. The objective is to describe the process for obtaining the diagnosis.
CASE REPORT: A 52-year-old man experienced mental confusion associated with hypoxaemia after surfacing. All decompression procedures were fully respected. The diver had a spatio-temporal disorientation accompanied by a marked tendency to fall asleep spontaneously. He had no dyspnoea and no cough, but crepitations at both lung bases were found with oxygen saturation at 80%.

This short review focuses on pulmonary injury in breath-hold (BH) divers. When practicing their extreme leisure sport, they are exposed to increased pressure on pulmonary gas volumes, hypoxia, and increased partial gas pressures. Increasing ambient pressures do present a serious problem to BH deep divers, because the semi-rigid thorax prevents the deformation required by the Boyle-Mariotte law. As a result, a negative-pressure barotrauma (lung squeeze) with acute hemoptysis is not uncommon. Respiratory maneuvers such as glossopharyngeal insufflation (GI) and glossopharyngeal exsufflation (GE)

PURPOSE: Underwater divers face several potential neurological hazards when breathing compressed gas mixtures including nitrogen narcosis which can impact diver's safety. Various human studies have clearly demonstrated brain impairment due to nitrogen narcosis in divers at 4 ATA using critical flicker fusion frequency (CFFF) as a cortical performance indicator. However, recently some authors have proposed a probable adaptive phenomenon during repetitive exposure to high nitrogen pressure in rats, where they found a reversal effect on dopamine release.
METHODS: Sixty experienced divers breathi

OBJECTIVES: Swimming has been reported as the most common recreational activity among American youths, while diving remains a popular youth activity as well. We characterize the most common facial injuries occurring during these activities and evaluate facial fracture mechanisms and demographic trends, as this information may be helpful in preventative counseling as well as diagnosis.
METHODS: The National Electronic Injury Surveillance System was assessed for swimming- and diving-related facial injuries in children from 2007 to 2016. Estimates of national injury incidence were recorded, and …

INTRODUCTION: Onset of cochleovestibular symptoms (hearing loss, dizziness or instability, tinnitus) after a dive (scuba or breath-hold diving) warrants emergency transfer to an otology department. One priority is to investigate the possibility of the development of decompression sickness with a view to hyperbaric oxygen treatment of bubble-induced inner-ear damage. If this injury is ruled out, inner-ear barotrauma should be considered together with its underlying specific injury pattern, perilymphatic fistula.
METHODS: We report on a series of 11 cases of perilymphatic fistula following ear …

OBJECTIVE: The glossopharyngeal insufflation maneuver (lung packing) is largely performed by competitive breath-hold divers to improve their performance, despite observational evidence of fainting and loss of consciousness in the first seconds of apnea.
METHODS: We describe here the time course of hemodynamic changes, induced by breath-holding with and without lung packing, in 2 world-class apnea competitors.
RESULTS: When compared with apnea performed after a deep breath (100% vital capacity), lung packing leads to a decrease in cardiac output, blood pressure, and cerebral blood flow during

During a survey of black yeasts of marine origin, some isolates of Hortaea werneckii were recovered from scuba diving equipment, such as silicone masks and snorkel mouthpieces, which had been kept under poor storage conditions. These yeasts were unambiguously identified by phenotypic and genotypic methods. Phylogenetic analysis of both the D1/D2 regions of 26S rRNA gene and ITS-5.8S rRNA gene sequences showed three distinct genetic types. This species is the agent of tinea nigra which is a rarely diagnosed superficial mycosis in Europe. In fact this mycosis is considered an imported fungal inf…

Population decline and a shift in the geographical distribution of some ectothermic animals have been attributed to climatic warming. Here, we show that rises in water temperature of a few degrees, while within the thermal window for locomotor performance, may be detrimental to diving behaviour in air-breathing ectotherms (turtles, crocodilians, marine iguanas, amphibians, snakes and lizards). Submergence times and internal and external body temperature were remotely recorded from freshwater crocodiles (Crocodylus johnstoni) while they free-ranged throughout their natural habitat in summer and…

Brain (T(brain)), intra-aorta (T(aorta)), latissimus dorsi muscle (T(m)) and rectal temperature (T(r)) were measured in harp (Pagophilus groenlandicus) and hooded (Cystophora cristata) seals during experimental dives in 4 degrees C water. The median brain cooling was about 1 degrees C during 15 min diving, but in some cases it was as much as 2.5 degrees C. Cooling rates were slow for the first couple of minutes, but increased significantly after about 5 min of diving. The onset of cooling sometimes occurred before the start of the dive, confirming that the cooling is under cortical control, li…

Ever since the first deep diving competitions were organized, there has been debate about when the ultimate limits of human apnoeic performance will be reached, and which factors will determine these limits. Divers have thus far surpassed all former predictions by physiologists in depth and time. The common factor for all competitive apnoea disciplines is apnoeic duration, which can be prolonged by any means that increase total gas storage or tolerance to asphyxia, or reduce metabolic rate. These main factors can be broken down further into several physiological or psychophysiological factors,…

Scuba diving has become a popular leisure time activity with distinct risks to health owing to its physical characteristics. Knowledge of the behaviour of any mixture of breathable gases under increased ambient pressure is crucial for safe diving and gives clues as to the pathophysiology of compression or decompression related disorders. Immersion in cold water augments cardiac pre- and afterload due to an increase of intrathoracic blood volume and peripheral vasoconstriction. In very rare cases, the vasoconstrictor response can lead to pulmonary oedema. Immersion of the face in cold water is …

Underwater diving is a very closely medically managed activity. Performing it, the human organism is under the physical laws of pressure and following consequences. The expiratory flows are significatively reduced, enhancing the risk of alveolar hypoventilation at exertion, the central nervous system is the privileged target during inopportune tissue degassing related accidents (leaving 20% of sequellae), barotraumatic injuries threaten middle and inner ear or lung (pulmonary barotrauma is the most severe accident), the toxicity of gas under pressure (i.e. oxygen, nitrogen) exposes to specific…

This consensus statement is the result of a workshop at the SPUMS Annual Scientific Meeting 2014 with representatives of the UK Sports Diving Medical Committee (UKSDMC) present, and subsequent discussions including the entire UKSDMC. Right-to-left shunt across a persistent or patent foramen ovale (PFO) is a risk factor for some types of decompression illness. It was agreed that routine screening for PFO is not currently justifiable, but certain high risk sub-groups can be identified. Divers with a history of cerebral, spinal, inner-ear or cutaneous decompression illness, migraine with aura, a fam…

BACKGROUND: Predator movements vary across different ecological contexts, offering valuable insights into their foraging strategies. However, studying these contexts in marine predators is challenging due to the difficulty of observing them and their prey over sufficient spatiotemporal scales. Using bio-loggers and detailed life history information, we investigated abiotic and biotic factors shaping the diving behavior of a highly social apex predator—the false killer whale (Pseudorca crassidens)—around the Hawaiian Archipelago where three partially sympatric, genetically differentiated popula…

INTRODUCTION: The microbiological safety of compressed air in SCUBA diving cylinders is a crucial yet often overlooked aspect of diving safety. Despite regular maintenance, microbial contamination can cause respiratory illnesses, especially during the air-filling procedure. Given the impact of temperature increases on bacterial viability, this study evaluates the survival of Streptococcus pneumoniae (S. pneumoniae) and Pseudomonas aeruginosa (P. aeruginosa) within diving SCUBA cylinders during and after the air-filling process.
METHODS: A mixture of Streptococcus pneumoniae (ATCC 49619) and P…
