Congenital aberrant tearing: a re-look.

PURPOSE: Congenital aberrant tearing is characterized by tearing when eating ("crocodile tears"), lack of emotional tearing, or both. Most reported cases are associated with Duane syndrome. In our previous studies we observed aberrant tearing in individuals with thalidomide embryopathy and Möbius sequence. This report summarizes the literature on the subject and adds 3 new studies that give information on this unusual condition. METHODS: Twenty-eight individuals with Möbius sequence were interviewed about tearing symptoms at a support group meeting in Italy. In Sweden 30 adults primarily from

The association of an epibulbar dermoid and Duane syndrome in a patient with a SALL1 mutation (Townes-Brocks Syndrome).

INTRODUCTION: Townes-Brocks Syndrome (TBS) is an autosomal dominant condition characterized by renal, anal, ear and thumb anomalies caused by SALL1 mutations. Ocular manifestations reported have included congenital cataracts, unilateral microphthalmia, optic nerve atrophy, and unilateral visual loss with bilateral Brushfield Spots. Iris and chorioretinal colobomata were described in one individual whose daughter had Duane syndrome. METHODS: We present a case of TBS with a proven SALL1 mutation associated with unique ophthalmic features. CASE REPORT: The first child of healthy unrelated paren

Ocular-induced abnormal head postures: A systematic review and analysis.

Abnormal head postures (AHPs) are frequently adopted as compensatory mechanisms by individuals affected by various ocular diseases to optimize the utilization of their visual field or alleviate symptoms such as diplopia. We review the causal relationship between ocular diseases and the resulting AHPs. An extensive literature search was conducted to identify and summarize relevant medical studies providing evidence of AHPs in patients diagnosed with conditions like Duane retraction syndrome, Brown syndrome, superior oblique palsy, and other ocular disorders. The analysis explores the proportion

Clinical characterization of the HOXA1 syndrome BSAS variant.

BACKGROUND: The Bosley-Salih-Alorainy syndrome (BSAS) variant of the congenital human HOXA1 syndrome results from autosomal recessive truncating HOXA1 mutations. We describe the currently recognized spectrum of ocular motility, inner ear malformations, cerebrovascular anomalies, and cognitive function. METHODS: We examined nine affected individuals from five consanguineous Saudi Arabian families, all of whom harbored the same I75-I76insG homozygous mutation in the HOXA1 gene. Patients underwent complete neurologic, neuro-ophthalmologic, orthoptic, and neuropsychological examinations. Six indi

Xq26.3 microdeletion in a male with Wildervanck Syndrome.

BACKGROUND: Wildervanck Syndrome (WS; cervico-oculo-acoustic syndrome) consists of Duane retraction syndrome (DRS), the Klippel-Feil anomaly, and congenital deafness. It is much more common in females than males and could be due to an X-linked mutation that is lethal to hemizygous males. We present the genetic evaluation of a male with WS and his family. MATERIALS AND METHODS: Clinical evaluation and neuroimaging, sequencing of candidate genes, and array comparative genomic hybridization. RESULTS: The patient had bilateral type 1 DRS, fusion of almost the entire cervical spine, and bilateral

The Frequency and Manifestations of Ocular Causes of Abnormal Head Posture.

PURPOSE: To determine the frequency and manifestations of different ocular causes of abnormal head posture (AHP). METHOD: This prospective, consecutive case series study was performed on 149 patients with ocular AHP at Farabi hospital, Iran, from February 2020 to June 2021. All patients underwent routine ophthalmic examinations. The manifestation of AHP was determined by direct observation from three viewing angles, while the patient read the smallest line on the vision chart that they could see. In front, above, and lateral gazes, observations were performed to find head tilt, head turn, and

Thromboembolism and congenital malformations: from Duane syndrome to thalidomide embryopathy.

OBJECTIVE: To propose a pathophysiologic mechanism to unify a variety of disparate sporadic congenital malformations. METHODS: Inductive and deductive analyses to correlate malformation laterality with asymmetries in thoracic anatomy, critical analysis of malformations with female predominance, and concepts of hydrodynamic pressure gradients in vascular growth were applied to the ensuing development of guiding tissue scaffolds for cellular proliferation, differentiation, and apoptosis. RESULTS: Duane syndrome may develop following a focal vascular insult to the sixth nerve trunk with axonal

Strabismus in infancy.

In order to prospectively study the development of strabismus in infancy, the ocular alignment of a large population of normal neonates was studied and follow-up examinations were obtained in a subset of these infants. The characteristic findings of congenital esotropia subsequently developed in three infants who were either orthotropic or exotropic at birth. Pathologic exotropia developed in two infants; both were exotropic at birth, but no more so than most normal neonates. In infants with congenital esotropia or pathologic exotropia, the characteristic deviation appears to develop between 2

Pseudotumor cerebri in a patient with Goldenhar's and Duane's syndromes.

A 4-year-old boy presented with Goldenhar's syndrome, Duane's syndrome and bilateral papilledema. Magnetic resonance imaging of the brain was unremarkable. On lumbar puncture, the cerebrospinal fluid (CSF) pressure measured 36 cm H(2)O. CSF examination was normal. The diagnosis of pseudotumor cerebri was made, and treatment with acetazolamide was started. As the papilledema did not resolve, steroids were added to the treatment. Lumbar puncture was repeated after 1 month, and pressure was found to be 30 cm H(2)O. Because medical treatment was not effective in lowering the CSF pressure, optic ne

Adjustable vertical rectus muscle transposition surgery.

Transposition of the vertical rectus muscle to the insertion of a paralytic or aberrantly innervated lateral rectus muscle is useful in treating esotropia associated with abducens palsy and Duane syndrome. However, this procedure may cause a manifest vertical deviation, requiring either prismatic or additional surgical correction. In an attempt to reduce the incidence of this surgical complication, we performed vertical rectus muscle transposition surgery, using an adjustable suture technique on each muscle, in 10 consecutive patients with abnormal lateral rectus function. Preoperative diagnos

Case report: Identification of a novel variant p.Gly215Arg in the CHN1 gene causing Moebius syndrome.

Background: Moebius Syndrome (MBS) is a rare congenital neurological disorder characterized by paralysis of facial nerves, impairment of ocular abduction and other variable abnormalities. MBS has been attributed to both environmental and genetic factors as potential causes. Until now only two genes, PLXND1 and REV3L have been identified to cause MBS. Results: We present a 9-year-old male clinically diagnosed with MBS, presenting facial palsy, altered ocular mobility, microglossia, dental anomalies and congenital torticollis. Radiologically, he lacks both abducens nerves and shows altered symme

First-trimester prenatal diagnosis of Okihiro syndrome.

A case of Okihiro syndrome (OS) detected by 2- and confirmed by 3-dimensional ultrasound at 13+2 gestational weeks is reported. While the pregnant woman affected by the OS presented with limb anomalies, the fetus showed severe thoracoabdominal and skeletal anomalies. Termination of pregnancy was performed at 14+1 gestational weeks and confirmed the sonographically detected symptoms. The diagnosis was confirmed by autoptic, radiologic and molecular genetic analysis. To our knowledge, this is the first case of prenatal diagnosis of OS.

Ptosis in childhood: A clinical sign of several disorders: Case series reports and literature review.

Blepharoptosis (ptosis) is a common but often overlooked sign that may serve as a sign/manifestation of other conditions, ranging from a mild and purely cosmetic presentation to a severe and occasionally progressive disorder. Ptosis may show an acute onset or may manifest as a chronic disorder. Its presentation may vary: unilateral versus bilateral, progressive versus non-progressive, isolated versus complex which occurs in association with other symptoms, and congenital versus acquired (often concomitant with neuromuscular disorders).Congenital ptosis includes the isolated type-the congenital

A TUBB6 mutation is associated with autosomal dominant non-progressive congenital facial palsy, bilateral ptosis and velopharyngeal dysfunction.

Congenital cranial dysinnervation disorders (CCDDs) comprise a heterogeneous spectrum of diseases characterized by congenital, non-progressive impairment of eye, eyelid and/or facial movements including Möbius syndrome, Duane retraction syndrome, congenital ptosis, and congenital fibrosis of the extraocular muscles. Over the last 20 years, several CCDDs have been identified as neurodevelopmental disorders that are caused by mutations of genes involved in brain and cranial nerve development, e.g. KIF21A and TUBB3 that each plays a pivotal role for microtubule function. In a five-generation pedi

Does abnormal head posture aid in amblyopia prevention?

PURPOSE: To assess the prevalence and severity of unilateral amblyopia in patients with ocular abnormal head posture (AHP) and to investigate whether the presence of compensatory head posturing provides protection against the development of amblyopia. METHOD: In this retrospective cross-sectional study, the medical records of 1400 patients with various etiologies of AHP were reviewed. This cohort included individuals with congenital superior oblique palsy (SOP) (n = 758), Duane retraction syndrome (DRS) (n = 582), and Brown syndrome (n = 60), all of whom were candidates for surgical treatment

The genetic basis of incomitant strabismus: consolidation of the current knowledge of the genetic foundations of disease.

In recent years, our understanding of the genetic foundations of incomitant strabismus has grown significantly. Much new understanding has been gleaned since the concept of congenital cranial dysinnervation disorders (CCDDs) was introduced in 2002, and the genetic basis of CCDDs continues to be elucidated. In this review, we aim to provide an update of the genetic and clinical presentation of these disorders. Disorders reviewed include Duane syndrome (DS), HOXA1 and HOXB1 syndromes, Moebius syndrome, congenital fibrosis of the extraocular muscles (CFEOM), and horizontal gaze palsy with progres

A reformed "release hypothesis" for Marcus Gunn Syndrome, based on newer clinic observations and experimental evidences.

Marcus Gunn Syndrome (MGS), mostly occurring in congenital ptosis patients, is characterized by jaw movement evoking ptotic eyelid retraction, followed by collapse. Inverted, bilateral and acquired MGS were also reported. Some cases manifest MGS only temporarily in life. These features suggest MGS may be due to multiple pathogeneses, which are still unclear. People also classify MGS as a kind of trigeminal oculomotor synkenesis (TOS), like Duane syndrome (DS), in which ocular adduction prompts eyelid moving or eyeball retraction. The most popular hypothesis for TOS is congenital miswiring, as

Assessing facial asymmetry before and after early treatment of abnormal head posture caused by ocular problems.

CLINICAL RELEVANCE: Identifying and managing ocular causes of abnormal head posture (AHP) in children is crucial, as early intervention may not only improve visual function but also prevent the development and progression or even resolve facial asymmetry. BACKGROUND: The effect of early AHP treatment on halting the development of facial asymmetry, or even eliminating or decreasing it with age, has not yet been evaluated. METHODS: In this prospective observational study, the quantitative and qualitative facial asymmetry of paediatric patients with superior oblique palsy (n = 8, 53.3%) and Dua
