A 62-year-old female was admitted to our clinic in October 1999 with a 6-year history of hearing loss, tinnitus and otorrhea in the right ear. Physical examination showed a polypoid mass arising from the right middle ear and extending into the external auditory canal. Serohemorrhagic otorrhea was also observed. Serious hemorrhage occurred while taking a biopsy. The histopathologic diagnosis was a glomangioma. MRI demonstrated a hypervascular mass (10 x 15 mm(2)) completely filling the right middle ear. Preoperative embolization of the ascending pharyngeal artery was carried out. The tumor was …

Normal ears come in a wide variety of sizes and shapes, likely because of the impact of racial and familial characteristics (phenotypes), the complexity of the embryologic development (malformations), and the potential for intrauterine factors such as oligohydramnios or intrauterine compression to produce distinct physical characteristics in normally formed tissues (deformations). This article will provide an overview of the embryologic origins of the external ear. Relevant terminology used to describe the ear will be reviewed, along with objective guidelines for the assessment of ear size, sh…

The inner ear, one of the most complex organs, contains within its bony shell three sensory systems, the evolutionary oldest gravity receptor system, the three semicircular canals for the detection of angular acceleration, and the auditory system--unrivaled in sensitivity and frequency discrimination. All three systems are susceptible to a host of afflictions affecting the quality of life for all of us. In the first part of this review we present an introduction to the milestones of inner ear research to pave the way for understanding the complexities of a proteomics approach to the ear. Minut…

Osteomas are benign pedunculated tumours of the lamellar bone, which commonly originates from paranasal sinuses. Within the temporal bone they are seen commonly in the external ear canal. Osteomas originating from the middle ear are very rare. There are only 12 cases reported in the medical literature up to now. Five of those cases caused conductive hearing loss and the others were asymptomatic and diagnosed incidentally.

Another case of actinomycosis of the ear is described in a nine-year-old boy, drawing attention to the increasing incidence of diseases of the ear considered to be rare. The patient presented with the clinical signs of chronic purulent otitis media, not responding to conservative treatment. After the diagnosis of actinomycosis was established the patient was treated by surgery and long-term antibiotic medication, responding favourably.

The inflammatory diseases of external and middle ear are one of the commonest conditions encountered by the pediatric physician. Inner ear inflammations are less common and need special and urgent attention. Special management in each case requires detailed history, examination, necessary investigations and appropriate referral to otolaryngologist when necessary. The article is aimed to help formulate a plan in managing the inflammatory conditions of ear. Otalgia constitutes the most prominent of the symptoms in external and middle ear inflammations whereas vertigo, tinnitus and sensory hearin…

Adenomas, although the most common benign tumors in the middle ear, are of low incidence. They may arise in a previously normal adult ear, causing a conductive deafness and later an aural discharge. Histologic definition may be difficult in these tumors, which are assumed to arise from middle ear epithelium. Fairly extensive surgery may be required for their eradication. Two cases are described.

The recent advances in MRI of the temporal bone brought about by the availability of intravenous contrast agents sensitive to the disruption of blood-brain barrier have further expanded the role of MRI in the evaluation of patients with vestibulocochlear symptoms. Not only can MRI be used to diagnose vestibular schwannomas (and other intracanalicular and cerebellopontine angle lesions mimicking it) with a high degree of accuracy, but it can now identify a variety of inflammatory and neoplastic processes of the membranous labyrinth which, in the past, were only made presumptively and occasional…

Management of melanoma of the external ear is controversial. Thirty-one patients treated for this disease were identified at our institution between January 1, 1974 and December 31, 1989. Follow-up was an average of 7.12 years (range 1-15). Local therapy performed included 16 wedge resections, 3 split thickness skin grafts after excision to the perichondrium, 10 partial and 2 total amputations. There were two local recurrences, four metachronous cervical metastases, and four distant metastases. Three elective and five therapeutic neck dissections were performed. Survivors at 1, 5, and 10 years…

The authors describe the circumstances for detection of an AIDS lymphomatous complication revealed by an otological picture in one patient. The lymphomatous pathology not related to HIV is reviewed. The particularity of these lymphomas, when they are related to HIV, is their frequent initial extra-ganglionic location, their high grade histological type and their poor prognosis. The authors emphasize the need to perform a biopsy rapidly when confronted with any external otitis resistant to general and local treatments.

The course of middle ear injuries secondary to blast effect was evaluated in 147 soldier-patients (210 ears) during the years 1967-1986. All perforations were in the pars tensa of the tympanic membrane. Ossicular chain interruption (11 ears) was associated mainly with dislocation of the incudo-stapedial joint. Mixed-type hearing loss was most prevalent (37%). Spontaneous closure occurred in 155 ears (73.8%), most of them (65.5%) within a 3-month period. Seven ears (3.3%) developed simple chronic suppurative otitis media; in 10 (4.8%), an invasive cholesteatoma developed, and in 6 others (2.8%)…

Anesthesia of the ear canal is produced by injection of lidocaine hydrochloride into the skin of the lateral external ear canal. Ear canal, tympanic membrane, and middle ear surgeries are performed with this injection. The fluid found in the middle ear during tympanotomy was collected and analyzed. The percentage of lidocaine in the fluid was calculated by an enzyme immunoassay technique. Fifteen surgical cases were undertaken in which perilymphatic fluid in the middle ear would not be suspected, such as tympanotomy for otosclerosis. Lidocaine was found in all middle ears in which there was su…

Cholesteatoma of the middle ear is bilateral in 10% of cases. Depending upon the ethnical characteristics or socio-economic levels, the frequency of cholesteatoma for a normal population varies from 1 in a 1,000 to 1 in 10,000. So, it becomes evident that the controlateral ear of cholesteatoma has to be considered at risk. The chances of seeing a cholesteatoma develop and particularly of observing the evolutionary stages are multiplied by 100 or 1,000! This report concerns the results of systematic examination of controlateral ear of 200 consecutive cholesteatomas, using binocular microscope a…

147 soldiers, in whom 210 ears were blast-injured, were studied retrospectively during 1967-1986. The nature of eardrum perforations, hearing impairment, complications, changes over the course of time and different modes of treatment were reviewed. In 92.8 of the ears the perforation was up to half of the eardrum surface, most significantly of the lower part of the drum. In 75% the perforation closed spontaneously: in 40% within a month, in 59% within 2 months and in 73% within a year. Most had impaired hearing, of whom 74% had sensorineural or mixed hearing loss; 60% complained of tinnitus an…

Malformations of the ossicles in 52 ears of 48 patients were analyzed on the basis of surgical findings. The external meatus of each of these ears was patent. The long process of the incus and the stapes are frequently involved in this series of observations. There are distinct groups of ossicular anomalies, in each of which patients show similar degrees of hearing loss and ossicular findings. These include spindle handle, malleus bar, and dislocated annulus. Spindle handle gets its name from the shape of the malleus handle. There is disconnection between the long process of the incus and the …

Haemangioma is an extremely rare tumour of the middle ear. A case of a 60-year old woman patient who complained of a hearing loss for about a year, is reported. Microotoscopy showed a blue pulsating mass behind the tympanic membrane. Angiographic and computer-tomographic investigations led to the suspicion of a glomus tympanicum tumour that was removed bitemporally. The histological preparation showed a capillary haemangioma. Only few cases have been reported in the literature of the last few decades. The symptoms may be loss of hearing, pulsating tinnitus, bleeding, pain and, in extreme cases…
