Pulse Index Continuous Cardiac Output-Guided Hemodynamic Therapy in Living Donor Kidney Transplantation: A Prospective Cohort Study.

BACKGROUND: Central venous pressure (CVP)-guided fluid therapy remains common during kidney transplantation, although CVP is a static marker with limited ability to predict fluid responsiveness. This prospective observational cohort study evaluated whether pulse index continuous cardiac output-guided goal-directed hemodynamic therapy (GDHT), using dynamic and volumetric hemodynamic variables, was associated with improved early graft function compared with conventional CVP-guided management in living donor kidney transplantation. METHODS: Eighty living donor kidney transplant recipients at Cho

Musculoskeletal tissue engineering: Adipose derived stromal cell implementation for the treatment of osteoarthritis.

Osteoarthritis (OA) is a progressive degenerative joint disease which results in chronic degeneration of articular cartilage and sclerosis of bone. While tendons and ligaments may heal to a limited extent, articular cartilage has poor intrinsic regenerative potential, and critical-sized bone defects and pathological fractures cannot regenerate spontaneously. OA represents a significant burden of disease globally, affecting 240 million people in the world. The objective of tissue engineering is to recapitulate the natural healing cascade and developmental process by transplanting stromal and pr

Hepatitis C Virus Treatment as Prevention: Challenges and Opportunities in Men Who Have Sex With Men.

Since 2002, a global epidemic of acute hepatitis C virus (HCV) infection has been noted in men who have sex with men (MSM). Transmission of HCV, particularly in the context of traumatic sex practices that increase the risk of blood-blood contacts (eg, anal sex and fisting), was initially found in human immunodeficiency virus (HIV)-coinfected and more recently in HIV-uninfected MSM, especially those receiving pre-exposure prophylaxis (PrEP). Early HCV treatment with all-oral direct-acting antiviral combination therapy has been associated with very high HCV cure rates of up to 100%. Indeed, imme

Next-generation pharmacological strategies for Parkinson's disease: a systematic literature review of symptom management and disease modification.

Parkinson disease is an ongoing neurodegenerative disorder, and both motor and non-motor symptoms are involved with the existing therapies having minimal effects on the disease but only symptom alleviation. The goal of this systematic literature review was to generalize up-to-date evidence on the next-generation pharmacological strategies to manage the symptoms and the disease in a patient with Parkinson disease. The review was based on PRISMA 2020 and included the analysis of peer-reviewed studies of human research published in 2015-2026 and retrieved in the key databases, such as PubMed, Sco

Diagnosis, Prevention, Management, and Prognostication of Delirium in Acute-Care Neurosurgical Patients: A Systematic Scoping Review.

Delirium represents a frequent and serious complication in neurosurgical patients, contributing to worse clinical outcomes and increased healthcare burden, yet evidence specific to this population remains fragmented. This scoping review systematically mapped literature on diagnosis, prevention, management, and prognostication of delirium in adult neurosurgical patients, including both surgical and nonsurgical settings, to identify knowledge gaps and inform future research and practice. Following JBI methodology and PRISMA-ScR guidelines, PubMed, MEDLINE, Scopus, and Web of Science were searche

Rethinking Fluid Resuscitation in Septic Shock: A Phase-Adapted, Endothelium-Sparing Approach to Mitigate Capillary Leak.

Sepsis-induced endothelial dysfunction and the resultant capillary leak syndrome are defining features of septic shock, posing a profound ch … ative stress and nitric oxide dysregulation. We trace the evolution of resuscitation paradigms from early goal-directed therapy to modern conservative approaches, highlighting the limitations of crystalloid-dominant protocols, including intravascular volume depletion despite a positive fluid balance and iatrogenic organ injury. Drawing on evidence from landmark clinical tria …

Immediate versus early urinary catheter removal after gastrectomy under enhanced recovery after surgery protocols: randomized clinical trial.

BACKGROUND: Compliance with enhanced recovery after surgery (ERAS) protocols in gastrectomy, including urinary catheter management, remains poor. This study evaluated the feasibility of immediate urinary catheter removal after radical gastrectomy. METHODS: This was a non-inferiority randomized clinical trial performed at a university-affiliated hospital in China. Patients undergoing radical gastrectomy were randomized in a 1 : 1 ratio to either immediate removal (IR) or early removal (ER) of the urinary catheter. The randomization sequence was computer generated; the investigators and patient

Cardiac output measurements during high-risk Cesarean section using electrical bioreactance or arterial waveform analysis: assessment of agreement.

OBJECTIVE: Maternal hemodynamics change significantly during Cesarean section complicated by massive hemorrhage or severe hypertensive disease. Cardiac output (CO) monitoring aids early, goal-directed hemodynamic therapy. The aim of this study was to record hemodynamic changes observed during Cesarean section in pregnancies at high risk of hemodynamic instability, using invasive (LiDCOrapid™) and non-invasive (NICOM®) devices, and to assess agreement between the two devices in measuring CO. METHODS: Simultaneous intraoperative hemodynamic measurements were taken using the LiDCOrapid and NICOM

Treatment of Kaposi's sarcoma--an update.

Kaposi's sarcoma (KS) is an angioproliferative disease of multifactorial origin arising in different clinic-epidemiologic forms, which show the same histopathological features. It generally starts as a hyperplastic reactive-inflammatory and angiogenic process, which may evolve into monomorphic nodules of KS cells that can be clonal (late-stage lesions) and resemble a true sarcoma. Infection with the human herpesvirus 8, cytokine- and angiogenic factor-induced growth together with an immuno-dysregulated state represent fundamental conditions for the development of this tumor. Several local ther

Common complication of crush injury, but a rare compartment syndrome.

Compartment syndrome (CS) is a common complication of crush injury but it is rare to find bilateral gluteal compartment syndrome (BGCS). Only six cases of BGCS have been reported in the literature. This syndrome has been reported after crush injury, drug overdose, surgical positioning, and vascular surgery. Apart from CS, crush injury is associated with multi-system adverse effects and these patients are at high risk for renal failure and sepsis. CS patients may present with dehydration; coagulation disorders; elevated creatine phosphokinase and myoglobin levels; hyperkalemia and hypocalcaemia

The Effects of Statins, Ezetimibe, PCSK9-Inhibitors, Inclisiran, and Icosapent Ethyl on Platelet Function.

This review aims to examine the complex interaction between dyslipidemia, platelet function, and related drug treatments. In particular, the manuscript provides an overview of the effects of major hypolipidemic drugs on platelet function. Indeed, growing evidence supports the view that statins, ezetimibe, PCSK9 inhibitors, inclisiran, and icosapent ethyl also act as antithrombotics. It is known that platelets play a key role not only in the acute phase of coronary syndromes but also in the early phase of atherosclerotic plaque formation. The goal of cholesterol-lowering therapy is to reduce ca

Outcomes in Fournier's Gangrene Using Skin and Soft Tissue Sparing Flap Preservation Surgery for Wound Closure: An Alternative Approach to Wide Radical Debridement.

INTRODUCTION: Fournier's gangrene (FG) remains a forbidding necrotizing soft tissue infection (NSTI) that necessitates early recognition, prompt surgical excision, and goal-directed antibiotic therapy. Traditionally, surgical management has included wide radical excision for sepsis control, but this management often leaves large, morbid wounds that require complex wound coverage, prolonged hospitalizations, and/or delayed healing. OBJECTIVE: The purpose of this case series is to report the outcomes of FG using a surrogate approach of concurrent debridement of spared skin and soft tissue, nega

Effect of estrogen replacement therapy on parathyroid hormone secretion in elderly postmenopausal women.

OBJECTIVE: Women undergo two phases of involutional bone loss that have opposing effects on parathyroid hormone (PTH) secretion. During the early phase, the loss of the direct restraining effect of estrogen on bone resorption causes an outflow of skeletal calcium into the extracellular fluid. This causes a compensatory decrease in PTH secretion. In the late phase, loss of extraskeletal effects of estrogen (on intestinal and renal calcium handling) leads to increases in whole body losses of calcium and a compensatory increase in PTH secretion. Moreover, long-term estrogen replacement therapy (E
