An 85-year-old man had a bilateral periorbital rash and conjunctivitis leading to lower eyelid ectropion and epiphora within 6 weeks of treatment with erlotinib (Tarceva, Genentech, Inc., San Francisco, CA, and OSI Pharmaceuticals, Melville, NY), a second-line antineoplastic agent. The treatment was discontinued secondary to toxicity, and the periorbital rash completely resolved within 6 weeks of cessation of the drug. We report 4 cases of Harlequin ichthyosis, which is a rare and severe congenital ichthyosis involving the face. Facial appearance consists in severe ectropion, conjonctival edema, eclabium, flattened ears, broadened nose and large, thick, plate-like skin scales. Recent advances in neonatal care have been made, such as retinoid therapy, and have led to an increased survival rate. Early surgical correction of ectropion may be required because of ocular complications.

Among the major disorders of the lower eyelid due to peripheral facial paralysis are lagophthalmos, eyelid ptosis and ectropion, with or without epiphora. There are several surgical techniques for correcting ectropion and lower eyelid ptosis. This article describes a modification of the classic technique of suspension using tendons, which consists of anchoring the tendon to the frontal apophysis of the maxillary bone and external orbital bone with suture screw anchors. Using the described technique, we obtained significant improvement of epiphora.

Barber Say syndrome (BSS) is a rare ectodermal dysplasia with neonatal onset characterized by congenital generalized hypertrichosis, atrophic skin, ectropion and macrostomia. A literature review showed less than 20 previously reported cases of Barber Say syndrome. This presentation reports a one day old female with syndrome face, low hairline, coarse face, macrostomia, thin upper lip, bilateral ectropion and hypertelorism, hypertrichosis, senile skin appearance, hypoplastic nipples and one area of mild skin atrophy. These findings are consistent with BSS.

Fifty patients (55 incisions) requiring exploration for repair of zygomatico-orbital trauma were admitted to a prospective study using a uniform and standardised technique for the approach to the orbital rim and floor--the lower blepharoplasty incision combined with a post-orbicularis dissection. A post-operative 'Frost suture' was used in the majority of cases. The incidence of post-operative ectropion is compared to the incidence noted in a previous retrospective study (Pospisil & Fernando, 1984) and factors relating to the occurrence of ectropion are discussed.
