Importance of the Wrist Extensor Muscle Training: Two Cases of Elbow Flexorplasty following Traumatic Brachial Plexus Injuries.

The modified Steindler procedure-a reconstructive surgery used to restore elbow flexion following upper brachial plexus injuries-involves shifting the origins of the muscle groups responsible for wrist flexion and forearm pronation originating from the medial epicondyle to the proximal direction to be used as flexors of the elbow. In the postoperative rehabilitation, we focused on strengthening not only the transferred muscle but also the wrist extensor muscles as antagonist muscles. After reconstruction surgery for elbow flexion via the modified Steindler procedure for traumatic brachial plex

Epidemiology of shoulder and elbow injuries in National Collegiate Athletic Association wrestlers, 2009-2010 through 2013-2014.

OBJECTIVES: Examination of the incidence of shoulder and elbow injuries in the collegiate wrestling population is limited. Therefore, we sought to determine the incidence of shoulder and elbow injuries in wrestlers competing in the National Collegiate Athletic Association (NCAA), and investigate the risk factors involved. METHODS: All shoulder and elbow injuries in wrestlers from the 2009-2010 through 2013-2014 academic years in the NCAA Injury Surveillance Program database were extracted. The incidence of different injuries, sports, activity, time-in-game, competition status, and injury char…

Avulsion fracture of the supinator crest of the proximal ulna in the context of elbow joint injuries.

PURPOSE: While performing CT examinations of the elbow, we frequently observed a previously undescribed fracture fragment of the supinator crest of the ulna. According to the anatomy of the lateral collateral ligament complex, this fracture might be an avulsion fracture of the annular ligament and/or the lateral ulnar collateral ligament. The aim of this study was to further characterize these fragments and document their associated injuries. METHODS: Retrospective evaluation of CT scans of the elbow was performed. Conventional X-ray and CT diagnoses were used to systematically document any a

Restoration of elbow flexion by transfer of the phrenic nerve to musculocutaneous nerve after brachial plexus injuries.

Traumatic brachial plexus injuries are a devastating injury that results in partial or total denervation of the muscles of the upper extremity. Treatment options that include neurolysis, nerve grafting, or neurotization (nerve transfer) has become an important procedure in the restoration of function in patients with irreparable preganglionic lesions. Restoration of elbow flexion is the primary goal in treating patients with severe brachial plexus injuries. Nerve transfers are used when spinal roots are avulsed, and proximal stumps are not available. In the present study, we analyze the result

The long thoracic nerve as a donor for elbow extension reconstruction in traumatic adult brachial plexus injuries.

BACKGROUND AND AIMS: Elbow extension reconstruction in traumatic adult complete or extended partial brachial plexus injuries (BPIs) is challenging owing to the limited availability of reliable uninjured donor nerves. The intraplexal long thoracic nerve (LTn), offers an underused donor option is selected circumstances. Its segmental innervation of serratus anterior allows selective distal harvesting with minimal functional loss. However, its use has been described only in small, heterogeneous series with limited outcome data. We aimed to evaluate the clinical utility and outcomes of LTn transfe

Effect of indomethacin prophylaxis on heterotopic ossification after surgical fixation of elbow injuries: a randomized controlled trial.

BACKGROUND: Heterotopic ossification (HO) is a recognized complication following operative fixation of elbow trauma. While indomethacin prophylaxis is well established in hip and acetabular surgery, evidence for its role in elbow injuries remains limited. This randomized controlled trial evaluated whether oral indomethacin reduces the incidence and severity of HO after surgical treatment of acute elbow fractures and dislocations. METHODS: Eighty-six adults with acute elbow injuries requiring surgical fixation were randomized 1:1 to receive either oral indomethacin (25 mg 3 times daily for thr…

Neck range of motion prognostic factors in association with shoulder and elbow injuries in professional baseball pitchers.

BACKGROUND: The authors observed an association between cervical spine mobility and arm injury risk in baseball players; however, there is a need to assess the generalizability of cervical measurement data. Assessing the downstream associations of cervical dysfunction on shoulder and elbow injuries can inform clinical interventions to help reduce future arm injuries. The purpose of this study was to assess the generalizability of neck range of motion measures as arm injury prognostic factors in professional baseball pitchers. METHODS: A prospective cohort of professional baseball pitchers in …

Telehealth-Enabled In-Home Elbow Rehabilitation for Brachial Plexus Injuries Using Deep-Reinforcement-Learning-Assisted Telepresence Robots.

Due to damage to the network of nerves that regulate the muscles and feeling in the shoulder, arm, and forearm, brachial plexus injuries (BPIs) are known to significantly reduce the function and quality of life of affected persons. According to the World Health Organization (WHO), a considerable share of global disability-adjusted life years (DALYs) is attributable to upper limb injuries, including BPIs. Telehealth can improve access concerns for patients with BPIs, particularly in lower-middle-income nations. This study used deep reinforcement learning (DRL)-assisted telepresence robots, spec

Imaging Evaluation of Medial and Lateral Elbow Pain: Acute and Chronic Tendon Injuries of the Humeral Epicondyles.

Medial and lateral elbow pain are often due to degenerative tendinosis and less commonly due to trauma. The involved structures include the flexor-pronator tendon origin in medial-sided pain and the extensor tendon origin in lateral-sided pain. Multimodality imaging is often obtained to verify the clinically suspected diagnosis, evaluate the extent of injury, and guide treatment decisions. Image-guided procedures can provide symptom relief to support physical therapy and also induce tendon healing. Surgical debridement and repair are typically performed in refractory cases, resulting in good t

Phrenic to musculocutaneous nerve transfer for traumatic brachial plexus injuries: analyzing respiratory effects on elbow flexion control.

OBJECTIVE: In this study, the authors sought to identify the relationship between breathing and elbow flexion in patients with a traumatic brachial plexus injury (TBPI) who undergo a phrenic nerve (PN) transfer to restore biceps flexion. More specifically, the authors studied whether biceps strength and the maximal range of active elbow flexion differ between full inspiration and expiration, and whether electromyography (EMG) activity in the biceps differs between forced maximum breathing during muscular rest, normal breathing during rest, and at maximal biceps contraction. All these variables

Elbow Block Test: A Clinical Sign of C7 Nerve Injury in Residual Brachial Plexus Birth Injury.

PURPOSE: Concomitant limitation of shoulder abduction and external rotation is commonly seen in residual upper plexus brachial plexus birth injury. However, a subset of patients presents with preserved external rotation but diminished abduction, suggesting a disproportionate limitation. In these patients, application of an elbow extension splint improves abduction - a clinical observation termed the elbow block test (EBT). This study evaluates the clinical significance, etiological basis and pattern of brachial plexus involvement associated with EBT. METHODS: A total of 155 children (mean 5.4

A Peculiar Case of Open Complex Elbow Injury with Critical Bone Loss, Triceps Reinsertion, and Scar Tissue might Provide for Elbow Stability?

BACKGROUND: Complex elbow injuries (CEIs) are severe and rare lesions, difficult to treat correctly due to the different patterns of clinical presentations. Standard methods cannot often be applied. The main goals of the treatment are performing a stable osteosynthesis of all fractures, obtaining a concentric and stable reduction of the elbow by repairing the soft tissue constraint lesions, and allowing early motion. Since the introduction of virtual reality (VR) approaches in clinical practice, three-dimensional (3D) computed tomography (CT) and 3D printing have revolutionised orthopaedic sur…

Objective Return-to-Sport (RTS) Decision Frameworks for Adolescent Shoulder and Elbow Injuries: Tests, Thresholds and Exercise Targets.

BACKGROUND: Adolescent shoulder and elbow injuries are increasing due to sports specialization and year-round training, yet return-to-sport decisions typically rely on adult-derived heuristics that fail to address the unique physiology, growth plate vulnerability, and recovery needs of developing athletes aged 10-19 years. OBJECTIVE: This review proposes a novel, objective three-pillar RTS framework integrating: (1) foundational tissue capacity, (2) dynamic movement quality and neuromuscular control, and (3) sport-specific load tolerance. We synthesize current evidence to establish provisiona…
