The study and correction of human gait by electrical stimulation.

To gain a better understanding of the functions that the calf and vastus muscles perform in the human walking gait the author systematically increased the contractions of these muscles separately and in combination by applying Functional Electrical Stimulation (FES) to them, during walking tests performed by a subject with nonpathological gait, and a patient with a hemiplegic gait. A four-channel stimulator was used with foot switch activated control systems, which accurately sequenced the FES pulses and timed them in relation to the footswitch contacts. In normal gait FES applied to the calf

Diaphragm pacing by electrical stimulation of the phrenic nerve.

Sophisticated techniques for electrical stimulation of excitable tissue to treat neuromuscular disorders rationally have been developed over the past 3 decades. A historical review shows that electricity has been applied to the phrenic nerves to activate the diaphragm for some 200 years. Of the contemporary methods for stimulating the phrenic nerve in cases of ventilatory insufficiency, the authors prefer stimulation of the phrenic nerve in the thorax using a platinum ribbon electrode placed behind the nerve and an attached subcutaneously implanted radiofrequency (RF) receiver inductively coup

Effect of electrical stimulation on spasticity in hemiparetic patients.

Cyclical electrical stimulation has been applied to 10 hemiparetic patients with clinical signs of knee joint spasticity. The programme included 30 minutes of stimulation to the hamstrings followed by another 30 minutes of stimulation to the hamstrings and quadriceps. None of the patients experienced increased spasticity. The reduction in spasticity ranged from none to substantial with some other beneficial side-effects. No conclusions could be drawn as to whether hamstring stimulation is preferred to combined stimulation or to quadriceps stimulation alone. It is suggested that small portable

State of the art of functional electrical stimulation in France.

After summarizing the various physiological and functional effects of surface electrical stimulation, the authors review its main applications, including chronic-use or training orthoses, neuromuscular facilitation and automatic cyclical stimulation. Each of the therapeutic principles is illustrated by discussion of an example, with particular emphasis being placed on the practical methods of application that determine the success of the programme.

The effects of varying current levels of electrical stimulation.

An effort has been made to find an experimental delayed union of a long bone that could be used to evaluate the osteogenic effect of different current strengths. It is important that the optimum current strength be determined. Any such model should be able to produce a difference in new bone formation with an active and an inactive stimulator, particularly one using a 20 microA direct current. Attempts to produce a nonunion model in dogs were unsatisfactory, possibly because the defect was too small and surrounded by normal bone, and excessive movement occurred at the cathode plate. The optimu

Transcutaneous electrical nerve stimulator for stress incontinence.

It has been shown that some patients with stress incontinence after prostatectomy may have significant improvement with electrical anal stimulation. The use of a transcutaneous stimulator is the easiest and least invasive method to try initially. Results may be quite satisfying whether the mechanism is physiologic or placebo. This is the first report of the use of transcutaneous electrical nerve stimulation in patients with stress incontinence.

Grafting and electrical stimulation of the auditory nerve in cats.

The effect of implanting wire electrodes into the cochlea is limited by the deficit of nerve fibers in most diseases of the cochlea. Therefo … ory cortex, were compaired to potentials evoked by auditory stimulation. The answers to acoustic or electric stimulation showed no significant difference.

Electrode placements for transcutaneous electrical nerve stimulation.

Even though transcutaneous electrical nerve stimulation is gaining increasing acceptance as a noninvasive method of pain modulation, many professionals have not been adequately instructed in the many techniques involved in electrode placement. Alternative methods are presented that can be used when more commonly used methods fail to obtain satisfactory results. Anatomical and physiological considerations for determining appropriate electrode placement sites are stressed.

Uses of percutaneous electrical stimulation of the nervous system.

Temporary percutaneous electrical stimulation of peripheral nerves and the spinal cord is a valuable screening technique. This approach will provide a much higher success rate for identifying those patients suitable for a permanently implanted stimulation system. The system appears to be the treatment of choice for chronic, intractable pain caused by nerve injury. The author presents his surgical procedures for stimulation of peripheral nerve and spinal cord. A summary of clinical results is discussed.

Reptilase clot retraction induced by electrical stimulation.

Retraction of platelet rich plasma clotted by reptilase is induced by electrical stimulation. Optimal retraction is obtained by stimuli, applied for more than 4 min, with the following characteristics: intensity = 150 volts, duration = 50 msec each, frequency = 10/sec. Electrically induced reptilase clot retraction is shown to be inhibited by EDTA, EGTA, methyl-xanthines, PGE1, acetylsalicylic acid, indomethacin, but not by apyrase or by phosphoenolpyruvate-pyruvate kinase and MgCl2. The results indicate that electrical stimulation induces retraction of PRP clotted by reptilase by triggering o

Comparing stimulation-induced pain during percutaneous (intramuscular) and transcutaneous neuromuscular electric stimulation for treating shoulder subluxation in hemiplegia.

OBJECTIVE: To determine whether percutaneous (intramuscular) neuromuscular electric stimulation (perc-NMES) is less painful than transcutaneous neuromuscular electric stimulation (trans-NMES) for treating shoulder subluxation in hemiplegia. DESIGN: Double-blind, crossover trial. SETTING: University-affiliated tertiary care hospital. PARTICIPANTS: A convenience sample of 10 hemiplegic subjects with at least 1 fingerbreadth of glenohumeral subluxation. INTERVENTIONS: All subjects received 3 randomly ordered pairs of perc-NMES and trans-NMES to the supraspinatus and posterior deltoid muscles …
