Systemic administration of capsaicin has been shown to be antinociceptive in a number of acute nociceptive models. We present here two models of prolonged hyperalgesia in the rat induced by intra-articular injection of uric acid or Freund's complete adjuvant into the knee. The time course of hyperalgesia lasts up to 48 h with uric acid and 5 to 10 days with Freund's adjuvant. The hyperalgesia was maximal in both models after 18 h. One hundred patients were studied prospectively to assess the incidence of spasm of the choledochoduodenal sphincter during biliary tract surgery performed under a fentanyl-supplemented anesthetic technique. The incidence of failure of passage of contrast medium into the duodenum was 3%. In each of the three patients with cholangiographic evidence of sphincter spasm, contrast medium flowed freely into the duodenum after the intravenous administration of 2 mg of glucagon. Malignant hyperthermia (MH) is a pharmacogenetic disorder of skeletal muscle. In humans, MH is inherited in an autosomal dominant fashion; in swine, the principal model for MH, it is in a recessive fashion. Those with MH susceptibility usually are asymptomatic except in the presence of certain "triggering" anaesthetic agents such as isoflurane, enflurane and the muscle relaxant succinylcholine. Ethanol, diethylether, halothane and enflurane inhibited the veratridine-dependent uptake of 24Na by synaptosomes isolated from rodent brain.
