Short-term outcomes for preterm infants with surgical necrotizing enterocolitis.

OBJECTIVE: To characterize the population and short-term outcomes in preterm infants with surgical necrotizing enterocolitis (NEC). STUDY DESIGN: Preterm infants with surgical NEC were identified from 27 hospitals over 3 years using the Children's Hospitals Neonatal Database; infants with gastroschisis, volvulus, major congenital heart disease or surgical NEC that resolved prior to referral were excluded. Patient characteristics and pre-discharge morbidities were stratified by gestational age (<28 vs 28(0/7) to 36(6/7) weeks' gestation). RESULT: Of the 753 eligible infants, 60% were born at

Association of cord blood digitalis-like factor and necrotizing enterocolitis.

OBJECTIVE: Endogenous digoxin-like factor (EDLF) has been linked to vasoconstriction, altered membrane transport, and apoptosis. Our objective was to determine whether increased EDLF in the cord sera of preterm infants was associated with an increased incidence of necrotizing enterocolitis (NEC). STUDY DESIGN: Cord sera from pregnant women enrolled in a randomized trial of MgSO4 for fetal neuroprotection were analyzed for EDLF using a red cell Rb(+) uptake assay in which the inhibition of sodium pump-mediated Rb(+) transport was used as a functional assay of EDLF. Specimens were assayed blind

Transfusion associated necrotizing enterocolitis: a meta-analysis of observational data.

BACKGROUND AND OBJECTIVES: Several studies have reported the possibility of an association between recent exposure to transfusion and development of necrotizing enterocolitis (NEC). Our objective was to systematically review and meta-analyze the association between transfusion and NEC (TANEC), identify predictors of TANEC, and the assess impact of TANEC on outcomes. METHODS: Medline, Embase, CINAHL, and bibliographies of identified articles were searched for studies assessing association with recent (within 48 hours) exposure to transfusion and NEC. Two reviewers independently collected data

Core temperature falls during laparotomy in infants with necrotizing enterocolitis.

AIM OF THE STUDY: Intraoperative hypothermia may have a detrimental clinical effect. Preterm infants undergoing laparotomy for necrotizing enterocolitis (NEC) are particularly at risk. We investigated the relationship between intraoperative temperature and morbidity and outcome in infants with NEC. METHODS: A review of all laparotomies for NEC (n = 82, 69 infants) performed between Jan 2008 and Jan 2011 in our institution was done. Relationships between intraoperative temperature and intra- and postoperative fluid and blood product requirements, postoperative clinical status (sequential organ

Percutaneous transluminal angioplasty in recurrent necrotizing enterocolitis--a case report.

A 35-week gestation male neonate had 3 episodes of necrotizing enterocolitis (NEC), the first of which was managed surgically. Two further episodes of NEC were treated by medical management. Angiography then identified a stricture at the origin of the superior mesenteric artery. Percutaneous transluminal angioplasty successfully dilated the stenosis, and no further episodes of NEC occurred. Eight years later, the child exhibits a mild spastic diplegia and is thriving with no gastrointestinal disturbances. Angioplasty is technically feasible in the neonate and may be a treatment option in recur

[Necrotizing enterocolitis and bacterial translocation: role of minimal enteral nutrition].

INTRODUCTION: Both necrotizing enterocolitis (NEC) and bacterial translocation (BT) have in common that bacterial overgrowth, a decrease in immunity and intestinal mucosal damage, followed by a barrier failure, can act as trigger factors. The main objectives in NEC treatment are to reduce mortality due to sepsis and to promote feeding tolerance. To achieve that, Minimal Enteral Nutrition (MEN) (less than 25% of the calories provided by enteral route) is a more and more used technique in newborns who receive Parenteral Nutrition (PN) to slow down fasting related villi atrophy and to attenuate i

Laparoscopy assists surgical decision making in infants with necrotizing enterocolitis.

BACKGROUND: Lack of specific indications for surgical intervention is not uncommon in critically ill infants with necrotizing enterocolitis (NEC). The authors hypothesized that knowledge of the extent of intestinal disease would assist in deciding the operative treatment of choice. The authors report that initial experience with laparoscopy in infants with NEC. METHODS: Prospective evaluation was done of 11 infants with a clinical diagnosis of NEC who were deteriorating despite maximal medical therapy. All underwent laparoscopy on the intensive care unit (ICU). RESULTS: Median gestational ag

Intestinal transplantation for short gut syndrome attributable to necrotizing enterocolitis.

BACKGROUND: Necrotizing enterocolitis (NEC) is a life-threatening condition of the neonatal age, which frequently requires surgical intervention. After extensive bowel resection, a small proportion of these patients may develop chronic short gut syndrome (SGS) and require chronic total parenteral nutrition (TPN) use. Intestinal transplantation has been performed in these patients as a life-saving option. This study reviews our experience with intestinal transplantation for SGS attributable to NEC emphasizing the mode of presentation, natural history, timing, and outcome. METHODS: A retrospect

Complications after surgical intervention for necrotizing enterocolitis: a multicenter review.

Necrotizing enterocolitis (NEC) is a serious condition affecting predominantly the premature infant. The purpose of this study is to report a multicenter experience of complications in 252 infants requiring surgical therapy for NEC. Data from eight institutions for the years 1980 through 1990 were collected and analyzed for infants undergoing surgical therapy for NEC. Records were reviewed for gestational age, birth weight, age at operation, indications for operation, degree of intestinal involvement, operation(s) performed, complications, and 30-day mortality rates. A total of 264 infants und

[Congenital tuberculosis. Apropos of a case disclosed after necrotizing enterocolitis].

The authors report on the observation of a premature infant (gestational age 31 weeks, birth weight 1440 g) who experienced a necrotizing enterocolitis and a congenital tuberculosis. Congenital tuberculosis is a rare infection; transmission of Koch's bacilli is achieved through hematogenous route or more frequently through inhalation or ingestion of infected amniotic fluid. The diagnosis was done when miliary patterns were found on the mother's chest X-ray. At the same time, the newborn presented with a chronic lung disease, a poor clinical status with biological signs of inflammation. Usual a

Intestinal post-ischemic reperfusion injury: studies with neonatal necrotizing enterocolitis.

In the feline intestine studies have implicated superoxide (O.-) and other oxygen derived free radicals as initiators of injury as measured by increased capillary permeability during the reperfusion period. Biochemical mechanisms of this free radical generation include: xanthine oxidase dependent O.- production, hydrogen peroxide (H2O2) formation by superoxide dismutase (SOD), hydroxyl radical (OH-) production via the Haber-Weiss reaction, and lipid radical formation from membrane peroxidation. Pathological consequences of these events include inflammatory neutrophil infiltration, damage to th

Necrotizing enterocolitis in full-term infants. A case-control study.

We performed a case-control investigation of 43 full-term infants with necrotizing enterocolitis (NEC) to identify possible risk factors and unique features of the disorder in the more mature infant. Two control groups were used. The first consisted of "Работает" term infants. The second was a group of "sick" term infants who did not develop NEC. The 43 term infants with NEC represented 12.7% of all 338 neonates with NEC. The median age at onset of symptoms was 2 days, and 18 infants developed NEC on the first day of life. Two (4.7%) of the 43 affected term infants died, while 35 (11.9%) of 295

A decreased incidence of necrotizing enterocolitis after prenatal glucocorticoid therapy.

In a large multicentered, collaborative randomized and blinded trial utilizing antenatal corticosteroids, the goals included determining the effectiveness of these agents in accelerating lung maturation, as well as monitoring any short-term or long-term adverse effects of this treatment on the parturient, fetus, and/or infant. More than 100 specific items, pertaining to diagnoses, complications, and outcomes were recorded for the 696 mothers enrolled in the study and their 745 infants. A significantly decreased incidence of necrotizing enterocolitis (P = .002) was found in the infants treated

Mild form of neonatal necrotizing enterocolitis masked by anal fissures.

In three infants with clinically mild necrotizing enterocolitis, heme-positive stools were initially attributed to anal fissures, delaying diagnosis and treatment of the enterocolitis for 4-10 days. Radiographically, pneumatosis intestinalis involved the entire colon and rectum. A causal relation may exist between necrotizing enterocolitis and anal fissure.

Peritoneal drainage under local anesthesia for perforations from necrotizing enterocolitis.

During 1974 and 1975, five newborn infants weighing between 760 and 1600 g developed severe intraabdominal complications of NEC due to necrotic bowel with perforation and peritonitis. Because these tiny neonates were very ill with sepsis and other severe medical problems, no attempt at laparotomy under general anesthesia was carried out. Instead, using local anesthesia, the contaminated peritoneal cavity was drained via a small incision, usually in the right lower quadrant. This permitted evacuation of air, pus, and stool. In all babies there was improvement evident in the abdomen within one w

Does the initial surgery for necrotizing enterocolitis matter? Comparative outcomes for laparotomy vs. peritoneal drain as initial surgery for necrotizing enterocolitis in infants <1000 g birth weight.

PURPOSE: Quantify short-term outcomes associated with initial surgery [laparotomy (LAP) vs. peritoneal drain (PD)] for necrotizing enterocolitis (NEC) in extremely-low-birth-weight (ELBW) infants. METHODS: Using the Children's Hospitals Neonatal Database, we identified ELBW infants <32 weeks' gestation with surgical NEC (sNEC). Unadjusted and multivariable regression analyses were used to estimate the associations between LAP (or PD) and death/short bowel syndrome (SBS) and length of stay (LOS). RESULTS: LAP was the more common initial procedure for sNEC (n = 359/528, 68%). Infants receiving

Emerging Tools and Biomarker Approaches to Understand and Diagnose Necrotizing Enterocolitis.

Necrotizing enterocolitis (NEC) is a devastating gastrointestinal disease primarily affecting preterm infants and is associated with a high mortality and long-term morbidity. Despite decades of research, NEC pathogenesis remains incompletely understood, and early diagnosis is challenging. NEC is a multifactorial disease arising from a complex interplay of factors, including impaired intestinal barrier function, dysregulated immune response, microbial dysbiosis, and a range of antenatal, perinatal, and postnatal exposures. Diagnosis primarily relies on clinical assessment and radiography. This

Promising Treatments and Novel Therapeutic Approaches for the Management of Necrotizing Enterocolitis.

Necrotizing enterocolitis (NEC) is a deadly intestinal disease that most commonly occurs in premature infants. Current treatments are reactive rather than preventative, and include feeding cessation, antibiotics, gastric decompression, and supportive care. In severe NEC, surgical intervention is required for intestinal perforation or failure of medical management. Promising treatment modalities are being explored to prevent NEC and reduce NEC severity, including probiotics, probiotic delivery systems, growth factors, stem cell and small molecule therapeutics, human milk oligosaccharides, and h
