Outcomes of Catheter Ablation in Arrhythmogenic Right Ventricular Cardiomyopathy Without Background Implantable Cardioverter Defibrillator Therapy: A Multicenter International Ventricular Tachycardia Registry.

OBJECTIVES: This study sought to determine the long-term outcomes of catheter ablation (CA) of ventricular tachycardia (VT) in a series of p … yncope (13%). Prior to ablation, 22 patients (69%) failed a mean of 1.3 ± 0.5 antiarrhythmic drugs. Epicardial mapping and ablation was performed as first-line strategy (20 [63%]) or in case of recurrent VT or persistent inducibility after endocardial-only ablation (3 [9%]-surgical epicardial cryoablation in 1 patient). After a mean of 1.6 (range 1 to 3) procedures, …

Catheter ablation of monomorphic ventricular tachycardia.

PURPOSE OF REVIEW: Patients with ventricular tachycardia are subject to frequent recurrences and antiarrhythmic drug therapy has been disapp … sinus rhythm is often possible. Ablation is usually adjunctive therapy to an ICD in these patients. Epicardial mapping and ablation are needed in some patients. SUMMARY: Ablation is a reasonable alternative to antiarrhythmic drug therapy for controlling frequent ventricular tachycardia episodes in many patients. Further technological advances can be anticipated.

Catheter Ablation of Left Ventricular Summit Arrhythmias from Adjacent Anatomic Vantage Points.

Idiopathic ventricular arrhythmias (VA), particularly left ventricular outflow tract (LVOT) VA accounts for up to 10% of all VAs referred for ablative therapy. In addition to being infrequent, its intricate anatomy and its pathophysiology make catheter ablation (CA) of these arrhythmias a challenge even for experts. In this scenario, detailed right ventricular outflow tract as well as LVOT electroanatomic mapping including epicardial mapping are essential. In this article, we will emphasize our approach toward the CA technique used for LVOT VA, particularly IVS and/or LVS VA originating from i…

Mitral isthmus ablation with pulsed field ablation technology through an epicardial approach with the CENTAURI system.

UNLABELLED: Mitral isthmus (MI) ablation is an essential part of persistent atrial fibrillation (AF) ablation, particularly in redo procedur … tive in achieving nerve damage. The ablation catheter was then advanced into the coronary sinus for epicardial mapping, where a gap in the isthmus was identified. PFA delivered through the epicardium successfully achieved bidirectional block with no complications. Hybrid endo-epicardial ablation of mitral isthmus by PFA technology is effective and safe, providing a …

Ventricular Tachycardia in a Patient with Biventricular Noncompaction.

Patients with ventricular noncompaction are susceptible to developing ventricular tachycardia. Commonly, the origin of ventricular tachycardia is endocardial; however, epicardial origins and scar cannot be excluded and should be considered when poor endocardial mapping is present. Other cardiomyopathies, such as arrhythmogenic right ventricular cardiomyopathy, can coexist with ventricular noncompaction and should be excluded in these patients.

Electrocardiographic characteristics and mapping approach of ventricular arrhythmias originating from the left ventricular summit.

The left ventricular summit is the most common site of idiopathic epicardial ventricular tachycardia (VT). We report a case of a 48-yr-old lady who presented with VT of RBBB configuration, inferior axis and delayed initial activation. During electrophysiological study, local activation in the distal great cardiac vein (GCV) preceded surface QRS by 56 ms whereas mapping in the aortic sinuses and left ventricular outflow tract endocardially revealed late activation. Ablation in the distal GCV with an irrigated catheter successfully terminated the tachycardia. The 12‑lead electrocardiogram is an

Brugada syndrome genetics is associated with phenotype severity.

AIMS: Brugada syndrome (BrS) is associated with an increased risk of sudden cardiac death due to ventricular tachycardia/fibrillation (VT/VF) in young, otherwise healthy individuals. Despite SCN5A being the most commonly known mutated gene to date, the genotype-phenotype relationship is poorly understood and remains uncertain. This study aimed to elucidate the genotype-phenotype correlation in BrS. METHODS AND RESULTS: Brugada syndrome probands deemed at high risk of future arrhythmic events underwent genetic testing and phenotype characterization by the means of epicardial arrhythmogenic sub

[3-dimensional reentry in paroxysmal ventricular tachycardias: the results of electrophysiological mapping].

The mechanism of sustained ventricular tachycardias with a focal type of activation initiated by an extrastimulus was studied in isolated rabbit heart ventricle preparations by endocardial and epicardial mapping. Sixty-four electrograms were simultaneously recorded using unipolar electrodes (32 on each side of the preparation). The electrodes were spaced 3 mm apart. Data recording and processing were computerized. Sustained (greater than 0.5 min) tachycardias of a focal type were registered in 6 of 17 experiments. In each case, the three-dimensional macro-reentry mechanisms was involved. The c…

Transvenous ethanol ablation in epicardial localization of ventricular arrhythmias: a case report.

BACKGROUND: An endocardial radiofrequency ablation is a common approach for the treatment of idiopathic ventricular arrhythmia. However, rare cases have been reported in which ventricular arrhythmia could not be ablated from endocardium due to an epicardial origin of the arrhythmia. CASE SUMMARY: In this article, we describe the rarely used, but acceptable approach to terminate ventricular arrhythmias in the summit of the left ventricle. We present a case of a 56-year-old patient with sustained monomorphic premature ventricular complexes, originating from the summit of the left ventricle, tha

Epicardial left ventricular lead placement for cardiac resynchronization therapy: optimal pace site selection with pressure-volume loops.

OBJECTIVES: Patients in heart failure with left bundle branch block benefit from cardiac resynchronization therapy. Usually the left ventricular pacing lead is placed by coronary sinus catheterization; however, this procedure is not always successful, and patients may be referred for surgical epicardial lead placement. The objective of this study was to develop a method to guide epicardial lead placement in cardiac resynchronization therapy. METHODS: Eleven patients in heart failure who were eligible for cardiac resynchronization therapy were referred for surgery because of failed coronary si

Anticoagulation therapy for endocardial/epicardial ablation of infarct-related ventricular tachycardia in a patient with antithrombin deficiency.

UNLABELLED: In patients with a history of antithrombin deficiency, a direct thrombin inhibitor may be considered as an alternative to intraprocedural anticoagulation. A 55-year-old woman with antithrombin deficiency and prior myocardial infarction suffered from electrical storm and appropriate defibrillator shocks. Substrate mapping guided left ventricular endocardial and epicardial ablations were successfully performed. The direct thrombin inhibitor, argatroban, was safely used as intraprocedural anticoagulation therapy with no complications. LEARNING OBJECTIVE: Optimal anticoagulation durin

Effective ablation of atrial tachycardia with an epicardial circuit-insights from endocardial scars sites: a case study.

A previous study reported primary macroreentrant atrial tachycardia (AT) in the left atrium (LA), including the epicardial circuit on a left atrial anterior wall (LAAW) scar, without any prior cardiac intervention (Miyazawa et al. in J Cardiovasc Electrophysiol 2019; 30: 263-264). However, determining the target for terminating macroreentrant ATs is challenging. The mapping revealed a centrifugal pattern but did not fully elucidate the AT circuit. The reentrant mechanism of these ATs was confirmed using entrainment pacing. The earliest excitation site (EES) was traditionally selected as the ab

Cryoablation of medically refractory nodoventricular tachycardia.

Paroxysmal wide QRS tachycardia, based on a nodoventricular accessory connection, is an uncommon arrhythmia. In this report, the endocardial and epicardial mapping and cryoablation of a nodoventricular fiber, documented to participate in medically refractory tachycardia in an 11-year-old boy, are described. Epicardial cryothermia, applied at the earliest site of right ventricular activation, resulted in the abrupt termination of tachycardia. Endocardial cryothermia was subsequently applied in the perinodal region, the presumed site of origin of the nodoventricular fiber. No tachyarrhythmias we…

Focal monopolar pulsed field ablation from within the great cardiac vein for idiopathic premature ventricular contractions after failed radiofrequency ablation.

BACKGROUND: Idiopathic epicardial premature ventricular contractions (PVCs) originating from the left ventricular summit are difficult to eliminate. OBJECTIVE: The purpose of this study was to describe the feasibility and procedural safety of monopolar biphasic focal pulsed field ablation (F-PFA) from within the great cardiac vein (GCV) for treatment of idiopathic epicardial PVCs. METHODS: In 4 pigs, F-PFA (Centauri, CardioFocus) was applied from within the GCV followed by macroscopic gross analysis. In 4 patients with previously failed radiofrequency ablation, electroanatomic mapping was us

Mapping and Ablation of Ventricular Tachycardia in Nonischemic Left Ventricular Cardiomyopathy.

Nonischemic cardiomyopathy incorporates a diverse group of structural heart disease, some of which are genetic in etiology, many of which involve intramural or epicardial substrate for ventricular arrhythmias. Substrate distribution varies according to etiology and has prognostic and procedural implications with regard to mapping and ablation. Multimodality imaging has played an important role in characterization of cardiomyopathy subtypes, as well as in guiding approach for ventricular tachycardia ablation in this challenging patient population. This review will summarize the available data o

Successful Guidewire-Guided Ethanol Ablation for PVCs Originating From Epicardial Anterior Interventricular Groove Adjacent to LAD.

BACKGROUND: Premature ventricular complexes (PVCs) originating from epicardial anterior interventricular groove and closely adjacent to the left anterior descending artery (LAD) are difficult to eliminate and carry high risks of coronary damage or occlusion. We describe a successful guidewire-assisted ethanol ablation in this high -risk patient. CASE SUMMARY: A 30-year-old man who presented with palpitations and high-burden PVCs was admitted to our hospital. Pace mapping performed at the epicardial anterior interventricular groove yielded QRS morphology nearly identical to the native PVCs. Co

Myocardial ischaemia in patients with coronary endothelial dysfunction: insights from body surface ECG mapping and implications for invasive evaluation of chronic chest pain.

AIMS: Coronary endothelial dysfunction (ED), by predisposing to abnormal vasomotion, may cause chest pain in individuals with non-obstructed coronary arteries. The aim of this study was to correlate the magnitude of coronary ED with the presence and extent of inducible myocardial ischaemia using body surface electrocardiogram (ECG) mapping in symptomatic patients. METHODS AND RESULTS: In 30 patients with chest pain and angiographically normal coronary arteries or mild atherosclerosis, we studied endothelium-dependent responses with acetylcholine (ACH) and endothelium-independent function with
