The effects of epidural anaesthesia (EDA, mepivacaine) and EDA in combination with atropine and neostigmine on postoperative intestinal motility were studied in 17 patients undergoing operation for cancer of the rectum or sigmoid colon. Motility was recorded by a volumetric technique. Epidural anaesthesia (EDA) increased motor activity in the small bowel as well as in the left colon and rectum. Phasic motility dominated in the small intestine whereas tonic and segmental contractions were recorded from the large bowel. EDA induced a powerful tonic contraction with a concomitant shortening of th

The article analyses the experience in the treatment of 49 patients with caudal teratomas for which reoperations had to be conducted. The operations were repeated because of diagnostic errors, inadequate surgical approach, and the development of intraoperative complications. Prolonged epidural anaesthesia with concurrent application of trimecaine hydrochloride and morphine was considered the most adequate type of anesthesia. In cases of fistulas draining the caudal teratomas into the rectum, the surgical tactics is determined by the location and size of the fistulas openings. To improve the re

The spinal application of opiates is followed by a long-lasting and strong pain relief. This action is based upon the binding of opiates to specific opiate receptors situated in the substantia gelatinosa of the spinal cord. Two possible approaches exist - intrathecally or epidurally. The intrathecal opiate analgesia is combined with a very high incidence of side effects, so that this way cannot be recommended. The epidural opiate analgesia has proven good results with few side effects in the treatment of postoperative pain, pain of multiple rib fractures and other thoracic trauma or cancer pai

Metastases to the spinal epidural space with compression of the spinal cord or cauda equina are commonly encountered by physicians in a variety of clinical field. In the recent past, decompressive laminectomy followed by radiotheray was thought to be the best available treatment. More recently, radiotherapy alone has been advocated as an alternative treatment mode with a similar rate of effectiveness. This study compares laminectomy followed by radiotherapy to radiotherapy alone in the treatment of spinal epidural metastases in a randomized, prospective clinical trial. No significant differenc

A case of skull tumor simulating sinus pericranii is presented. The tumor was situated in the occiput near the midline and it was thought to satisfy the diagnostic criteria of sinus pericranii. At operation the tumor proved to be eosinophilic granuloma with intratumorous hemorrhage associated with epidural hematoma.

A 56-year-old woman was diagnosed with advanced rectal cancer, with tumor invasion to the sacrum and levator muscle of the anus and multiple lymph node metastasis. After construction of an artificial anus, chemotherapy was started. However, tumor invasion and the cancer pain progressed. Finally, she was hospitalized for pain control; an anesthesiologist planned to insert an epidural catheter. The epidural catheter was placed at the L5-S1 interspace, and continuous administration of 0.2% ropivacaine was started. Cancer pain in the buttocks improved quickly. Therefore, an epidural catheter with

A patient with a malignant cerebral mixed oligodendroglioma-astrocytoma presented with a severe, painful S-1 radiculopathy. Neuroradiological examinations revealed an extradural soft-tissue mass compressing the first sacral nerve root. Spinal fluid examination and myelography gave no evidence of intradural disease. Surgery confirmed the presence of metastatic glioma compressing the S-1 nerve root. The tumor was entirely epidural in location. Mechanisms of dissemination are discussed.

BACKGROUND: Cerebral venous outflow obstruction involves idiopathic intracranial hypertension, and the most common related condition is dural venous sinus stenosis or, in other words, an obstruction of the dural venous sinuses. In these cases, the pathological process is often chronic, displays only mild symptoms, and rarely requires urgent surgical intervention. In this study, we present a unique case involving an acute cerebral venous outflow obstruction that occurred during meningioma resection that ultimately had catastrophic consequences.
MATERIALS AND METHODS: The patient's preoperative

A 64-year-old man was diagnosed to have a tracheal tumor 2 cm proximal to the carina. He was scheduled for tracheal resection and reconstruction with omentopexy. An epidual tube was placed at the level of T5-6. After induction of anesthesia by propofol and remifentanil, a tracheal tube was inserted with the aid of bronchofiberscopy. Bilateral transversus abdominis plane block (TAPB) was performed with 0.5% ropivacaine 40 ml under ultrasound guidance. Anesthesia was maintained by sevoflurane and remifentanil, together with epidural infusion of ropivacaine and fentanyl. During tracheal resection

Postoperative paraplegia is a rare complication after epidural analgesia and often occurs with spinal hematoma or cord injury. We present the case of a 16-year-old girl who suffered from a tumor mass in the neck and abdomen who underwent gynecologic operation. Preoperatively, liver metastasis was found by computed tomography. Pathologic findings revealed that the abdominal mass was an ovarian dermoid cyst. After the operation, the patient complained of paraplegia while receiving epidural analgesia for postoperative pain control. A peripheral primitive neuroectodermal tumor in the thoracic and

A 59-year-old female underwent total hysterectomy for cancer of the corpus uteri. Epidural catheterization was performed at T 12/L 1 interspace. Anesthesia was induced with propofol (80 mg) and vecuronium (6 mg) and maintained with nitrous oxide (66%) and sevoflurane (0.8-1.5%) in oxygen. Six ml of 1.5% lidocaine containing 1: 200,000 epinephrine was injected intermittently through the epidural catheter for epidural anesthesia. Surgery was performed uneventfully. During her recovery from anesthesia, Mobitz II heart blocks and bradycardia were observed when the discharge in the oral cavity was

A 77-year-old male with lung cancer underwent a total left lung resection. During surgery, there was no ventilatory trouble such as hypoventilation or hypoxia. However, a mass was found in the right main bronchus by endobronchial fiberscopy before extubation. Because the mass could not be aspirated through the endobronchial fiberscope, we extracted it using basket-foreceps. We speculated that surgical procedures may have caused the tumor mass to move to the bronchus of the contralateral intact lung. Accordingly, fiberscopic examination of the airway before extubation should be mandatory, even

Polymyalgia rheumatica (PMR) is a common rheumatologic disorder in older individuals that usually requires chronic, low dose corticosteroid therapy for adequate control of symptoms. Epidural lipomatosis is a rare complication of chronic corticosteroid use in which neurologic deficits occur due to extradural fat compression of the spinal cord. The author describes the first reported case of epidural lipomatosis occurring as a complication of corticosteroid therapy in a patient with PMR.

A 23 year-old man, with a two month-history of back pain, presented with spastic paraplegia and hypesthesia of both lower limbs. A myelo C.T. revealed a spinal epidural dumb bell-shaped mass extending in the intra and extra vertebral spaces through the enlarged intervertebral foramen. Total recovery was observed three months after surgery. Pathological examination of the tumor showed it to be a cavernous angioma. Primary epidural angioma is rare: in 2 reported dumb bell-shaped spinal epidural cavernous angiomas, radiological findings were also suggesting a spinal neurinoma.

Therapy of pain induced by malignant diseases is an important task for any physician. A proper diagnosis is necessary for an adequate treatment. Pain in the bones can be treated successfully with peripherally acting analgesics such as acetylsalicylic acid, paracetamol or metamizole. On the other hand, certain tumors require local blockade as in cases with pancreatic or perianal tumors. If such a therapeutic approach is not possible or if pain is felt all over the body then centrally acting analgesics such as opiates are necessary. Opiates should be administered according to a tight schedule an

For several years opioids have been given epidurally with success to control chronic intractable pain. The authors report their 2-year experience with internalization of the epidural catheter and injection port in 18 patients at University Hospital, Saskatoon. In all, 21 devices were used. All patients had metastatic cancer; 16 are now dead. The need for other medications was eliminated with 9 and reduced with 12 devices. In the patients who died, the devices were effective for 82% of their remaining life-span. They were able to spend 46% of this time at home. Blockage of the epidural end of t
