From Freud to Feminism: Gendered Constructions of Blame Across Theories of Child Sexual Abuse.

Most theories of child sexual abuse are, to some degree, gendered, with nonoffending mothers bearing the burden of blame, ideologically and legally, for the transgressions of predominantly male offenders. This article explores the social construction of blame for child sexual abuse via critical analyses of evolving theoretical perspectives on maternal culpability for the inception and maintenance of abuse dynamics. Drawing on selected conceptual and research knowledge that supports and refutes anecdotal claims, this synthesis of the literature culminates in the proposal of an evidence-informed

Ethical Management of Patients with Cancer and Mental Illness.

Patients with co-existing cancer and mental illness must be given special attention due to the vulnerability that is created by their compromised psychological ability to comprehend the meaning of their cancer diagnosis, treatment, and prognosis. They are at increased risk for mortality due to many factors arising from their mental illness. To provide them with care that is just and compassionate, clinicians must be empathic and imaginative. Using a case and brief application of theories of justice involving vulnerable populations, we explore practical and ethical issues surrounding the care o

Free Riding in Healthcare Through a Game-Theoretic Lens: A Cross-Domain Narrative Review and Conceptual Synthesis.

Background/Objectives: Free riding in healthcare occurs when actors benefit from health-related public goods, risk-pooling arrangements, common resources, or cooperative institutions while contributing less than is socially optimal. This review clarifies how free-rider dynamics differ across vaccination, health insurance and universal health coverage, antimicrobial resistance, organ donation and transplant allocation, and global health cooperation. Methods: A narrative review with conceptual synthesis was conducted. Searches of PubMed and Scopus were complemented by citation tracking and targe

Protocol for 'Re: CBT Dialysis': a realist evaluation-why, for whom and in what circumstances does cognitive behaviour therapy work for people with depressive symptoms receiving dialysis?

INTRODUCTION: Nearly 40% of adults receiving life-saving dialysis for kidney failure report depressive symptoms. With more than 40 000 Canadians on dialysis, this is a significant health burden. Cognitive behavioural therapy (CBT) has been shown to be effective for treating depressive symptoms; however, it is rarely considered or used for people receiving dialysis. The aim of this realist study is to evaluate and explain how, why, for whom and in what circumstances therapist-guided and remotely delivered CBT works in order to provide equitable mental healthcare to individuals with depressive s

Capacity for decision-making in Alzheimer's disease: selfhood, positioning and semiotic people.

The aim of this article is to explore some of the ethical issues surrounding the assessment and determination of capacity of people with dementia in light of their meaning-making ability as shown through discourse. Discourse analysis, two illustrative cases, social construction theory and positioning are used to highlight some of the ethical dilemmas involved in basing a determination of capacity on the diagnosis of dementia and neuropsychological tests of cognitive function. Although neuropsychological tests have their place in assessing some aspects of cognitive function, aspects cognition s

Understanding peer support among healthcare assistants delivering hospice care at home: a protocol for a realist review.

INTRODUCTION: Globally, the demand for community palliative care, delivered within the home setting, is rising. Hospice support workers, also referred to as healthcare assistants, play a crucial role in providing this care, but evidence indicates they face challenges relating to inadequate training, isolation and emotional labour. This realist review aims to understand how peer support interventions can support healthcare assistants in delivering hospice care at home. METHODS AND ANALYSIS: The realist review will follow a five-step process to explore the research question: (1) locating existi

Injustice and inequality in the provision of perinatal palliative care.

Perinatal palliative care (PnPC) is a relatively new branch of pediatric palliative care (PPC), which focuses on providing holistic care in the antenatal, delivery, and neonatal settings. In this paper, we address previously unexplored justice-based ethical questions related to the provision of PnPC. We examine why some families who receive the diagnosis of a potentially life-limiting condition in their baby before or after birth receive PnPC support whilst others do not. We describe current inequities in the access to, and delivery of, PnPC. Drawing on philosophical theory (the Capabilities A

Operationalising the subjective: Coding visual art with AI.

The use of Artificial Intelligence (AI) to study socio-cultural phenomena in visual art is a rapidly growing field, yet it introduces fundamental methodological challenges around measurement. Drawing on recent debates surrounding measurement theory frameworks, this article examines the challenges of systematisation and operationalisation when using AI to analyse visual constructs in artworks. We argue that the shift from well-defined art historical attributes to subjective and contestable socio-cultural concepts marks a critical methodological break, demanding explicit attention to what is bei

Examining Physicians' Approaches to Treating Relatives in Primary Health Care Centers: Insights from a Qualitative Study.

Introduction: Family medicine physicians take care of a diverse population of patients with a variety of acute and chronic diseases. These patients include family, friends, and acquaintances who may ask for direct medical care or help in accessing healthcare products and services within or outside of officially approved procedures. This is ethically challenging due to an ambiguous medical code of ethics, but it is commonly accepted as normal behavior by society. The aim of this study was to explore family medicine physicians' perspectives regarding the benefits, difficulties, and ethics of res

Study protocol for the Collaborative Registrar Education and Training Enhancement (CREATE) project: a mixed-methods participatory study to co-design, implement and evaluate innovations to optimise GP training.

INTRODUCTION: General practice training in Australia is underpinned by early hospital attachments. These experiences are fundamental to the development of clinical and professional skills. However, reduced working hours of junior medical staff and limited opportunities for generalist supervision have raised concerns about the relevance of early training experiences in preparing doctors for careers in primary care. The Collaborative Registrar Education and Training Enhancement (CREATE) project aims to co-design and evaluate innovations that strengthen the educational value of hospital-based tra

Ethical challenges in biomarker research and precision medicine - a qualitative study in dermatology.

BACKGROUND: Over 300 million individuals worldwide live with Atopic Dermatitis and Psoriasis, which are among the most common chronic inflammatory skin diseases. Multimodal biomarkers are currently being developed using large-scale data and artificial intelligence to allow for more targeted prediction and to improve treatment of patients with Atopic Dermatitis/Psoriasis. Although this promises enormous benefits for patients, it comes with critical challenges. This article offers an in-depth analysis of the ethical challenges in research and application of data-driven biomarkers in chronic infl

Integrating New Fields of Study into Professional Identity: A social identity and landscapes of practice lens.

INTRODUCTION: The medical profession exists at the crossroads of many fields of study, including ethics, basic science, and communication. However, health professions students struggle with integrating new fields of study into their clinical practice and professional identities. This study examines how medical students perceive their professional identities as they consider, integrate, and identify with new fields of study related to the medical profession. METHODS: Three researchers qualitatively coded reflections on professional identity integration from 43 medical students exploring a new

The role of sport norms and organizational pressure on coaches' use of interpersonal violence: The mediating role of perceived instrumental effects of violence.

Interpersonal violence (IV) in sport is a pervasive issue that threatens athletes' physical, social, and psychological well-being. Sociological theories suggest that organisational pressure and adherence to sport norms, such as traditional masculinity and sport ethic, may contribute to coaches' beliefs in the instrumental benefits of violence, leading to the perpetration of IV. Despite prior research into these risk factors, no study has simultaneously examined individual, organisational, and societal factors influencing coaches' use of IV. This study aimed to explore how coaches' beliefs rega
