Antibacterial, Antifungal, Antiviral, and Anthelmintic Activities of Medicinal Plants of Nepal Selected Based on Ethnobotanical Evidence.

BACKGROUND: Infections by microbes (viruses, bacteria, and fungi) and parasites can cause serious diseases in both humans and animals. Heavy use of antimicrobials has created selective pressure and caused resistance to currently available antibiotics, hence the need for finding new and better antibiotics. Natural products, especially from plants, are known for their medicinal properties, including antimicrobial and anthelmintic activities. Geoclimatic variation, together with diversity in ethnomedicinal traditions, has made the Himalayas of Nepal an invaluable repository of traditional medicin

[Principle of evidence-based medicine and its application in clinical trial practice for glutamine and gut permeability].

Evidence-based medicine (EBM) was developed from the progress of traditional clinical model trial patterns. EBM was based on well-designed, randomized, double blind, controlled, multiple center trails in order to assess end pont parameters, prognosis parameters (as: mortality, morbidity of complications and hospital stay etc.), quality of life and cost/effect ratio. It has been accepted in developed countries and could be the guidelines for Chinese clinical trials in the future. This article summarized the principle of EBM and attached with a clinical trial sample about effects of glutamine di…

To what degree could clinical trials in evidence based medicine reflect reality in the treatment of candidemia?

INTRODUCTION: Evidenced based medicine (EBM) is necessary to standardize or treatment for infection since EBM is established based on the results of clinical trials. Entry criteria for clinical trials are very strict, and many patients have difficulties in being enrolled in any clinical trials regarding candidemia. It is questionable if the results of clinical trials reflect the real world of general medicine in this case. PATIENTS AND METHODS: For the purpose of examining how many patients could join any randomized clinical trials for the treatment of candidemia, we reviewed all the candidem

Thyrotoxicosis--surgical management in the era of evidence-based medicine: experience in western India with 752 cases.

The three modalities of treatment of thyrotoxicosis, antithyroid therapy (ATT), radio-iodine (I131) therapy and surgery are not cause-specific. In this paper, we describe our evolving experience with 752 thyrotoxic patients who underwent surgery during the last 40 years and discuss the current scenario with evidence-based data and observations wherever possible. Thyroidectomy was performed in 428 patients with Grave's disease (GD), 299 patients with toxic multinodular goitre, and 25 with toxic solitary nodules (TSN). Whereas 289 patients with GD had surgery for failed ATT, the other 139 had pr

From evidence-based to hope-based medicine? Ethical aspects on conditional market authorization of and early access to new cancer drugs.

There is a strong patient demand for early access to potentially beneficial cancer drugs. In line with this authorization agencies like the European Medicines Agency are providing drugs with conditional market authorisation based on positive interim analyses. This implies that drugs are used with insecure evidence of efficacy and adverse side-effects. Several authors have pointed to ethical problems with such a system but up to date no indepth ethical analysis of this system is found which is the aim of this article. Drawing of the four generally accepted principles of medical ethics: benefice

Off-label drug use in palliative medicine: Delphi study for the consensus of evidence-based treatment recommendations.

BACKGROUND: Off-label use of drugs is an integral part of everyday clinical practice in palliative medicine. However, it is associated with many uncertainties, that is, drug therapy safety or legal issues including cost coverage. Healthcare professionals often lack time and resources for comprehensive literature search and patient-specific risk-benefit analyses. AIM: The aim of this project is to develop, evaluate and rate agreement/disagreement on treatment recommendations for off-label use in adult palliative medicine. DESIGN: Online Delphi study with two rounds each to rate agreement/disa

Management of hand osteoarthritis: from an US evidence-based medicine guideline to a European patient-centric approach.

Hand osteoarthritis is the most common joint condition and is associated with significant morbidity. It is of paramount importance that patients are thoroughly assessed and examined when complaining of hand stiffness, pain, deformity or disability and that the patient's concerns and expectations are addressed by the healthcare professional. In 2019 the American College of Rheumatology and Arthritis Foundation (ACR/AF) produced guidelines which included recommendations for the treatment of hand osteoarthritis. An ESCEO expert working group (including patients) was convened and composed this pap

A 4-year review of human bite injuries presenting to emergency medicine and proposed evidence-based guidelines.

Human bite injuries are a common injury, roughly accounting for around 0.1% of attendances to Emergency Medicine departments. Morbidities associated with such injuries are legion. Of paramount importance is the potential for infection of the wound site, both bacterial and viral. A retrospective 4-year review of 3136 case notes was conducted, identifying 421 human bites. This amounts to one every 3 days! The majority of those bitten were young males (male:female ratio=3:1; 44% comprising the age group 16-25 years). The management of these wounds was found to be poor. 17% of patients did not rec

[Quality for the data basis of evidence-based medicine: rectal cancer and preoperative 5 x 5 irradiation].

Annually 25,000 people are diagnosed with rectal cancer in Germany. Neoadjuvant short-term radiotherapy is recommended for advanced local disease and is able to reduce by half the local recurrence rates even under high quality surgical conditions. However, the prognosis is not affected by neoadjuvant short-term radiotherapy. Key problems of neoadjuvant short-term radiotherapy are late toxic effects including faecal incontinence, sexual dysfunction, gastrointestinal symptoms, cardiovascular diseases as well as an increased frequency of secondary malignancies. Therefore, any overstaging has to b

Reporting quality and evidence support in randomized controlled trials of Bupleurum-based herbal medicine formulas for epilepsy.

ETHNOPHARMACOLOGICAL RELEVANCE: Bupleurum chinense DC (Chaihu) has been used extensively in traditional Chinese medicine (TCM), where it is traditionally prescribed to harmonize "Shaoyang" disorders and regulate liver function-concepts historically associated with neurobehavioral imbalance and convulsive symptoms. Classical formulas containing Bupleurum chinense, such as Xiao Chaihu Tang and Chaihu Shugan San, have long been applied in the management of epilepsy. This longstanding use provides an important ethnopharmacological context and supports the continued clinical evaluation of Bupleurum

Transforming tai ji quan into next-generation, evidence-based exercise medicine: A translational roadmap for falls prevention.

A substantial body of research provides compelling evidence that tai ji quan, a mind-body exercise rooted in traditional Chinese medicine, is effective in reducing falls and improving mobility and has gained increasing recognition as a form of evidence-based exercise medicine within mainstream healthcare. Despite this strong evidence base and growing policy support, the translation of tai ji quan from research into routine clinical practice remains limited. Addressing this evidence-to-practice gap will require innovative translational strategies that more effectively integrate evidence-based t
