Challenges in secondary prevention after acute myocardial infarction: A call for action.

Worldwide, each year more than 7 million people experience myocardial infarction, in which one-year mortality rates are now in the range of 10%, but vary with patient characteristics. The consequences are even more dramatic: among patients who survive, 20% suffer a second cardiovascular event in the first year and approximately 50% of major coronary events occur in those with a previous hospital discharge diagnosis of ischaemic heart disease. The people behind these numbers spur this call for action. Prevention after myocardial infarction is crucial to reduce risk and suffering. Evidence-based

Providing effective evidence-based catheter management.

Around a quarter of people admitted to hospital are catheterized at some point, however, urinary catheterization can have catastrophic effects on the bladder and a persons ability to regain continence in the future (Patel and Arya, 2001). Although most urinary catheters are used for a short-term period, bladder drainage complications, such as pain or bypassing, can occur (Niël-Weise and van den Broek, 2005). Sometimes, when these problems develop, or if the catheter is expelled, nurses react by changing the catheter, even though this is not necessarily the most appropriate solution. In fact, c

Prediction of Alcohol Withdrawal Severity Scale (PAWSS) for patients cared for in an emergency department/Level I Trauma Center: An evidence-based practice change.

BACKGROUND: Patients with alcohol use disorder (AUD) often come to the Emergency Department (ED) for unrelated complaints but can still experience withdrawal. The purpose of this Evidence-Based Practice (EBP) project was to detect the potential for alcohol withdrawal in the ED using the Prediction of Alcohol Withdrawal Severity Scale (PAWSS) and then provide necessary treatment to prevent negative patient outcomes (i.e., alcohol withdrawal-related seizures, admissions to the intensive care unit (ICU), and prolonged stays in the ED). METHODS: This EBP project was implemented at a Level I Traum

Analysis of EBN Interventions in the Perioperative Period of Minimally Invasive Surgery for Intrahepatic and Extrahepatic Bile Duct Stones.

CONTEXT: Evidence-based nursing (EBN) is a model of care that emphasizes the formation of scientific findings, together with the development of individualized care plans that consider a patient's condition. Few studies have occurred on EBN interventions for patients with intrahepatic or extrahepatic bile duct stones who are in the perioperative period of minimally invasive surgery. OBJECTIVE: The study intended to investigate the benefits of EBN interventions for patients with intrahepatic or extrahepatic bile duct stones who are in the perioperative period of minimally invasive surgery. Challenges for the adoption of evidence-based maternity care in Turkey.

Evidence-based medicine is an important tool for improving the quality of maternity care. However, getting providers to change their practices may not be an easy or rapid process, and other factors, in addition to knowledge of the literature, may be important. This study documents the current state of obstetric practices at three maternity hospitals in Istanbul, Turkey, and identifies attitudes, social pressures, and perceptions that, according to the theory of planned behavior, may pose challenges for adoption of evidence-based practices. Data were collected through interviews with administra

Intraoperative Irrigation: Fluid Administration and Management Amidst Conflicting Evidence.

Surgeons use irrigation during open cavity procedures to improve their view of the patient's anatomy and to reduce the patient's risk of infection. However, there are no standard guidelines that recommend a specific type of fluid, additive, or volume of irrigation to use during open procedures. Intraoperative hypothermia can occur if irrigation fluids have not been warmed or have cooled before use, causing adverse patient outcomes. In addition, failing to manage (eg, measure and document) fluid volume accurately may affect clinical decision making and cause other complications. Perioperative p

Enhancing neonatal wellness with home visitation.

We planned and implemented an evidence-based program to screen for jaundice and to try to increase the proportion of women breastfeeding for 6 months. The program involved home visitation by a registered nurse to provide education on and support of breastfeeding, and to perform physical assessment of both mothers and newborns, including screening for neonatal jaundice. Quantitative data showed increased breastfeeding rates at 6 months. In addition, readmission rates for jaundice were higher when compared to regional benchmarks. However, the average length of stay for treatment of jaundice was

Venous leg ulcers: Summary of new clinical practice guidelines published August 2014 in the Journal of Vascular Surgery.

The Society for Vascular Surgery (SVS) and the American Venous Forum (AVF) published guidelines for the management of venous leg ulcers in August 2014. The goal of this article (Part 2) is to summarize the guidelines that address diagnosis and treatment recommendations published jointly by the SVS and AVF that may affect the nursing practice of vascular nurses. Specific sections include wound evaluation, therapies used on the wound bed itself, compression, and operative or endovascular management. Part 1, published elsewhere in this issue, addressed the epidemiology and financial impact of ulc

Doctors record higher blood pressures than nurses: systematic review and meta-analysis.

BACKGROUND: The magnitude of the 'white coat effect', the alerting rise in blood pressure, is greater for doctors than nurses. This could bias interpretation of studies on nurse-led care in hypertension, and risks overestimating or overtreating high blood pressure by doctors in clinical practice. AIM: To quantify differences between blood pressure measurements made by doctors and nurses. DESIGN AND SETTING: Systematic review and meta-analysis using searches of MEDLINE, CENTRAL, CINAHL, Embase, journal collections, and conference abstracts. METHOD: Studies in adults reporting mean blood pres

Evidence-based prevention of pressure ulcers in the intensive care unit.

The development of stage III or IV pressure ulcers is currently considered a never event. Critical care patients are at high risk for development of pressure ulcers because of the increased use of devices, hemodynamic instability, and the use of vasoactive medications. This article addresses risk factors, risk scales such as the Norden, Braden, Waterlow, and Jackson-Cubbin scales used to determine the risk of pressure ulcers in critical care patients, and prevention of device-related pressure ulcers in patients in the critical care unit.

The role of magnesium sulfate in the prevention of cerebral palsy.

Magnesium sulfate (MgSO4) has been widely used in the perinatal arena for many decades. It has been used for tocolysis in the U.S. for more than 60 years. Estimations of MgSO4 use for preterm labor (less than 34 weeks of gestation) run as high as 80 percent. Magnesium sulfate is a smooth, skeletal, and cardiac muscle depressant. It is used for preterm labor because of its potential to decrease muscle contractility by interfering with calcium uptake in the cells. Thousands of moms and babies have been exposed to this medication even though tocolysis remains an off-label use, the exact mechanism

Healthcare workers' uniforms: roles, types and determining policy.

Demonstrating reductions in healthcare-associated infection has become a political and clinical priority in the NHS. A plethora of strategies have received the attention of evidence-based practice, one of which is the healthcare worker's uniform and the extent to which it may be implicated in the transmission of infection. Although existing evidence suggests that this risk is low, public opinion believes that such a correlation exists. However, it is difficult for researchers to quantify the level of risk, and organizations should be wary about developing uniform policies based on tenuous infe

A qualitative study of nursing care for hospitalized patients with acute mania.

Patients with a bipolar disorder and currently experiencing acute mania often require hospitalization. We explored patient problems, desired patient outcomes, and nursing interventions by individually interviewing 22 nurses. Qualitative content analysis gave a top five of patients problems, desired patient outcomes and nursing interventions, identified as most important in the interviews. We then conducted three focus group meetings to gain greater insight into these results. Intensive nursing care is needed, fine-tuning on the patient as a unique person is essential, taking into account the n

Assessment of oral mucositis in adult and pediatric oncology patients: an evidence-based approach.

Oral mucositis is a frequent side effect of cancer treatment and can lead to delayed treatment, reduced treatment dosage, altered nutrition, dehydration, infections, xerostomia, pain, and higher healthcare costs. Mucositis is defined as "inflammatory lesions of the oral and/or gastrointestinal tract caused by high-dose cancer therapies. Alimentary tract mucositis refers to the expression of mucosal injury across the continuum of oral and gastrointestinal mucosa, from the mouth to the anus" (Peterson, Bensadoun, & Roila, 2008, p. ii122). Evidence demonstrates that oral mucositis is quite distre

Atrial electrogram interpretation improves after an innovative education program.

BACKGROUND: To avoid adverse patient outcomes from inappropriate treatment, it is recommended that an atrial electrogram (AEG) be recorded whenever atrial arrhythmias develop in patients after cardiac surgery. However, AEGs are not commonly performed because nurses lack knowledge about differentiating atrial rhythms on AEGs. OBJECTIVE: To investigate whether completing a novel online evidence-based education program on interpreting AEGs would improve critical care nurses' AEG interpretation. METHODS: Specialized critical care nurses were taught about obtaining and interpreting atrial rhythms

Pain assessment and management practices in children following surgery of the lower limb.

AIMS: To examine paediatric nurses' pain assessment and management practices in relation to postoperative care for children following surgery of a fractured lower limb and to compare these practices with evidence-based guidelines. BACKGROUND: Managing pain is one of the most challenging issues in current paediatric practice. The incidence of lower limb fractures is high in children, which often leads to pain and related complications in the postoperative period. DESIGN: A retrospective clinical audit study. METHODS: A retrospective audit of all medical records (n = 106) was undertaken over
