Nasal contribution to exhaled nitric oxide at rest and during breathholding in humans.

We characterized the nasal contribution to exhaled nitric oxide (NO) at rest and during breathholding in humans. Exhaled NO was greater during nose breathing (141 +/- 17 nl/min/M2, mean +/- SEM) compared with mouth breathing (68 +/- 6 nl/min/M2, n = 8, p < 0.001). After voluntary closure of the soft palate (VCSP) to eliminate nasal NO, exhaled NO from the mouth decreased further (30 +/- 4 nl/min/M2, p < 0.001). Release of NO into nasal passages during VCSP (217 +/- 19 nl/min/M2) was greater than exhaled NO during nasal breathing (141 +/- 17 nl/min/m2, p < 0.001), suggesting that nasal NO is ta

Inhaled glucocorticoids decrease nitric oxide in exhaled air of asthmatic patients.

Exhaled nitric oxide (NO) is elevated in untreated patients with asthma but not in patients treated with inhaled glucocorticoids. This may reflect an inhibitory effect of glucocorticoids on the induction of the enzyme NO synthase in the respiratory tract. We have now studied the effect of an inhaled glucocorticoid (budesonide 800 micrograms twice daily via a dry powder delivery system for 3 wk) on exhaled NO in 11 patients with mild asthma in a double-blind crossover randomized-order placebo-controlled study. Exhaled NO was reduced from a baseline value of 203 +/- 29 parts per billion (ppb) to

A system for invitro inhaled/exhaled aerosol testing of personal protective equipment.

Social distancing at six feet (about two meters), frequent hand hygiene, and using personal protective equipment (PPE) such as masks and shields are common practices to reduce the transmission of airborne viruses. An in-vitro system for testing and comparing a selection of PPE to prevent aerosol escape and aerosol intrusion at three distances and three angles is described. In the system tested, at six feet, face masks or shields provided markedly decreased aerosol transmission in a room with limited ventilation. At two feet, with a type of protection wearing manikin emitting aerosols of normal

Applications and challenges of exhaled volatile organic compounds in critically ill patients.

Early detection of critical illness is essential for timely intervention and improved outcomes. Conventional diagnostic methods, such as laboratory tests and imaging, are invasive and often delayed. In recent years, non-invasive monitoring approaches, particularly exhaled breath analysis, have gained attention in critical care. Various analytical platforms, including gas chromatography-mass spectrometry, proton transfer reaction mass spectrometry, and electronic nose systems, have been employed to identify volatile organic compound (VOC) patterns associated with acute conditions. Elevated alde

Exhaled breath volatile organic compounds in cystic fibrosis: a systematic review.

Cystic fibrosis (CF) is a genetic disorder characterized by chronic airway infection and progressive lung damage. Early identification of biomarkers associated with respiratory pathogens and inflammatory processes is crucial for improving disease monitoring and guiding therapy. Analysis of volatile organic compounds (VOCs) in exhaled breath has emerged as a promising non-invasive approach for biomarker discovery. This systematic review aimed to identify and summarize VOCs detected in the exhaled breath of patients with CF that may serve as potential clinical biomarkers. A systematic literature

Propofol is in the air: can exhaled gas monitoring illuminate intravenous dosing?

Exhaled propofol monitoring has the potential to guide i.v. anaesthesia in a manner analogous to end-tidal monitoring of inhalation anaesthetics. Early proof-of-concept studies demonstrated the detection of propofol in breath, but adoption into routine clinical practice has been limited by the lack of compact, accurate, and rapid sensors, together with incomplete understanding of the pharmacokinetics of exhaled propofol across diverse patient populations. In this issue, a novel photoacoustic sensor that demonstrates strong correlation with mass spectrometry, the current gold standard for exhal

Continuous carbon dioxide monitoring in the exhaled breath of mechanically ventilated rats.

In small animals, carbon dioxide monitoring is either limited by the need to take blood samples for gas analysis, or it interferes with respiratory efficiency and lung mechanics analysis. We introduced a novel approach for continuous monitoring of CO2 in the expiratory limb of the breathing circuit. The relevance of the method is assessed by CO2 measurements at different respiratory settings. Rats were ventilated with a tidal volume (VT) of 8 mL kg-1. Respiratory rates were adjusted to achieve arterial CO2 partial pressure ( P aC O 2 ${P_{{\mathrm{aC}}{{\mathrm{O}}_2}}}$ ) between 35 and 4

Exhaled Breath Analysis in Lymphangioleiomyomatosis by Real-Time Proton Mass Spectrometry.

Lymphangioleiomyomatosis (LAM) is a rare progressive disease that affects women of reproductive age and is characterized by cystic lung destruction, airflow obstruction, and lymphatic dysfunction. Current diagnostic methods are costly or lack sufficient specificity, highlighting the need for novel non-invasive approaches. Exhaled breath analysis using real-time proton mass spectrometry (PTR-MS) presents a promising strategy for identifying disease-specific volatile organic compounds (VOCs). This cross-sectional study analyzed exhaled breath samples from 51 LAM patients and 51 age- and sex-matc

Discovery and analysis of the relationship between organic components in exhaled breath and bronchiectasis.

The prevalence of patients with bronchiectasis (BE) has been rising in recent years, which increases the substantial burden on the family and society. Exploring a convenient, effective, and low-cost screening tool for the diagnosis of BE is urgent. We expect to identify the accuracy (ACC) of breath biomarkers (BBs) for the diagnosis of BE through breathomics testing and explore the association between BBs and clinical features of BE. Exhaled breath samples were collected and detected by high-pressure photon ionization time-of-flight mass spectrometry in a cross-sectional study. Exhaled breath

Detection of COVID-19 by quantitative analysis of carbonyl compounds in exhaled breath.

COVID-19 has caused a worldwide pandemic, creating an urgent need for early detection methods. Breath analysis has shown great potential as a non-invasive and rapid means for COVID-19 detection. The objective of this study is to detect patients infected with SARS-CoV-2 and even the possibility to screen between different SARS-CoV-2 variants by analysis of carbonyl compounds in breath. Carbonyl compounds in exhaled breath are metabolites related to inflammation and oxidative stress induced by diseases. This study included a cohort of COVID-19 positive and negative subjects confirmed by reverse

Value of exhaled hydrogen sulfide in early diagnosis of esophagogastric junction adenocarcinoma.

Esophagogastric junction adenocarcinoma (EJA) has increased in recent years, and it exhibits a poor prognosis and a short survival period for patients. Hydrogen sulfide (H2S) plays an important role in the pathogenesis of cancer and has been studied as a diagnostic factor in some tumor diseases. However, few studies have explored the diagnostic value of H2S for EJA. In the present study, a total of 56 patients with early-stage EJA were enrolled while 57 healthy individuals were selected as the healthy control group. Clinical features were recorded, and exhaled H2S and blood samples were collec

Role of fractional exhaled nitric oxide in patients with sickle cell disease.

INTRODUCTION: Patients with sickle cell disease exhibit different patterns in pulmonary function tests. In particular, there is little evidence on the fractional exhaled nitric oxide (FeNO) test, and its value ranges and its interpretation in these patients have been under debate in recent years. METHODS: We conduced a cross-sectional, observational and descriptive study between November 2021 and January 2023 including patients aged 6-18 years with sickle cell disease able to perform the FeNO test. We applied the GLI-2012 reference values and the ERS/ATS standards. We defined statistical sign

Exhaled breath analysis: A promising triage test for tuberculosis in young children.

The diagnosis of paediatric pulmonary tuberculosis is difficult, especially in young infants who cannot expectorate sputum spontaneously. Breath testing has shown promise in diagnosing respiratory tract infections, but data on paediatric tuberculosis are limited. We performed a prospective cross-sectional study in Kenya in children younger than five years with symptoms of tuberculosis. We analysed exhaled breath with a hand-held battery-powered nose device. For data analysis, machine learning was applied using samples classified as positive (microbiologically confirmed) or negative (unlikely t

Analysis of a Broad Range of Carbonyl Metabolites in Exhaled Breath by UHPLC-MS.

Analysis of volatile organic compounds (VOCs) in exhaled breath (EB) has shown great potential for disease detection including lung cancer, infectious respiratory diseases, and chronic obstructive pulmonary disease. Although many breath sample collection and analytical methods have been developed for breath analysis, analysis of metabolic VOCs in exhaled breath is still a challenge for clinical application. Many carbonyl compounds in exhaled breath are related to the metabolic processes of diseases. This work reports a method of ultrahigh-performance liquid chromatography coupled with high-res
