A clinicopathological case report of a 71-year-old Caucasoid man with an unusual right lower eyelid lesion, which proved to be an eccrine poroma, is presented. Benign eccrine poromas have not previously been reported to occur on the eyelid. Compete surgical excision of this lesion proved to be curative, with no recurrence after 3 years follow up. Eccrine poromas are common benign tumours of the intraepidermal sweat duct unit. Sweat glands occur commonly on the eyelids and eccrine poroma should be considered in the differential diagnosis of eyelid tumours.

After a short description of the history of eyelid reconstruction, the causes of defects are discussed. For the primary and secondary treatment of thermic injuries, a full-skin graft is recommended and the indication of combined skin-cartilage transplants is mentioned. For deep defects, numerous methods of eyelid reconstruction are available. The differentiated indication for manifold initial situations is discussed in detail. The most important techniques are described on the basis of various examples.

A 15-year-old girl presented with a left lower eyelid tumor consistent with a nonresolving chalazion. The specimen was biopsied. Histopathologic examination revealed eosinophilic granuloma, an entity that rarely presents on the eyelid. The histopathologic features of eosinophilic granuloma--a member of the histiocytosis X group of diseases--are reviewed. Our patient responded well to incisional biopsy and has required no additional treatment.

Many systemic diseases or orbital disorders can induce or increase lid pockets: thyroid related diseases, floppy eyelid, systemic edema problems, true blepharochalasis, inflammatory pseudo-tumors, orbital tumors, vascular orbital disorders. These diseases are the subject of an iconographic presentation and must be considered before discussing a possible surgical treatment.

A 37-year-old man with a recurrent papillomatous lesion of the upper eyelid and four separate bulbar conjunctival ulcers was found to have North American blastomycosis. This represents the first reported case with conjunctival lesions that were not simply contiguous with eyelid involvement. This case underscores the importance of considering blastomycosis in the differential diagnosis of granulomatous conjunctivitis and when examining a lesion of the eyelid resembling a squamous cell carcinoma or atypical papilloma.

A case of eyelid gas gangrene caused by Clostridium hastiforme is reported. The infection followed an alkali injury that probably aided proliferation of the organism by inducing local tissue ischemia. Debridement combined with antibiotic therapy controlled the infection, and the large lid defect healed satisfactorily by secondary intention.

A five-year-old patient presented with nodular lesions on the left upper lid, index finger and thigh. Biopsy revealed granuloma annulare. The lesions resolved without treatment. This entity should be considered in the differential diagnosis of eyelid nodules.

Sebaceous adenocarcinoma is a rare neoplasm of the eyelid that often masquerades as a chalazion, blepharitis, or blepharoconjunctivitis. Controversy exists as to the proper mode of therapy for patients with a tumor confined to the eyelid. We believe that wide local excision (5 to 6 mm of normal tissue) with frozen section analysis and eyelid reconstruction is the preferred treatment. Only 1 of 11 patients had a recurrence with this technique. No regional or distant metastasis has occurred in an average of 68 months of follow-up.

Retraction of the lower eyelid is a useful sign of disease. It is an early manifestation of weakness of the facial muscles, occurring with myopathies, myasthenia, and upper and lower motor neuron facial paresis. Rarely, lower and upper lid retraction occur without proptosis in patients with Graves disease. Lower lid retraction occurs in proptosis and varies directly with the degree of proptosis. It is also seen with senile entropion or ectropion, after eye muscle or orbital surgery, and with contraction of lid tissues.
