Gastroesophageal reflux disease (GERD) is a common condition in infants. Symptoms from pathologic GERD include regurgitation, irritability when feeding, failure to thrive, and respiratory problems. Treatment typically starts with dietary modifications and postural changes. Antireflux medications may then be added. Indications for operative management in neonates and infants include poor weight gain, failure to thrive, acute life-threatening events, and continued respiratory symptoms. Laparoscopic Nissen fundoplication has become the standard of care for surgical treatment of children with GERD…

Association between celiac disease and intussusception has been reported in adult. Although intussusception is common in children, it rarely has been reported in association with celiac disease. Two children, 5 and 7 years old, with celiac disease are reported here, whose initial presentation was intussusception prior to investigation for concomitant failure to thrive. They presented with acute and severe abdominal distention with vomiting, and donuts and pseudo-kidney appearance in abdominal ultrasonography. One patient's intussusception had reducted spontaneously, however the other had requi…

Aldosterone synthase deficiency due to mutations in the CYP11B2 gene usually presents in infancy with electrolyte abnormalities and failure to thrive, whereas affected adults are usually asymptomatic. We describe a patient who first came to medical attention in middle age when he developed hyperkalemia after preparation for a barium enema. Past medical history was notable for failure to thrive in infancy. He had elevated PRA with low serum and urinary levels of aldosterone and its metabolites and normal or slightly elevated levels of 18-hydroxycorticosterone. These findings suggested a diagnos…

Although computed tomography (CT) is used primarily for diagnosis in humans, it can also be used to diagnose disease in veterinary patients. CT and associated three-dimensional reconstruction have a role in diagnosis of a range of illnesses in a variety of animals. In a sea turtle with failure to thrive, CT showed a nodal mass in the chest, granulomas in the lungs, and a ball in the stomach. CT of a sea dragon with balance and movement problems showed absence of the swim bladder. In a sloth with failure to thrive, CT allowed diagnosis of a coin in the intestine. CT of a puffin with failure to …

Hyaline fibromatosis syndrome is a rare, progressive and fatal autosomal recessive disorder characterised by multiple subcutaneous skin nodules, osteopenia, joint contractures, failure to thrive, diarrhoea and frequent infections. There is diffuse deposition of hyaline material in the skin, gastrointestinal tract, muscle and endocrine glands. The disease is often underdiagnosed since infants affected with the disease pass away early prior to establishing a final diagnosis. We describe an infant presenting with failure to thrive, progressive severe joint contractures and skin changes. Clinical …

Food faddism is a growing scenery. Since children are also involved in these unusual food habits of their parents, the pediatrician is faced with new nutritional problems. The consequence may be failure to thrive in infancy and childhood noticed mainly in families with strictly vegetarian food habits. Moreover the pediatrician should know the possible sequelae of all the other forms of food faddism. Only a careful nutritional history paralleling the usual medical history may then uncover the origin of a chronic failure to thrive. First and foremost infants after weaning are at special risk in …

A male Caucasian infant presented at 6 weeks of age with failure to thrive, diarrhoea, macrocytic anaemia, and decreased IgG. He had normal serum B12 and folate levels. Serum cobalamin binding capacity showed no detectable transcobalamin II. Both parents showed levels consistent with a heterozygous state. The literature is extensively reviewed, and the importance of early diagnosis to prevent neurological dysfunction is stressed.

BACKGROUND: Asian infants are at a higher risk of having Hirschsprung's disease (HD). Although HD is surgically correctable, serious and eve … were more likely to develop adhesive bowel obstruction after operation (33% vs. 3%, p = 0.013) and failure to thrive (33% vs. 3%, p = 0.013). Angiomyolipomas are commonly found in patients with tuberous sclerosis (TS), a rare genetic disorder. Angiomyolipomas are benign renal tumors composed of heterogeneously arranged blood vessels, smooth muscle tissue, and fat. Most commonly, angiomyolipomas are small and asymptomatic, but are more prone to enlargement and symptomatic bleeding in TS patients. This case report describes a rare case of massive bilateral angiomyolipomas in a TS patient who presented with spontaneous symptomatic bleeding, anorexia, and failure to thrive. Selective renal embolization was done to decrease the bleeding …

OBJECTIVES: Repair of complete atrioventricular septal defect (cAVSD) is routinely performed at around 3 months of age with good results. However, some patients require earlier surgery due to heart failure or failure to thrive. It is uncertain whether cAVSD repair performed on patients ≤3.5 kg leads to increased mortality and reoperation on the left atrioventricular valve.
METHODS: All patients who underwent cAVSD repair from 1990 to 2019 at a single institution were included in the study. Data were obtained from retrospective review of medical records and correspondence with cardiologists.

BACKGROUND: Laryngomalacia is the most common cause of congenital stridor in infants. While most cases are mild and self-limiting, some develop severe disease requiring surgical intervention due to airway obstruction, feeding difficulties, or failure to thrive. This study evaluates the outcomes of surgical management for severe laryngomalacia at a tertiary care pediatric center.
METHODS: A retrospective review was conducted of 56 children with severe laryngomalacia who underwent surgery between January 2013 and January 2023. Patients with secondary airway lesions or significant comorbidities …

Thalassaemia is the most common inherited haemoglobin disorder worldwide and a major health problem. Beta-thalassaemia major generally presents with a normal serum lipid profile, but there have been a few reports of its association with hypertriglyceridaemia, the exact pathogenesis of which remains unknown, and where early and regular blood transfusions ameliorate hypertriglyceridemia, thereby preventing its complications and the need for medical treatment of hypertriglyceridemia. This entity was seen in a 6-month-old infant girl who presented with severe anaemia, failure to thrive and organom…

Sodium-dependent multivitamin transporter (SMVT) deficiency is a recently described multivitamin-responsive inherited metabolic disorder (IMD) of which the phenotypic spectrum and response to treatment remains to be elucidated. So far, four pediatric patients have been described in three case reports with symptoms ranging from severe neurodevelopmental delay to feeding problems and failure to thrive, who demonstrated significant improvement after initiation of enhancement of targeted multivitamin treatment (biotin, pantothenic acid, and lipoic acid). We describe a fifth case of a patient prese…

Infantile hemangioma is the most common soft tissue tumor of infancy. Extensive organ involvement is rare. This report describes an infant with biopsy confirmed infantile hemangioma with diffuse organ involvement causing anemia and failure to thrive. Treatment was initiated with propranolol and led to initial improvement; however, course was complicated by several episodes of respiratory failure secondary to pulmonary edema. Propranolol therapy was interrupted for several months while patient was maintained on a diuretic regimen and treated with vincristine and high-dose corticosteroids. Patie…

Renal tubular acidosis (RTA) often presents as failure to thrive in children. In a resource-limited country, the diagnosis of renal tubular acidosis can be delayed or even missed because of the co-existing high burden of malnutrition. Here we report a case of a neonate/infant who presented with diarrhoea and failure to thrive and subsequently developed septic shock. During the management of the child, proximal renal tubular acidosis was diagnosed based on the persistence of metabolic acidosis with hyperchloremia. Proximal renal tubular acidosis (pRTA) can lead to complications, such as electro…

During the last two years two babies with failure to thrive developed distal renal tubular acidosis (RTA). We report a female patient with classic RTA where the failure to thrive occurred from the 8th month of age associated with polyuria and polydipsia. A 45 day old infant had RTA type 4 associated with unilateral renal agenesis; type 4-RTA is characterized by inadequate urinary potassium excretion in the face of hyperkalemia, in distinct contrast to the other type of RTA in which enhanced kaliuria is observed. Treatment with oral bicarbonate resulted in sustained normalization of blood acid-…
