Streptococcal toxic shock syndrome with fatal outcome: Report on four forensic autopsy cases.

Streptococcal toxic shock syndrome (STSS) is a severe infection most commonly caused by group A streptococcus. It is clinically characterized by rapidly progressive multiple organ failure and septic shock. This report presents four fatal cases associated with STSS. In two cases (cases 2 and 3), the portals of entry may be ulcer and mucosa at the surgical site; the initial symptoms in these cases included fever. In the other cases, the portal of entry was unknown; the initial symptom was pain. In two cases (cases 1 and 3), malpractice was suspected before autopsy. At autopsy, blood culture was

[Fatal Outcome of Agranulocytosis after Re-Exposure to Metamizole and Cefepime-Induced Encephalopathy].

Fatal Outcome of Agranulocytosis after Re-Exposure to Metamizole and Cefepime-Induced Encephalopathy Abstract. We present the case of an 83-year-old female patient who died as a result of likely drug-induced complications, namely agranulocytosis caused by metamizole and cefepime-induced encephalopathy. Agranulocytosis precipitated a cascade of events that eventually led to death. As prescription of metamizole has increased over the past decades, it is important to keep in mind its serious adverse drug reactions. Metamizole must be stopped immediately at the onset of symptoms such as fever, muc…

Pancreatitis as a severe complication of leptospirosis with fatal outcome: a case report.

Leptospirosis is a globally distributed zoonosis with a broad clinical spectrum. This disease mostly affects liver and kidney tissues. Other organs such as the pancreas, can be affected by leptospirosis-induced vasculitis. In addition, cardiac manifestations are common, and the presence of transient ECG abnormalities can be found in 70% of the patients. We report a male patient who presented with an atypical leptospirosis that progressed with severe acute pancreatitis, acute kidney injury and atrial fibrillation. Early diagnosis and adequate supportive therapy are crucial for the appropriated

Unrecognized staphylococcal vertebral osteomyelitis leading to fatal outcome in a previously healthy patient.

New onset low back pain which is recalcitrant to usual treatment should be evaluated aggressively even in the absence of fever or neurologic deficits. Corticosteroids given for back pain will accelerate occult spinal infection and may mask symptomatology leading to more severe disease.

[Pansclerotic morphea with a rapidly fatal outcome in an 11-year-old girl].

BACKGROUND: Pansclerotic morphea is a poorly described but extremely debilitating variant of localized scleroderma. We report a case with a rapidly fatal outcome in an 11-year-old girl. PATIENTS AND METHODS: An 11-year-old girl with a 2-year history of morphea presented at our institution in April 2012. The sclerosis had started on her trunk and progressed rapidly to involve her entire skin. Initial treatment with corticosteroids was ineffective and she presented extremely painful ulcerations of the lower limbs. The outcome was rapidly fatal, in early 2014, due to cachexia and sepsis after tw…

Elusive Cardiac Angiosarcoma in a Young Pregnant Female: Rare Presentation With Fatal Outcome.

Heart tumors are rare occurrences. They can present diagnostic challenges and severe complications especially in pregnant women. We report a rare case of angiosarcoma (AS) cordis of the right atrium in a young healthy pregnant female. Her diagnosis remained elusive for some time until development of advanced disease symptomatology. The diagnosis was unfortunately clinched when her tumor grew to be detected by imaging modalities. An emergency cesarean section was performed delivering a healthy baby. The patient was aggressively treated with chemotherapy to no avail. She passed away 2 months aft

Guillain-Barré Syndrome with Fatal Outcome during HIV-1-Seroconversion: A Case Report.

Guillain-Barré syndrome (GBS) is an acute or subacute peripheral polyneuropathy characterized by symmetrical muscle weakness. Its occurrence has been reported during acute HIV seroconversion since 1985. Among HIV-infected subjects, GBS has generally a favourable outcome. We report a case of GBS with fatal outcome during HIV seroconversion.

[De novo development of dyspnoea in a young healthy woman with fatal outcome].

A 20-year old previously healthy woman who had been taking oral contraceptives for 2 years develops increasing dyspnoea, general ill-feeling and lower back pain over a matter of weeks. During this time she attends her general practitioner several times. She is primarily treated for what is interpreted as asthma and later for pneumonia due to the increase in CRP (C-reactive protein). Finally, she is admitted to hospital with severe dyspnoea, cyanosis, and un-measurable blood pressure. She is referred immediately to MR scan, but suffers heart arrest in bed. Twenty minutes' attempt to revival was

Strongyloidiasis: a mistaken diagnosis and a fatal outcome in a patient with diarrhoea.

A patient who died in the UK from Strongyloides infection, which he had contracted in the West Indies, is described. The diagnosis was not suspected initially because he had not been forthcoming about his origins. The infection was more severe because the patient was also infected with the human T cell leukaemia/lymphoma virus type 1 (HTLV-1) and this may explain why the infection with Strongyloides was fatal. The features of the case are outlined to help other clinicians faced with such a patient.

Inflammatory myofibroblastic tumour of paranasal sinuses with fatal outcome: reactive lesion or tumour?

Inflammatory myofibroblastic tumours (IMTs) are clinicopathologically distinctive but biologically controversial entities, which have been described in the lungs, abdomen, retroperitoneum, and extremities, but rarely affect the head and neck region. IMT usually follows a benign clinical course after radical excision, but invasive, locally recurrent, and metastatic forms of abdominal and mediastinal IMT have also been described. This report describes a case of IMT of the paranasal sinuses with a fatal outcome. A 22 year old woman was admitted to hospital as a result of epistaxis. Computed tomog…

[A fatal outcome of false croup in lacunar angina complicated by peritonsillar abscess].

The paper reports a lethal outcome of false croup in a 62-year-old man. In pathogenetic chain of the disease the leading role belonged to purulent tonsillitis with peritonsillar abscess and epiglottitis with phlegmone of the adjacent soft tissues including the vocal cords. A severe edema of the latter provoked a complete obstruction in the larynx and asphyxia with all its macro- and microscopic morphological features. Chronic alcoholic intoxication seemed to cause immunodeficiency and slow progression of the disease. Tonsillitis, chronic with exacerbation, was alleviated by chronic alcoholic i

Fungemia due to Scedosporium prolificans: a description of two cases with fatal outcome.

Two cases, probably related, of fungemia due to Scedosporium prolificans are described in two patients with acute leukemia. Both were admitted to the hematological ward in nearby rooms, during building work in the hospital. After a previous bacterial sepsis in the neutropenic phase, which improved with antibiotic treatment, the respiratory status in both patients deteriorated presenting acute dypsnea, with a lung infiltrate in one of them. A few hours later both patients died. Blood cultures were positive for S. prolificans. These two new cases of S. prolificans infection stress the importance

[A patient of MELAS with 3271 mutation with fatal outcome after alcohol intake].

A patient of MELAS is reported. A 28-year-old woman was admitted to Shimada Municipal Hospital because of nausea, vomiting, and right homonymous hemianopsia. She had past history of dizziness and convulsion. A brain magnetic resonance imaging showed an ischemic lesion in the left occipital lobe, which disappeared in the follow-up study. Laboratory examination indicated elevated lactate and pyruvate levels in both blood and cerebrospinal fluid. The muscle biopsy demonstrated ragged-red fibers and strongly SDH-reactive blood vessels. PCR-RFLP analysis of DNA extracted from her muscle and blood a

[Late manifestation of a fatal Behçet's disease with cardiac involvement and lethal outcome].

HISTORY AND CLINICAL FINDINGS: One year before admission a 67-year-old man of German origin developed extensive ulcerations of the limbs, later also of the trunk and neck. In addition to recurrent oral aphthous ulcers it was associated with erythema nodosum and intermittent arthritis of the elbow knee and ankle joints. During the last month he also had dyspnoea. Echocardiography revealed a dilated cardiomyopathy (DCM). He was admitted because of the progressive skin disorder. Physical examination showed a disoriented man with dyspnoea, cyanosis of the lips and pretibial oedema. He had aphthous

[Necrotizing fasciitis cause by beta-hemolytic streptococci with fatal outcome--a case report].

The authors report two cases of necrotizing fascilitis (NF) occurring after surgical interventions on the breast (reduction mammoplasty in one patient and mastectomy for breast cancer in the other). In both cases the etiologic agent were group A pyogenic streptococci and both patients died. NF caused by hemolytic streptococci is a highly lethal disease (over 70% in literature). It starts typically with a purple lesion of the skin, followed by necrotizing fascial infection with secondary necrosis of the overlying skin and rapid progression to septic shock and multiorgan failure. This developmen
