Retained faecolith: an avoidable complication of laparoscopic appendicectomy.

A 26-year-old woman presented to the accident and emergency department 9 days post laparoscopic appendicectomy for a non-perforated, but gangrenous appendicitis. She was found to have a retained faecolith with a pelvic abscess. This case demonstrates one of the common pitfalls of the laparoscopic appendicectomy and we discuss some technical points to avoid such complications.

[Obstructed defecation syndrome].

Constipation has always been a common problem but recently it appears to be on the rise in the western world. Its prevalence in the general population is estimated at around 20% with reports suggesting significantly higher levels in the elderly, especially above the age of 65. There have also been reports of females being affected more then males, Higgins et al reporting a male to female ratio of 1: 2.2. Constipation can be classified as either primary or secondary. Primary constipation is either due to prolonged transient time through the colon (colonic inertia) or a disturbance in defecation

The radio-opaque appendicolith--its significance in clinical practice.

Four cases are reported, in whom radiologically opaque appendiceal coproliths (appendicoliths) were recognised on plain abdominal radiographs. These were undertaken for diagnostic purposes, in patients with right-sided abdominal pain, in whom the diagnosis was not immediately apparent. The pathology, radiological features, and clinical significance of the phenomenon are discussed, and the association of appendicoliths with appendiceal perforation and gangrene is stressed. The fact that clinical signs and symptoms of acute appendicitis are often misleading and sometimes minimal, even in the pre

Acute diarrhoea in the elderly.

One hundred consecutive patients admitted to hospital because of acute diarrhoea are described. The commonest causes of diarrhoea were found to be faecal impaction, infection and drugs, although in a third of the cases no cause was found. Faecal impaction was significantly more common in those living alone or in institutions than in those living with their families. The study suggests that the elderly population is very intolerant of the symptom of diarrhoea, and a case is made for the provision of facilities for more efficient assessment and treatment.

[Treatment of meconium ileus equivalent].

A 12-year old boy with mucoviscidosis developed a mechanical obstruction of the small intestine. The diagnosis of a meconium ileus equivalent was made and a conservative treatment was started. After this treatment failed, a laparotomy was performed which verified the diagnosis of a meconium ileus equivalent. It was possible to move the inspissated feces inside the terminal ileum into the ascending colon. The postoperative course was uneventful. The etiology, differential diagnosis, and both conservative and operative treatment of this complication of the mucoviscidosis are discussed.

[Coprolith-ileus of the small intestine].

The case of a 71 year old man is reported, in which coproliths developed in the small intestines over a period of 13 years and which finally lead to a complete ileus. This condition is discussed in connection with other rare forms of ileus; specific clinical phenomena are pointed out, which might be helpful in establishing a correct diagnosis. Among the transient forms of ileus associated with food intake, which may cause the patient only slight discomfort, one should bear in mind a corpus alienum-ileus. Only operative therapy of ileus is promising success.

Is digitation associated with proctographic abnormality?

Constipated patients referred for evacuation proctography were questioned as to the need for digital manoeuvres to assist evacuation, to determine the frequency of digitation, and to investigate whether any proctographic feature was associated with this symptom. Seventy one percent of patients interviewed admitted to digitation. Proctographic findings in 20 constipated patients who digitated per vagina and 20 who digitated per anum were compared to 20 who did not. There was no significant difference between groups with respect to pelvic floor descent, anal canal width or any anrectal angle mea

Golytely: a new approach to faecal impaction in old age.

Forty-five elderly subjects with faecal impaction were entered into a study of the efficacy of golytely. Twenty-three were treated with golytely 2 l/day for 2 days and then lactulose 30 ml b.d. and daily enemas. Twenty-two were treated with lactulose 30 ml b.d. and daily enemas alone. A good response was achieved in 13 (56%) within 1 week and in 20 (87%) within 2 weeks when golytely was used, as compared to only 5 (23%) within 1 week and 9 (41%) within 2 weeks when it was not used.

[Calcified appendiceal calculi in children (author's transl)].

Authors present, a series of 73 infantile appendicular interventions, the high incidence of coproliths found in the appendix lumen (30.13%) which contrasts with the number presented by other authors which oscillate between 0.64 and 15%. The radiologic study of the extirpated appendix shows coproliths which has not been visualized in the preintervention-radiologic exploration. Due to this, authors consider convenient the study of the extirpated piece in order to the exact evaluation of the number of coproliths. Microscopic analysis and infrared spectrophotometry show the existence in the compos

Rectal bleeding secondary to fecal disimpaction: angiographic diagnosis and treatment.

Two cases of bleeding caused by manual disimpaction of fecal material are presented. Angiography played a critical role in the diagnosis and treatment. One case was managed with balloon tamponade, and the other was successfully treated by selective intra-arterial vasopressin infusion, after prior intravenous infusion had been ineffective. A history of fecal disimpaction should suggest the possibility of low rectal bleeding which can be diagnosed and treated angiographically.

Intestinal neuronal dysplasia in twins.

Intestinal neuronal dysplasia (IND) has been described proximal to the aganglionic segment in Hirschsprung's disease and less frequently as an isolated condition. Familial occurrence of IND is rare. We report a case of monozygotic twins affected with IND. One-year-old male monozygotic twins presented with a history of chronic constipation and abdominal distension since birth. At presentation, both twins had fecal impaction with palpable fecal-filled loops of bowel. Barium enema in both patients showed gross fecal loading of the sigmoid colon and rectum. Delayed films taken at 48 h showed marke…

Physiological Optimization of Digital Self-Disimpaction Using a Step Stool: A Case Report.

Digital disimpaction, which is often required for cases of severe fecal impaction, carries significant risks of mucosal injury and vasovagal reflex. Evidence-based guidance for safe home-care disimpaction is limited. Moreover, detailed physiological self-assessments of medically trained individuals during such events have seldom been reported. Herein, we report a rare case of self-disimpaction performed by a gastroenterological surgeon, providing a detailed clinical analysis of the physiological mechanisms involved. A 49-year-old male gastroenterologist presented with recurrent distal fecal im…

The Appropriateness of Glycerin Enema in Pediatric Patients Visiting the Emergency Department.

OBJECTIVES: We determined whether glycerin enemas were appropriately prescribed in pediatric fecal impaction patients using the Leech score and identified factors that influenced the prescription of glycerin enemas in the pediatric emergency department (PED). METHODS: We included patients who received a glycerin enema at the PED of a tertiary teaching hospital. We divided the study subjects into two groups on the basis of their Leech scores: an appropriate enema group (Leech score ≥ 8), and an inappropriate enema group (Leech score < 8). Logistic regression was performed to determine the fact…
