Should the lumbar or femoral neck young adult mean percentage be measured in patients with distal radius fractures? A case-control study.

BACKGROUND: In the 2015 guidelines for the prevention and treatment of osteoporosis in Japan, the young adult mean percentage (%YAM) of the lumbar spine or femoral neck is required to diagnose osteoporosis in patients with distal radius fracture (DRF). When the %YAM of multiple locations is measured, the lower %YAM is adopted for the diagnosis. If the area with a lower %YAM is known, bone mineral densitometry tests may only need to be performed at one site. This study aimed to compare the lumbar and femoral neck %YAMs of patients with DRFs to find out the lower %YAM site. METHODS: We retrospe

Retrograde Nailing of an Unstable Distal-Third Femoral Shaft Fracture Using Pure Indirect Reduction: A Case Report in Osteoporotic Bone.

INTRODUCTION: Distal-third femoral shaft fractures in the elderly are rarely straightforward. Reduction and fixation are challenging due to poor bone quality, an unstable fracture pattern, and not enough room to work. Retrograde nailing is an accepted solution, but in long-oblique fractures, maintaining alignment is often the real problem. Many surgeons depend on poller screws, cerclage wires, or temporary plates to hold the reduction. In this report, we describe a case where none of these were used. The fracture was reduced and held using indirect methods alone. CASE REPORT: A 72-year-old fe

Distal Femoral Nail as a Cost-Effective Alternative to Tibiotalocalcaneal Nail for Ankle Fracture Non-Union in an Elderly Patient: A Case Report.

INTRODUCTION: Ankle fracture non-union in elderly patients is challenging due to osteoporotic bone, comorbidities, and risks associated with open reduction and internal fixation (ORIF). Tibiotalocalcaneal (TTC) nail fixation enables early weight-bearing but is costly and not widely available in resource-limited settings. CASE REPORT: We present an 83-year-old male with a painful, deformed left ankle and non-union of a bimalleolar fracture treated conservatively. Comorbidities included diabetes mellitus and hypertension. Due to fragile skin and high ORIF-associated risks, a distal femoral nail

Aneurysmal Bone Cyst of the Distal Femoral Metaphysis in a Three-Year-Old Male Patient Presenting With a Pathologic Fracture: A Case Report.

This case report discusses the management of a three-year-old male presenting with a pathological fracture of the distal femur due to an aneurysmal bone cyst (ABC). Due to diagnostic uncertainty and limited resources, the treatment involved an en bloc resection followed by the Masquelet technique with a fibular and cancellous graft. While this approach successfully reconstructed the bone defect, the patient developed a progressive valgus deformity and limb length discrepancy, likely due to physeal damage from the resection rather than the reconstruction technique itself. This case highlights t

How wide of a distal metaphyseal femoral fracture gap is a high risk of varus collapse and fixation failure? A finite element study.

BACKGROUND: Severe metaphyseal comminution and sizable bone defect of the distal femur are high risks of fixation failure. To date, no exact magnitude of comminution and bone loss is determined as an indication for augmentation of fixation construct. The present study aimed to investigate the influence of metaphyseal gap width, working length, and screw distribution on the stability of the fixation construct. METHODS: A finite element model of a fractured femur with 0-80 mm metaphyseal gap width stabilized by an 11-hole distal femur locking compression plate (LCP-DF) was generated. The differ

Iatrogenic distal femur fracture following medial femoral supracondylar bone graft harvest: a case report and finite element analysis.

Objective: This report presents a case of supracondylar femur fracture with finite element analysis and discusses its causes and prevention. Patient and Methods: A 53-year-old man presented with right talar osteonecrosis after osteosynthesis for a talus fracture. A medial femoral condyle-free vascularized bone graft (size, 20 × 12 × 17 mm) from the contralateral femur was performed, including the posteromedial cortical corner. The patient suffered a donor-site supracondylar femoral fracture while standing up from a cross-legged sitting position on the bed on postoperative day 6. The fracture w

The Reversed Less Invasive Stabilisation System-Distal Femur Technique: Application in an Adult Patient with Osteogenesis Imperfecta Sustaining a Femoral Fracture.

INTRODUCTION: The aim of this study was to report the surgical management and to discuss the options for fracture fixation in an adult patient with osteogenesis imperfecta (OI) who sustained a trochanteric femoral fracture after a simple fall from standing position. CASE REPORT: As a result of multiple fractures during childhood, this adult patient with OI presented with a short stature. The radiographs revealed a displaced, intertrochanteric fracture with subtrochanteric extension of the left femur. The intramedullary canal was narrow, the femur presented with a severe bowing deformity, and

[Salter and Harris type-II distal femoral physeal fracture-separations at adolescent age: a new therapeutic approach (preliminary study)].

PURPOSE OF THE STUDY: The prognosis of distal femoral physeal fracture-separation is poor in children. In adolescents, more than half of the cases are classified as Salter and Harris type-II. The gold-standard treatment for a displaced fracture combines anatomic reduction with internal fixation with a pin or screw, preserving the growth cartilage. Despite this treatment, the rate of mid- and long-term complications has been high in the literature, most problems being related to leg length discrepancy and misalignments (genu valgum and genu varum). In order to avoid these problems, for adolesce

Management of open periprosthetic distal femoral fracture using a novel hybrid technique combining Masquelet and DAIR procedure: A case report.

INTRODUCTION: Periprosthetic distal femoral fractures (PPDFF) present complex clinical challenges, particularly when accompanied by open injuries and significant bone loss. Conventional treatments, including open reduction and internal fixation (ORIF) or distal femoral replacement (DFR), carry high complication rates such as infection, non-union, and implant failure. This report describes a novel hybrid approach combining the Masquelet technique with Debridement, Antibiotics, and Implant Retention (DAIR) for managing open PPDFF. CASE PRESENTATION: A 74-year-old female patient with a prior tot

Neurovascular Bundle Entrapment and Through Knee Amputation After Cerclage Cabling of Distal Femoral Peri-implant Fracture: A Case Report.

CASE: We report the case of an 82-year-old woman with diabetes, arteriosclerosis, chronic heart failure, and hypertension treated with an anatomical locking plate and multiple cables for a spiral-wedged periprosthetic fracture of the distal femur which was complicated by direct occlusion of the femoral artery and crush of the sciatic nerve, resulting in leg necrosis and, ultimately, through-knee disarticulation despite early recognition and arterial repair. CONCLUSIONS: Neurovascular injury is a potential complication of cerclage cables placement around the femoral shaft during complex fractu

Aneurysmal Bone Cyst of the Distal Femoral Metaphysis in a Four-year-old Female Patient Presenting with a Pathologic Fracture: A Case Report.

Aneurysmal bone cysts (ABCs) are benign osteolytic vascular lesions that are capable of aggressive local expansion and bone destruction. These tumors are most common in adolescent patients and constitute approximately 9% of benign tumors. ABCs can present a diagnostic challenge, as they share several histological and radiographic characteristics with more aggressive lesions, including giant cell tumors and malignant telangiectatic osteosarcomas. The management of ABCs is diverse, but the most common approach includes lesion curettage with bone grafting. Here, we present the case of a large, ce

Risk factors for post-operative complications in patients older than 80 years treated surgically for periprosthetic distal femoral fractures after total knee arthroplasty.

BACKGROUND: Periprosthetic distal femoral fractures after total knee arthroplasty (TKAPF) are challenging in very elderly patients. This study aimed to identify the incidence and risk factors for post-operative complications in patients aged over 80 years surgically treated for these fractures. METHODS: A multicentre SOFCOT database (2012-2019) was analyzed. Patients aged >80 years with TKAPF were compared to a < 80 control group. Outcomes included complications, reoperation, mortality, and operative delay. Multivariable logistic regression was used to identify independent risk factors for re

Outcomes of distal femoral fractures treated with minimally invasive plate osteosynthesis versus open reduction internal fixation with combined locking plate and interfragmentary screws.

PURPOSE: Fractures of the distal femur remain challenging to treat, and numerous fixation methods are designed to promote stability and fracture healing. Locking plate constructs have recently become the mainstream fixation method, but debate exists on whether to use locking plates alone or to augment them with interfragmentary screws. This article compares outcomes of distal femur fractures treated with a single locking plate alone versus those treated with a locking plate and interfragmentary screws. METHODS: We retrospectively reviewed 57 patients treated for distal femur fractures from 20

Finite element analysis, biomechanics, and clinical advantages of percutaneous medial column screw reinforced locking plate in the treatment of complex distal femoral fractures.

AIMS: The main treatment method for distal femoral fractures is open reduction and internal fixation with a lateral locking plate. However, the literature indicates that the failure rate for this method is high, the healing is slow, and the prognosis is poor. Therefore, this study aimed to solve the problems of high failure rate and poor rehabilitation. METHODS: Data from 140 patients with AO/OTA type 33 A/33 C fractures (September 2014 to December 2023) were retrospectively analyzed. After excluding 20 cases (loss to follow-up, Gustilo III fractures, polytrauma), 120 patients were categorize

Comparison of the RFN-advanced femoral nailing system versus locked lateral plating in the management of distal femur fractures: A matched-cohort analysis.

INTRODUCTION: Distal femur fractures are commonly managed with retrograde femoral nailing or locked lateral plating (LLP). As implant design has evolved, more distal and complex patterns are being treated with intramedullary implants. The aim of the present study was to compare early outcomes in distal femur fractures managed with the novel DePuy Synthes RFN-Advanced (RFNA) Retrograde Femoral Nailing System to a similar cohort treated with locked lateral plating. PATIENTS AND METHODS: This is a retrospective cohort study of operative distal femur fractures that presented to our Level I trauma

Early ambulation and good outcomes after using combined nail plate construct for fixation of distal femoral fractures: a retrospective series of 14 cases.

BACKGROUND: Combined nail/plate technique is a relatively novel method for surgically managing distal femur fractures. It was supposed to allow for early weight bearing and achieve adequate fixation that allow for good bone healing. This study aims to describe our single institution experience of treating distal femur fractures using the combined nail/plate technique. METHODOLOGY: This is a retrospective study of 14 cases who had AO/OTA fractures 33A_C that were managed with this technique. Patients mean age was 67.6 years and all of them had either obvious osteopenia/osteoporosis or comminut
