New-onset Immune-mediated Necrotizing Myopathy and Trigeminal Neuropathy after SARS-CoV-2 mRNA Vaccination in a Patient with Rheumatoid Arthritis and Sjögren's Syndrome.

We present the case of a 42-year-old woman with rheumatoid arthritis and Sjögren's syndrome treated with adalimumab who developed immune-mediated necrotizing myopathy (IMNM) and trigeminal neuropathy after severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) mRNA vaccination. Trigeminal neuralgia and elevated serum creatine kinase levels emerged 12 days post-vaccination, followed by myalgia in the femoral muscles. IMNM was histologically diagnosed. The pathogenesis may involve molecular mimicry between the SARS-CoV-2 spike glycoprotein and autologous tissues triggered by vaccination. T

What's in the literature?

The neuromuscular literature over the past 3 months has been diverse, including useful information on the epidemiology of several disorders. Our understanding of the genetics of amyotrophic lateral sclerosis continues to grow, and in the process, it makes the distinction between familial and sporadic forms of the disorder increasingly murky. Some interesting articles about peripheral neuropathy provide insight into relationships with diabetes and with Parkinson disease and summarize the state of knowledge of the increasingly complex topic of hereditary neuropathies in children. Epidemiology an

Prevalence of femoral nerve neurodynamic disorder in patients with patellofemoral pain syndrome: a cross-sectional study.

BACKGROUND: Patellofemoral pain syndrome (PFPS) is among the most prevalent causes of knee pain in young adults. Emerging evidence suggests that musculoskeletal pain syndromes, including PFPS, may be associated with neurodynamic disorders. The aim of this study was to investigate the prevalence of femoral nerve neurodynamic disorders in patients with PFPS and to examine the association between these disorders and various clinical and demographic variables, including age, sex, body mass index, pain intensity, history of low back pain, functional ability, and rectus femoris shortness. METHODS:

Lateral Femoral Cutaneous Nerve Radiofrequency Ablation for Meralgia Paresthetica: A Description of a Novel Technique.

Meralgia paresthetica (MP) is neuropathic pain in the anterolateral thigh, most often caused by compression or injury to the lateral femoral cutaneous nerve (LFCN). Conservative treatments include weight loss, avoidance of compressive clothing, medical management with anti-inflammatory and neuropathic agents, and corticosteroid injections. For patients with MP refractory to these measures, radiofrequency ablation (RFA) is a minimally invasive option that may provide pain relief. This article introduces a novel ultrasound-guided continuous RFA technique designed to create a controlled thermal l

Comprehensive analysis of surgical treatments for meralgia paraesthetica: Case series and meta-analysis encompassing results of decompression vs neurectomy.

BACKGROUND: Meralgia paraesthetica (MP) is caused by lateral femoral cutaneous nerve (LFCN) compression. Surgical intervention is considered for refractory cases, with LFCN decompression being the most commonly performed procedures. In this study, we present the clinical outcomes of LFCN neurectomy performed at a single neurosurgical department. Our findings are juxtaposed meta-analysis assessing the success rates of different surgical strategies. MATERIAL AND METHOD: 14 LFCNs neurectomies were performed in 13 MP patients at our Department between 2015 and 2022. Follow-up evaluations were con

Meralgia Paresthetica as a Result of Surgery With an Emphasis on Harvesting Iliac Bone Grafts: A Review.

Meralgia paresthetica is a neurological disorder characterized by a symptom complex of numbness, burning, tingling, aching, or stabbing in the anterolateral portion of the upper thigh. Typically, this disorder is seen in patients with diabetes mellitus, obesity, and pregnancy. Also, it may result from a wide array of surgical interventions involving the region of the anterior superior iliac spine. Underlying pathophysiology concentrates on entrapment neuropathy of the lateral femoral cutaneous nerve (LFCN). Due to its location and wide anatomic variation, the LFCN is susceptible to compression

Lateral femoral cutaneous neuralgia: an anatomical insight.

A detailed anatomic study was carried out on the lateral femoral cutaneous nerve to better understand the etiology and treatment of lateral femoral cutaneous neuralgia. As it passed from the pelvis into the thigh, the lateral femoral cutaneous nerve ran through an "aponeuroticofascial tunnel," beginning at the iliopubic tract and ending at the inguinal ligament; as it passed through the tunnel, an enlargement in its side-to-side diameter was observed, suggesting that the fascial structures proximal to the inguinal ligament may be implicated in the genesis of lateral femoral cutaneous neuralgia

[Cervical spondylosis revealed after general anesthesia].

We present a case of complicated postoperative nerve injury. A 70-year-old man complained of partial paralysis and numbness of the left forearm 1.5 hour after operation for femoral artery bypass graft. Postoperative cervical spine MRI revealed cervical spondylotic radiculopathy. The MRI findings could account for his symptom. Not only peripheral nerve injury but the cervical radiculopathy may play an important role in perioperative neuropathy.

Femoral nerve compression caused by a hibernoma in the right thigh: a case report and literature review.

BACKGROUND: A hibernoma, also known as a brown fat tumor, is a rare benign soft tissue tumor, which originates from brown adipose tissue remaining in the fetus after the gestational period. It is often detected in adult men, presenting as a painless slow-growing mass. Hibernomas of the thigh have been reported; however, motor and sensory disorders caused by the tumors compressing the femoral nerve have not been reported. We report a case of a histopathologically proven hibernoma that induced femoral mononeuropathy. CASE PRESENTATION: A 26-year-old man was admitted to the hospital due to a mas

High-resolution ultrasound of the posterior femoral cutaneous nerve: visualization and initial experience with patients.

OBJECTIVE: The posterior femoral cutaneous nerve (PFCN) is a sensory nerve originating from the sacral plexus. PFCN neuropathy leads to pain within the inferior gluteal region and the posterior aspect of the thigh. As electrophysiological assessment is challenging, diagnosis of PFCN neuropathy has been, thus far, primarily based on clinical findings, which can result in misdiagnosis. Therefore, alternative confirmatory assessments such as an imaging modality that could aid in the diagnosis of PFCN neuropathy would be desirable. The purpose of this study was to determine the feasibility of visu

[Ilio-inguinal and genito-femoral neuralgia. Causes, clinical aspects, therapy].

Entrapment neuropathy of the ilioinguinal and of the genito-femoral nerves is a rare but sometimes very painful complication of abdominal surgery (herniorrhaphy, appendectomy, nephrectomy, gynecological surgery, removal of bone from the inner table of the iliac crest, etc.). Sometimes symptoms develop during a normal pregnancy or delivery. Spontaneous entrapment of the ilioinguinal nerve as it passes through the Mm. obliquus internus and transversus abdominis has also been observed. We have operated such a case. The causes, features and the associated findings of entrapment neuropathy of the t

Electrodiagnostic evaluation of lower extremity mononeuropathies.

This article discusses the anatomy of lower limb mononeuropathies and reviews the general approach to evaluating patients in the electrodiagnostic laboratory with suspected mononeuropathies of the lower limb. Through illustrative cases of patients presenting with a floppy foot, buckling knee, or painful foot, the approaches using nerve conduction studies and needle electromyography are reviewed, and the pattern of findings of peroneal, tibial, sciatic, femoral, and obturator neuropathies is shown.

[Peripheral nerve blocks for a patient with spondylolysis undergoing anterior cruciate ligament reconstruction].

A 24-year-old man, 178 cm tall and weighting 82 kg, underwent anterior cruciate ligament reconstruction. We gave general anesthesia and ultrasound-guided combined femoral and sciatic nerve block. Neuraxial blocks increase the risk of neuropathy in the patient with spondylolysis compared with peripheral nerve blocks. In this report, we describe the successful postoperative pain control with peripheral nerve blocks to a patient with spondylolysis.

Nerve Block Extends Nerve Function Recovery in Patients with Diabetic Foot Ulcers.

OBJECTIVE: Research has revealed that patients with diabetes and peripheral neuropathy exhibit significantly elevated nerve stimulation thresholds. However, the minimum stimulation thresholds of peripheral nerves in patients with diabetic foot, along with the recovery of nerve function, remain undetermined. The aim of this study is to investigate the minimum stimulation thresholds of the femoral and sciatic nerves, as well as the duration of nerve block, in patients diagnosed with diabetic foot. METHODS: From July 2020 to March 2022, a prospective study was conducted involving patients aged 5

Sensory conduction study of the infrapatellar branch of the saphenous nerve.

Although neuropathies of the infrapatellar nerve (infrapatellar branch of the saphenous nerve, IPBSN) have been reported clinically, no electrophysiological method has been defined to evaluate IPBSN conduction. We therefore studied a total of 60 saphenous nerves and 60 IPBSNs from 36 volunteers. The IPBSN was stimulated medially with a surface electrode 2 cm below the patella. The response was recorded with a needle electrode located close to the nerve 1 cm lateral to the femoral artery in the inguinal region. Sensory nerve action potentials were obtained from each subject; mean latency of the

Malignant secondary deposit in the iliac crest masquerading as meralgia paresthetica.

Meralgia paresthetica is an entrapment neuropathy from compression of lateral femoral cutaneous nerve of the thigh as it passes under the inguinal ligament. The entrapment produces pain, paresthesia, and sensory loss over the anterolateral aspect of the thigh, without motor loss. The etiology of this condition is often obscure and the pathogenesis is thought to be an entrapment of this nerve at the inguinal ligament where it pierces the fascia to reach the skin. Rarely, retroperitoneal lymphomas or other neoplasms can mimic symptoms of meralgia paresthetica. We report a case of a 60-year-old m
