PURPOSE: We report the use of an ultrathin fibreoptic bronchoscope (Olympus N20: external diameter: 2.2 mm) as the means of airway endoscopic monitoring during anaesthesia for the excision of mediastinal bronchogenic cysts in two young children.

A 36-year-old male presented with knife-inflicted neck trauma. He was intubated in the emergency department but subsequently experienced an unplanned extubation. Multiple attempts at reintubation were unsuccessful with cricothyrotomy providing lifesaving. The patient was then fiberoptically intubated under controlled conditions sparing the victim from what otherwise would probably have been a tracheostomy.

Twenty-four patients suspected to have sarcoidosis were subjected to fibrebronchoscopy. Histopathological support for the diagnosis was ultimately obtained in 20 patients. Fibrebronchoscopy provided the diagnosis in 17 patients, while histopathological confirmation was obtained from extrapulmonary biopsy sites in 3 patients. The diagnostic efficacy of routine double-contrast barium enema and fiberoptic coloscopy for detection of cancer was retrospectively studied in 303 patients consecutively operated for colorectal adenocarcinoma in a 52-month period from January 1980. Double-contrast barium enema was performed in 180 patients with 184 carcinomas, 157 (85%) of which were revealed by this examination. Twenty-three pediatric patients with blunt esophageal foreign bodies underwent flexible endoscopic removal without general anesthesia. No object was in place longer than 14 days. In 18 patients the foreign body was successfully removed, while in five patients the object was dislodged into the stomach. All of the latter patients were encountered early in the series. No complications were encountered in any patients. This technique appears to be a safe alternative to other proposed methods of blunt esophageal foreign-body removal.

In critically ill patients, measurement of mixed venous oxygenation is useful as an indicator of the adequacy of tissue oxygenation and as an indirect reflection of cardiac output. The flexible fiberoptic pulmonary artery catheter oximeter permits continuous in vivo measurements of the mixed venous oxygen saturation. However, in some groups of critically ill patients, such as those with adult respiratory distress syndrome or sepsis, measurements of mixed venous oxygen may be misleading and should be interpreted with caution.

Where epiphora results from canalicular obstruction, it is usually possible to cure the patient by reconstructing the canaliculi themselves. Resort to artificial bypass tubes is thus not often necessary. This paper discusses the role of the operating microscope in facilitating reconstruction of the lacrimal canaliculi so that longer segment of obstructed canaliculus can be reopened and more accurate reconstitution of the anatomy achieved. The place of fibre optics is outlined. Easier techniques for repairing lacerated canaliculi are described.

Systematic performance of fibroduodenoscopy in patients with diseases of bile tracts and the pancreas made it possible in 90 cases to reveal polyps of papilla duodeni major, which were the cause of chronic recurrent pancreatitis. A 38-year-old man who developed abdominal distention and pain following fiberoptic gastroscopy is described. The patient had had an upper gastrointestinal roentgenographic series earlier, and the dilatation of the colon was thought to be due to entrapment of air in the proximal colon secondary to inspissated barium in the descending colon. Premedication with meperidine hydrochloride and atropine also may have been contributory. A saline enema relieved the patient's symptoms and a barium enema examination later showed a normal colon.
