Platelet-rich plasma (PRP) and Platelet-Rich Fibrin (PRF): surgical adjuvants, preparations for in situ regenerative medicine and tools for tissue engineering.

The recent developement of platelet concentrate for surgical use is an evolution of the fibrin glue technologies used since many years. The initial concept of these autologous preparations was to concentrate platelets and their growth factors in a plasma solution, and to activate it into a fibrin gel on a surgical site, in order to improve local healing. These platelet suspensions were often called Platelet-Rich Plasma (PRP) like the platelet concentrate used in transfusion medicine, but many different technologies have in fact been developed; some of them are even no more platelet suspensions

Chitin nanowhiskers based on Periplaneta americana residue for preparing deep eutectic solvent (DES)-thrombin bio-adhesive with hemostatic effect.

In this study, chitin nanowhiskers (ChNWs) were extracted and prepared from discarded Periplaneta americana residues after extraction. These ChNWs were used as the polymeric matrix of a new bio-adhesive, while a biocompatible aqueous deep eutectic solvent (DES) served as the system medium. Thrombin (T) was incorporated as the hemostatic agent, and gelatin (G) was added to improve the overall mechanical properties. In above system, the relative activity of thrombin in 30% DES increased to 116.12%. The prepared ChNWs exhibited stable dispersion and crystallinity exceeding 90%, and the adhesive s

Rapid Forming, Robust Adhesive Fungal-Sourced Chitosan Hydrogels Loaded with Deferoxamine for Sutureless Short-Gap Peripheral Nerve Repair.

Clinically, conventional sutures for repair of short-distance nerve injuries (< 5 mm) may contribute to uncontrolled inflammation and scar formation, thus negatively impacting nerve regeneration. To repair transected peripheral nerves with short distances, a rapid-forming, robust adhesive chitosan hydrogel is prepared by synthesizing maleic and dopamine bi-functionalized fungal-sourced chitosan (DM) and subsequently photopolymerizing DM precursor solution. The hydrogel rapidly polymerized under UV light irradiation (≈2 s) and possessed a strong adhesive strength (273.33 ± 55.07 kPa), facilitat

Platelet concentrates in diabetic foot ulcers: A comparative review of PRP, PRF, and CGF with case insights.

Diabetic foot ulcers (DFUs) are severe complications of diabetes, often leading to chronic wounds, amputations, and increased mortality risk. Platelet concentrates (PCs)-natural biomaterials utilized in regenerative medicine-have garnered attention for their capacity to enhance tissue repair and wound healing. This study reviews the preparation methods, biological mechanisms, and clinical efficacy of three generations of PCs: platelet-rich plasma (PRP), platelet-rich fibrin (PRF), and concentrated growth factors (CGF). Comparative analysis reveals that PRP, the first generation, provides abund

Long-term Investigation of Annulargrams and Intra-annular Fibrin to Treat Chronic Discogenic Low Back Pain and Radiculopathy: 1-, 2-, and 3-Year Outcome Comparisons of Patients with and without Prior Surgery.

BACKGROUND: Discogenic chronic low back pain (cLBP) and radiculopathy are the most prevalent causes of disability worldwide. Older spine treatments often lack reliability and are associated with adverse events. Among surgical treatment options, discectomies weaken discs, and fusions cause direct damage to adjacent discs, so both treatments accelerate disc degeneration. Other regenerative medicine treatments, including "stem cell" (centrifuged bone marrow aspirate, BMC), and platelet-rich plasma (PRP), lack fibrin's bio-adhesive properties. Specifically, fibrin is a strong bio-adhesive, so it i

Modified fibrin sealant-cellulose efficacy on wound healing: Histopathological study.

BACKGROUND: Wound healing takes a long time. To heal correctly, cells need specific conditions. Natural materials may assist cells in faster wound healing. AIM: This study aimed to test the healing qualities of a modified fibrin sealant containing cellulose isolated from date palm fronds (Phoenix dactylifera). METHODS: Twelve local dogs were used, and they were divided into two groups: 1: control, administered normal saline; 2: test, treated with the fibrin-cellulose mixture. Each dog was given a circular wound of 20 mm. The assigned therapy was administered for 7 consecutive days. On days 7

Treatment of dermal ulcer with autologous fibrin glue: Two case reports of an exploratory prospective pilot study.

INTRODUCTION: The healing of recurrent and refractory skin ulcers requires a long time, during which there is risk of infection, and hospital admission is occasionally required for surgical or daily conservative treatment. Therefore, the development of promising treatments that promote faster, uneventful healing is a must. Composed of cryoprecipitate and thrombin, fibrin glue has a history of surgical use for preventing bleeding and spinal fluid leakage. Moreover, in-house cryoprecipitates contain higher concentrations of coagulation factors and cytokines that may enhance wound healing than co

Fibrin glue assisted amniotic membrane graft reconstruction of conjunctival fornix and canthus following excision of conjunctival tumours.

Conjunctival tumors involving non-limbal locations, such as the fornix and canthus, are typically excised using a "non-touch" technique, often with a wide surgical margin. Reconstruction of these large defects can be difficult due to the contour of the ocular surface and are often complicated by shortening of the fornix, symblepharon formation, and restriction of eye movements. In our experience, the use of amniotic membrane grafts combined with the sealant properties of fibrin glue such as Tisseel® has improved our surgical outcomes during the reconstruction phase. We would like to highlight

Fibrin glue coating of the surgical surfaces may facilitate formation of a successful bleb in trabeculectomy surgery.

Trabeculectomy is commonly conducted when medical therapy fails to control intraocular pressure (IOP). The success of trabeculectomy for the treatment of glaucoma depends on the wound-healing response at the subconjunctival filtering bleb site. Postoperative scar formation is a serious problem in this surgery. Current strategies to counteract scarring include local antimetabolite treatment, which is associated with severe side effects, limiting its application. Therefore, additional means to safely modulate wound healing are desirable. In ophthalmic surgery, fibrin glue is used mainly for seal

Conjunctival closure in strabismus surgery: Vicryl versus fibrin glue.

Six patients undergoing bilateral symmetric strabismus surgery were enrolled in this study. For each subject, incisions were closed with Vicryl in one eye and glue in the other. The conjunctival closure with the Vicryl suture resulted in increased discomfort and inflammation during the early postoperative period compared with fibrin glue. However, the difference between the Vicryl and the glue disappeared after 14 days.

Factors Predictive of Treatment Success in CT-Guided Fibrin Occlusion of CSF-Venous Fistulas: A Multicenter Retrospective Cross-Sectional Study.

BACKGROUND AND PURPOSE: CSF-to-venous fistulas contribute to spontaneous intracranial hypotension. CT-guided fibrin occlusion has been described as a minimally invasive treatment strategy; however, its reproducibility across different institutions remains unclear. This multi-institution study evaluated the clinical and radiologic outcomes of CT-guided fibrin occlusion, hypothesizing a correlation among cure rates, fibrin injectate spread, and drainage patterns. MATERIALS AND METHODS: A retrospective evaluation was conducted on CT-guided fibrin glue treatment in patients with CSF-to-venous fis

Novel nonbiological surgical sealant to control air leakage after lung resection.

OBJECTIVES: Pulmonary air leakage is a major complication in patients undergoing lung resection, particularly with the increasing use of segmentectomy in Japan. We developed a non-biological polyethylene glycol (PEG) hydrogel sealant to control pulmonary air leakage. This study compared the adhesiveness, expansion, and safety of PEG hydrogel with those of fibrin glue. METHODS: Adhesiveness was evaluated using isolated ventilated pig lungs subjected to a stepwise increase in airway pressure. A pleural defect was created, and PEG hydrogels with elastic moduli (G') = 38.2, 646, 3900, and 7780 Pa

A Dual-Layer Hydrogel Barrier Integrating Bio-Adhesive and Anti-Adhesive Properties Prevents Postoperative Abdominal Adhesions.

Postoperative abdominal adhesions are a common and painful complication after surgery, leading to high healthcare costs and diminished quality of life. This report presents a novel bilayer hydrogel barrier featuring an inner adhesive layer and an outer antiadhesive layer. The inner adhesive layer hydrogel (PT) is prepared by mixing polyethyleneimine (PEI) and thioctic acid (TA). The outer layer (HP) hydrogel is fabricated by the conjugation reaction of thermoresponsive zwitterionic hyaluronic acid, phenylboronic acid, and epigallocatechin gallate complex and polyvinyl alcohol based on dynamic
