Diuretic therapy in heart failure: current controversies and new approaches for fluid removal.

Hospitalization for heart failure is a major health problem with high in-hospital and postdischarge mortality and morbidity. Non-potassium-sparing diuretics (NPSDs) still remain the cornerstone of therapy for fluid management in heart failure despite the lack of large randomized trials evaluating their safety and optimal dosing regimens in both the acute and chronic setting. Recent retrospective data suggest increased mortality and re-hospitalization rates in a wide spectrum of heart failure patients receiving NPSDs, particularly at high doses. Electrolyte abnormalities, hypotension, activatio

Spontaneous cerebrospinal fluid rhinorrhea in untreated macroprolactinoma--an indication for primary surgical therapy.

BACKGROUND: Medical therapy is usually indicated as first-line treatment for prolactinomas. Surgery is generally reserved as second-line therapy if prolactinomas are non-responsive to dopamine agonists (DA) or DA therapy is not tolerated. Herein, we draw attention to the rare occurrence of spontaneous CSF rhinorrhea in prolactinomas requiring primary surgical therapy. Only 8 cases of confirmed prolactinomas with spontaneous rhinorrhea have been reported in the literature so far. CASE REPORTS: Two out of 267 surgical cases with pituitary adenomas presented with spontaneous rhinorrhea. Both pat

Cerebrospinal fluid leak and meningitis associated with nasal continuous positive airway pressure therapy.

Clear rhinorrhea is a common symptom in patients with obstructive sleep apnea (OSA) and may worsen with continuous positive airway pressure therapy. Clear rhinorrhea can also be the presenting symptom of cerebrospinal fluid (CSF) leak, which is evidence of a communication between the subarachnoid space and the nasal cavity or sinuses. While CSF leak has been reported to occur with nasal continuous positive airway pressure (nCPAP) therapy following trauma to the skull base, its association with OSA and nCPAP therapy in the absence of trauma has not been previously described. We report two patie

A randomised controlled trial of prehospital intravenous fluid replacement therapy in serious trauma.

RESULTS: In total 1309 patients were entered in the study: 699 (53.4%) were treated by paramedics operating protocol A and 610 (46.6%) were treated by paramedics operating protocol B. The randomisation worked well and there were no significant differences between treatment groups in incident characteristics, ambulance performance times, or patient or injury characteristics, apart from slightly more moderate or severe head injuries in the protocol A group (25.3% versus 20.3%). Protocol compliance was poor, with only 31% of protocol A patients receiving prehospital fluids and only 80% of protoco

Central and regional hemodynamics during crystalloid fluid therapy after uncontrolled intra-abdominal bleeding.

OBJECTIVE: To study the effect of graded crystalloid fluid resuscitation on central hemodynamics and outcome after intra-abdominal hemorrhag … d 3:1 fluid programs. Survival was highest with the 1:1 and 2:1 programs. CONCLUSIONS: Crystalloid fluid therapy improved the hemodynamic status but increased the risk of rebleeding. Therefore, a moderate fluid program offered the best chance of survival.

Optimising fluid therapy during venoarterial extracorporeal membrane oxygenation: current evidence and future directions.

Venoarterial extracorporeal membrane oxygenation (VA-ECMO) offers an immediate and effective mechanical cardio-circulatory support for critically ill patients with refractory cardiogenic shock or selected refractory cardiac arrest. As fluid therapy is routinely performed as a component of initial hemodynamic resuscitation of ECMO supported patients, this narrative review intends to summarize the rationale and the evidence on the fluid resuscitation strategy in terms of fluid type and dosing, the impact of fluid balance on outcomes and fluid responsiveness assessment in VA-ECMO patients. Severa…

Endoscopic Therapy for Pancreatic Fluid Collections: A Definitive Management Using a Dedicated Algorithm.

BACKGROUND/AIMS: Endoscopic ultrasonography (EUS)-guided drainage is the preferred approach for infected or symptomatic pancreatic fluid collections (PFCs). Here, we developed an algorithm for the management of pancreatitis complicated by PFCs and report on its effcacy and safety. METHODS: Between September 2011 and October 2017, patients were prospectively managed according to the algorithm. PFCs were classified as poorly organized fluid collections (POFCs), pancreatic pseudocysts (PPs), or walled-off pancreatic necrosis (WOPN). Clinical success was defined as a decrease in PFC size by ≥50%

Continuous renal replacement therapy in children: fluid overload does not always predict mortality.

BACKGROUND: Mortality among critically ill children requiring continuous renal replacement therapy (CRRT) is high. Several factors have been identified as outcome predictors. Many studies have specifically reported a positive association between the fluid overload at CRRT initiation and the mortality of critically ill pediatric patients. METHODS: This study is a retrospective single-center analysis including all patients admitted to the pediatric intensive care unit (PICU) of our hospital who received CRRT between 2000 and 2012. One hundred thirty-one patients were identified and subsequently

Intrathecal enzyme replacement therapy: successful treatment of brain disease via the cerebrospinal fluid.

Treatment of brain disease with recombinant proteins is difficult due to the blood-brain barrier. As an alternative to direct injections into the brain, we studied whether application of high concentrations of therapeutic enzymes via intrathecal (IT) injections could successfully drive uptake across the ependyma to treat brain disease. We studied IT enzyme replacement therapy with recombinant human iduronidase (rhIDU) in canine mucopolysaccharidosis I (MPS I, Hurler syndrome), a lysosomal storage disorder with brain and meningeal involvement. Monthly or quarterly IT treatment regimens with rhI

[Cerebrospinal fluid filtration as experimental therapy in therapy refractory psychoses in Borna disease virus seropositive patients. Therapeutic effects, findings].

According to previous investigations, mild Borna disease virus encephalitis may underlie a subgroup of affective or schizophrenic type psychoses. And virus-induced immune pathology may underlie even a larger subgroup of psychoses. We treated BDV seropositive patients suffering from therapy resistant schizophrenic or affective spectrum psychoses by cerebrospinal fluid filtration (CSFF) in an experimental add-on treatment. CSFF was shown previously to be an effective immune modulatory treatment in autoimmune neurological disease, Guillain-Barré syndrome. CSFF appears to be an effective treatment

Balanced multi-electrolyte solution versus normal saline for fluid therapy in aneurysmal subarachnoid hemorrhage: an influence on fluid and electrolytes balance and outcome.

INTRODUCTION: Isotonic crystalloids are commonly used for maintaining fluid balance and cerebral perfusion pressure in critical care patients with aneurysmal subarachnoid hemorrhage (aSAH). However, the relatively high concentration of chloride in normal saline (NS) might lead to hyperchloremia or acute kidney injury, comparing with multi-electrolyte solutions (BMES). The aim of the study is to compare the incidence of hyperchloremia in aSAH patients and provide feasibility and safety research for further study. METHODS: This is a pilot study of a single center, randomized, controlled trail.

Effect of isotonic versus hypotonic maintenance fluid therapy on urine output, fluid balance, and electrolyte homeostasis: a crossover study in fasting adult volunteers.

BACKGROUND.: Daily and globally, millions of adult hospitalized patients are exposed to maintenance i.v. fluid solutions supported by limited scientific evidence. In particular, it remains unclear whether fluid tonicity contributes to the recently established detrimental effects of fluid, sodium, and chloride overload. METHODS.: This crossover study consisted of two 48 h study periods, during which 12 fasting healthy adults were treated with a frequently prescribed solution (NaCl 0.9% in glucose 5% supplemented by 40 mmol litre -1 of potassium chloride) and a premixed hypotonic fluid (NaCl 0.
