Cardiotoxicity associated with 5-fluorouracil (FU) is an uncommon, but potentially lethal, condition. The case of an 83-year-old man with colon cancer who developed chest pain during 5-FU infusion is presented. The electrocardiogram (ECG) showed pronounced ST elevation in the lateral leads, and the chest pain was resolved after infusion of nitroglycerin. Topical 5-fluorouracil (5-FU) has been used for the treatment of dermatological conditions for nearly half a century. Although the indications for 5-FU approved by the US Food and Drug Administration include actinic keratosis and superficial basal cell carcinoma, studies also have demonstrated therapeutic efficacy for 5-FU in the treatment of many other dermatological conditions, including squamous cell carcinoma in situ, warts, psoriasis of the nail, keratoacanthoma, and vitiligo. This review discusses clinical data which support the use of 5-FU for these disease states.

PURPOSE: A 53-year-old female patient was treated with combined radiochemotherapy with 5-fluorouracil (5-FU) because of adenocarcinoma of the lung. She died after the 2nd day of the first course on fatal myocardial infarction. The histological evaluation of the heart revealed no severe chronic fibrosis. Seventy nonpregnant women with extensive condylomata of the vagina were treated either with intravaginal 5-fluorouracil (5-FU) cream or the CO2 laser. Treatment results were correlated with the morphologic type of condylomata, ie, papillary acuminata (58 patients), and flat condylomata (12 patients). Ten and 31% of women with Condylomata acuminata had persistent disease within nine months after a single course of 5-FU and laser therapy, respectively. In two patients with liver metastases from colorectal cancer an isolated perfusion of the liver with 5-fluorouracil was performed. The primary tumors had been resected 5 weeks and 11 months before the perfusion treatment. By means of special cannulation system the hepatic artery and the portal vein were both arterialized. The patients recovered well after the operation. Sonographic measurements of the metastases showed colliquation areas in the tumours postoperatively.

Seventeen patients, after abnormal Papanicolaou smears, were found to have vaginal lesions ranging from moderate dysplasia to carcinoma in situ. They were evaluated by colposcopy and biopsy.
