Potential renal solute load of infant formulas.

The potential renal solute load (PRSL) of infant feedings is the sum of dietary nitrogen (expressed as mmol of urea, i.e., mg nitrogen divided by 28), sodium, potassium, chloride and phosphorus. The PRSL determines the renal solute load, and, therefore, the osmolar concentration of the urine. When water intake is reduced and/or water losses are increased, the renal concentrating ability may be exceeded, and negative water balance (dehydration) may ensue. Under these circumstances, feedings providing high PRSL lead more rapidly to dehydration than do feedings providing lower PRSL. On the basis

Chronic protracted diarrhea of infancy: a nutritional disease.

Diarrhea is an extremely common cause of morbidity in infancy. Occasionally, it becomes protracted, leading to a vicious cycle of malabsorption, malnutrition, and failure to thrive. A number of causes of chronic diarrhea in infancy are discussed, including postinfectious enteritis, celiac sprue, cow's milk allergy, and parasitic infection. Although many mechanisms may contribute to diarrhea, a similar pathophysiologic syndrome of mucosal atrophy, inflammation, and malabsorption results. Attention should be paid to recognition of malnutrition as well as etiologic diagnosis. Therapeutic efforts

The effects of FG 7142 on sensory-specific satiety in rats.

Three experiments used intake, clusters, and licks per cluster to study the effects of the GABA inverse agonist, FG 7142, on sensory-specific satiety in rats. In Experiment 1, rats were offered one of two palatable solutions and 30min later tested with the same or the other solution. Rats that received the same solution consumed less, exhibited fewer clusters, fewer licks per cluster and a more rapid decline in rate of licking than rats tested with the other solution. In Experiments 2 and 3, rats tested with the same solution under FG 7142 showed fewer clusters and fewer licks per cluster than

Rationale for Use of Micronized Isotretinoin for Treatment of Acne Vulgaris: Practical Considerations and Therapeutic Advantages.

OBJECTIVE: Isotretinoin is a widely used and clinically efficacious treatment for severe, recalcitrant, nodular acne vulgaris (AV). Clinicians are generally familiar with safety concerns regarding isotretinoin use, especially teratogenicity. However, there are specific "real-world" challenges with use of conventional formulations based on the original isotretinoin formulation, including poor solubility and food-dependent absorption requiring high fat intake with each dose. This review describes the development and use of new isotretinoin formulations and their potential to improve long-term ou

Bixin stability enhancement through novel nanoemulsion system for advancing its applications.

Bixin is a highly demanded apocarotenoid known for its impeccable coloring attributes in the food industry. Nonetheless, bixin is highly susceptible to oxidative degradation under high temperature, light, and pH, rendering its application obscure. Although there are stability concerns, limited investigations have been reported on the systematic approaches in augmenting the stability of bixin over the years. Hence, a novel water-in-oil emulsion system was formulated to encapsulate bixin, thereby conferring stability for its widespread food applications. The formulated emulsion was assessed for

Experience with the management of patients requiring home enteral nutrition in routine clinical practice in Spain: the ECONES study.

Introduction: enteral nutrition is an effective method for restoring the nutritional status in patients unable to eat or fulfil nutritional requirements orally. Objectives: the ECONES study aimed to describe the treatment of patients requiring home enteral nutrition (HEN) in routine practice and the experience of specialists with the high-protein hypercaloric formula 2.0 with fiber IS50 (HP/HC 2.0). Methods: Spanish specialists answered a 38-questions survey about their last six patients with HEN and their treatment with HP/HC 2.0 formula. Results: the formulas were chosen based on the patient

Geographic Atrophy and micronutritional supplements: A complex relationship.

In 2019, the Age-Related Eye Disease Study (AREDS) remains the basis for micronutritional supplement guidelines aiming to slow the progression of Age-Related Macular Degeneration (AMD) and reduce the risk of neovascularization. However, for Geographic Atrophy (GA) patients specifically, there seem to be more arguments for prescribing micronutritional supplements for the prevention of neovascular complications than to slow the progression of the atrophy. The AREDS report 8 showed a significant decrease in AMD progression over a five-year follow-up associated with a formulation containing antiox

Metastatic mineralisation caused by vitamin D intoxication in hand-raised short-beaked echidnas (Tachyglossus aculeatus).

CASE REPORT: Two cases of metastatic mineralisation caused by dietary excess of vitamin D are reported in hand-raised short-beaked echidna (Tachyglossus aculeatus) puggles at the Australian Wildlife Health Centre. Oversupplementation was attributed to excessive levels of vitamin D in Wombaroo Echidna Milk Replacer fed to the puggles. No information exists regarding the naturally occurring vitamin D levels in echidna milk, but, given the low serum levels that have been observed in free-ranging animals, it is likely to be low. CONCLUSION: The vitamin D concentration in Wombaroo Echidna Milk Rep

Intestinal lactose and mineral concentration affect the microbial ecophysiology along the gastrointestinal tract of formula-fed neonatal piglets.

Hyperprolificacy in modern pig breeds has led to increased use of artificial rearing and formula feeding of neonatal piglets, which may change their intestinal bacterial ecophysiology. Here, newborn piglets ( = 8 per group) were fed a bovine milk-based formula (FO) or allowed to suckle their mothers (sow milk [SM]) for 2 wk, and digesta samples from the stomach, jejunum, and colon were subsequently analyzed for enzyme activities, bacterial metabolites, and 16S rRNA transcripts of bacterial groups by quantitative real-time PCR. Jejunal lactase activity was lower and lactose concentration was gr

Tyrosinemia type 1: a rare and forgotten cause of reversible hypertrophic cardiomyopathy in infancy.

BACKGROUND: Tyrosinemia type 1 (TT1) is an autosomal recessive disorder caused by deficiency of the enzyme fumarylacetoacetate hydrolase (FAH). TT1 usually presents in infancy with features suggestive of liver disease or with sepsis-like symptoms. CASE PRESENTATION: We report two Saudi siblings with TT1. Case 1 was a male infant who presented at 2 months old with fever, vomiting and refusal of feeding. Examination revealed a sick-looking infant with signs of severe dehydration and hypovolemic shock. He was jaundiced, and had hepatomegaly and elevated liver enzymes. Echocardiography was perfor

Guidelines for specialized nutritional and metabolic support in the critically-ill patient: update. Consensus SEMICYUC-SENPE: severe acute pancreatitis.

Severe acute pancreatitis (SAP) causes local and systemic complications leading to high catabolic, hypermetabolic and hyperdynamic stress states with marked morbidity and mortality. In the last decade, nutritional support has become a key element in the treatment of SAP. Thus, specialized nutrition is indicated from admission, with enteral nutrition being preferred to parenteral nutrition. Enteral nutrition should be initiated early using infusion through the jejunum beyond the ligament of Treitz to minimize pancreatic stress. There are no specific studies that establish the type of diet to be

A non-terminal surgical procedure for chronic collection of exocrine pancreatic secretions from unrestrained dogs (Canis familiaris).

The ability of dogs to adaptively modulate secretion by the exocrine pancreas to match changes in the amounts and sources of macronutrients is poorly understood. We evaluated the use of re-entrant pancreatic catheters as a non-terminal, temporary approach for the chronic collection of exocrine pancreatic secretion using unrestrained dogs fed diets differing in composition. Re-entrant catheters were surgically placed in the accessory pancreatic duct of two adult mongrel dogs. Secretions were collected for 40 days, during which the dogs were fed three diets with different amounts and sources of

Nutrition in chronic critical illness.

Nutritional management of patients with respiratory failure can be a model of nutritional management in chronically critically ill patients. This model requires recognition of the differing metabolic states of starvation and hypermetabolism. Starvation can result in malnutrition, with adverse effect on respiratory muscle strength, ventilatory drive, and immune defense mechanisms. General nutritional goals include preservation of lean body mass by providing adequate energy and positive nitrogen balance. General nutritional prescriptions for both states include a substrate mix of 20% protein, 60

Clinical and physiological consequences of rapid tryptophan depletion.

We review here the rapid tryptophan depletion (RTD) methodology and its controversial association with depressive relapse. RTD has been used over the past decade to deplete serotonin (5-hydroxy-tryptamine, or 5-HT) in humans and to probe the role of the central serotonin system in a variety of psychiatric conditions. Its current popularity was stimulated by reports that RTD reversed the antidepressant effects of selective serotonin reuptake inhibitors (SSRIs) and monoamine oxidase inhibitors (MAOIs) in remitted patients with a history of depression but not in patients treated with antidepressa

[Menu design for total parenteral nutrition: simplified model derived from a computerized system].

After the design and use of a computerized system for the prescription of parenteral nutrition, the authors drew up clinical choice tables, as well as others of a pharmaceutical elaboration, based on the same algorithm of the computer program. The conclusion reached was that the nutrition selection process based on these tables is simpler, saves more time, is based on a logical plan of decision making and does not need a computer support.

[Artificial nutrition in bone marrow transplantation].

The issue of nutritional support in oncologic patients is reviewed. Special reference is made in relation to patients undergoing bone marrow transplant and their possible digestive tract and nutritional changes induced by the different therapies available in these situations. Nutritional evaluation and caloric and nitrogen requirement calculations have been proposed. Likewise, several possibilities have been considered regarding the route of administration, timing and quality of the nutriments to be administered, both to adult and pediatric patients.

Evaluation of a phenylalanine-free product for treatment of phenylketonuria.

Ten children with classic phenylketonuria (PKU) participated in a controlled study of a phenylalanine-free formula recently released in the United States (PKU-2). Control data were obtained in the clinic while the children were receiving their baseline formula. The children were given the study formula and returned to the clinic for follow-up after they had been receiving the new formula for 4, 8, and 12 months. Serum phenylalanine and tyrosine concentrations, other hematologic measurements, urine analysis, growth, electroencephalogram, and physical findings remained similar to baseline values
