Collapsing pes plano valgus deformity and pes cavus deformity have been identified and treated for many years. Recently, a combination of these deformities has been recognized occurring simultaneously. This multiplanar deformity is known as the cavo-valgus foot. Treatment of this foot type requires a firm understanding of planal dominance. The authors present a case report on this subject.

Foot injuries constitute a spectrum of problems that can be classified by severity. The development of successful techniques for the treatment of lower leg injuries has made the severity of a concomitant foot injury a key factor in determining the overall salvageability of the leg. A more complete classification of foot injuries is therefore needed and has been proposed. Preoperative assessment of foot injuries differs in the acute versus the delayed presentation. The acute case requires evaluation of wound conditions, exposed structures, and associated proximal injuries. The chronic injury re…

A general approach to the child with foot pain has been described. In that the pediatrician is so often the recipient of parent's questions about children's shoes, he or she should be aware of problems relating to poorly fitting shoes, probably the most common source of complaints of foot pain in children. With a systematic approach, the pediatrician is very capable of managing the majority of such complaints.

Previous reports have suggested that the blood supply derived from the anterior tibial artery is absent or markedly diminished in 85% of severe, untreated club feet. To investigate these claims, we used a Doppler technique to study the arterial pulses in 40 children with 63 club feet. In feet with mild or moderate deformities the anterior tibial pulse was always present; in feet with severe deformities it was absent in two out of 30 feet in children under three years and in seven out of 18 feet in children over three years. These results confirm that the incidence of pulselessness increases wi

Redness and scaling of the feet in prepubertal children is likely to represent a skin disease other than tinea pedis. If the dermatitis involves the dorsum of the feet, allergic contact dermatitis is a likely cause. In this study, 8/19 (42%) children with involvement of the dorsum of the feet had clinically relevant positive patch tests related to shoe allergens. All who avoided the allergens remained free of disease on a 2-year follow-up examination. Those children with dermatitis on the weight-bearing surface of the foot (juvenile plantar dermatosis) did not have positive patch tests (P = .1…

Arteriography was done on fourteen children with nineteen typical, severe, untreated club feet. The age range of the patients was from 1.3 to 13.5 years (average, 6.3 years). Only three feet were found to have normal arterial anatomy. In the remaining sixteen feet the posterior tibial artery was the dominant structure; the anterior tibial artery was hypoplastic and ended at the ankle, so that no dorsalis pedis artery was present. After operative correction of the feet, no change was noted in the arteriograms of three patients. The importance of protecting the posterior tibial artery during ope

The congenital clubfoot of infancy to-day is curable in 90% of cases. Aim of the treatment is normalization of form, function and growth. With combined therapy according to Imhäuser optimal results are possible: 1. Immediately after birth--depending on the general condition of the newborn--reposition of all components without the talipes equinus component; plastercasts are changed frequently till the third or fourth month of life; 2. surgical development of the heel; calcaneal equinus is corrected by elongating the tendocalcaneus combined with arthrolysis of the ankle joint; 3. intensive clini…

Foot and ankle injections are commonly performed by family physicians for the management of musculoskeletal conditions such as plantar fasciitis, osteoarthritis, interdigital neuromas, tarsal tunnel syndrome, and pathology of the first metatarsophalangeal joint. If symptoms persist despite conservative measures, injections may improve function and reduce pain. Before an injection, the clinician should discuss procedural risks, including pain, swelling, redness, stiffness, infection, and damage to surrounding tissues, and obtain informed consent. Corticosteroid injections can provide short-term…

The National Diabetes Statistics in 2021 estimated that 38.4 million people have diabetes. The lifetime risk of developing a foot ulcer in patients with diabetes is more than 33%. About two-thirds of diabetic foot ulcers will eventually heal, but approximately 28% may result in some form of lower extremity amputation. Advanced wound care products, offloading techniques, minor surgeries, and revascularizations still leave wounds unhealed and require amputation. Research has pointed out that the dysfunction in the microcirculation may contribute to these failures. This article discusses techniqu…

Multiple pathophysiologic and biomolecular processes lead to bullae, including disruption of adhesion molecules, accumulation of cell injury, and traumatic injury. Blistering disorders of the foot can cause symptoms such as pruritus, pain, and drainage and significantly impact quality of life. Microbiologic and histopathologic examination of tissue specimens should be considered for concerns regarding atypical etiology. This retrospective case series describes patients seen in a community hospital outpatient wound center in southeastern Wisconsin between January 2021 and June 2023 for atypical …
