[Application of the pedicle screws for cervicothoracic fracture-dislocation].

OBJECTIVE: To explore pedicle screw implantation for fracture-dislocation injuries at cervicothoracic junction (C6-T2). METHODS: Pedicle screw implantation was used for twenty-six patients with cervicothoracic fracture-dislocation from May 2001 to Jan 2008. There were 17 males and 9 females with an average of 48.5 years (range,20 to 75 years). Single posterior pedicle screw fixation was performed for 17 cases, and posterior screw fixation combined with anterior reduction and plate fixation for the other nine. The accuracy of the pedicle screws were evaluated by CT views after surgery. Complic

Transstyloid, translunate fracture-dislocation of the wrist: case report.

Earlier reports have described characteristic fracture-dislocation patterns of the carpus and distal radius. Most result from a fall onto an outstretched arm, with an applied force directed in a radial-to-ulnar direction across the wrist. We present the clinical, radiographic, and arthroscopic findings noted with an unusual pattern of injury resulting from a force applied at the ulnar side of the wrist.

Complete fracture-dislocation of the thoracolumbar spine without paraplegia.

Complete fracture-dislocation of thoracolumbar spine without significant neurologic deficits is a rare entity. The possible mechanism may result from the spontaneous decompressive fractures of the spinal elements with sparing of the spinal cord. Here, we report a case of a healthy 50-year-old man who presented with complete fracture-dislocation of T12 through L1 vertebrae without paraplegia. Early diagnosis and early surgery with decompression, reduction, and stabilization is the best policy in management of this unstable spine injury.

Percutaneous closed reduction of fracture-dislocation of the shoulder.

A technique using a cannulated tap in a percutaneous manner allows for easy reduction of the dislocated humeral head with minimal soft-tissue trauma and presents a useful alternative to repeated attempts at open or closed reduction.

Bilateral posterior fracture dislocation of the shoulders following seizure.

Bilateral posterior fracture dislocation is a rare injury known to be associated with seizures. Convulsion was found to be the cause of fracture dislocation in 78% of the cases reported. The mechanism of injury was described by Shaw in 1971. The management depends largely on the severity of the injury. In many cases reported, the fracture was a large compression defect in the anteromedial aspect of the articular surface of the humeral head. It has been suggested that for defects that involve less than 20% of the articular surface closed reduction can be attempted. Rush nail or percutaneous K w…

Locked lateral patellar dislocation with impaction fracture of patella.

Irreducible lateral patellar dislocation is rare. It has previously been described with rotation around a vertical axis. We describe a case of irreducible lateral patellar dislocation in a 66-year-old lady with an impaction fracture of medial facet of patella locked on the lateral femoral condyle. Closed reduction was unsuccessful and the patient was treated by total knee replacement. Such a case has not been described previously in the literature.

Hamatometacarpal fracture-dislocation: distinctive three dimensional computed tomographic appearance.

We report here two fully documented cases of hamatometacarpal fracture-dislocation following trauma and treated in our hospital. In our cases, the patients suffered hamate fracture in association with metacarpal dislocation. In the first case, a dorsal oblique fracture of the hamate was associated with a dorsal dislocation of the base of the fourth metacarpal. In the second case, a dorsal oblique fracture of the hamate was not associated with a dorsal dislocation of the base of the fifth metacarpal. This diagnosis should be suspected on initial review of plain radiographs, which must include a

Initial closed reduction of cervical spine fracture-dislocation injuries.

STANDARDS: There is insufficient evidence to support treatment standards. GUIDELINES: There is insufficient evidence to support treatment g … l attempts at closed reduction is recommended. Prereduction MRI performed in patients with cervical fracture dislocation injury will demonstrate disrupted or herniated intervertebral discs in one-third to one-half of patients with facet subluxation. These findings do not seem to significantly influence outcome after closed reduction in awake patients; therefore, the u …

Traumatic posterior rotatory fracture-dislocation of lumbo-sacral spine.

Fracture-dislocation of the lumbo-sacral spine was an unusual injury and was divided into anterior, posterior and lateral types depending on the displacement of the cephalad portion of the spine over the caudal portion. According to the authors' knowledge, only 31 cases of traumatic fracture-dislocation of the lumbo-sacral spine were reported in the English literature. Only 3 previous reports referred to this injury with a posterior displacement, which was an even rarer injury. This was the fourth report of this type of injury.

Hinged elbow fixation for recurrent instability following fracture dislocation.

The purpose of this study was to evaluate the use of an articulated external fixator of the elbow in the management of instability after fracture dislocation. We retrospectively reviewed results of eight patients treated with an articulated external fixator between 1998 and 1999. Study inclusion criteria included recurrent/chronic dislocation following fracture dislocation. Patients were divided into two groups based on the onset of instability. Group I (n=3) included patients with acute instability. The indication for use of the articulated external fixator in this subset of patients was the …

Asymmetrical bilateral traumatic hip dislocation with ipsilateral acetabular fracture.

We report a case of simultaneous asymmetrical bilateral traumatic hip dislocation, with one hip dislocated anteriorly and the other posteriorly, with ipsilateral acetabular fracture, suffered in a traffic accident by a 36-year-old man. Closed reduction of both hips was performed, followed by delayed internal fixation of the acetabular fracture.

An intra-articular fracture-dislocation of the acromioclavicular joint.

A hybrid acromioclavicular joint fracture-dislocation in which an intra-articular fracture of the distal clavicle coexisted with a coracoclavicular ligament injury is described. The proposed mechanism of injury is a fall on the point of the shoulder that simultaneously drives the scapula both anteriorly and inferiorly. Forced anterior scapular protraction caused a displaced horizontal plane fracture of the inferior half of the distal clavicle, which was separated from the remainder of the clavicle. The inferiorly acromioclavicular ligament was still attached to this distal clavicle fragment. T

Posterior fracture-dislocation of the hip in a jogger.

Posterior fracture-dislocation of the hip is an uncommon athletic injury, occasionally seen in contact and high-energy sports. The mechanism of injury in this case highlights the high hip joint forces possible during running. The key treatment principle is early reduction of the hip joint, since the incidence of osteonecrosis of the femoral head and degenerative arthrosis increases with delay. Operative treatment will frequently be required to achieve a stable, congruent reduction of articular surfaces, essential for good long-term results. Partial weight-bearing exercise such as swimming and

Central fracture-dislocation of the hip in a child.

A case report of a 10-year-old child with a central fracture-dislocation of the hip, managed by open reduction is presented, with a long-term result. The rarity of this injury in children is discussed, with the possible complications of inadequate joint congruity and potential growth injury.

Palmar lunate dislocation with a dorsal scaphoid fracture variant.

A 20-year-old man had a dorsally displaced fracture of the proximal pole of the scaphoid and an ipsilateral palmar lunate dislocation--a rare combination of injuries that has not been previously reported. The most likely pathomechanics of this injury is a sheer fracture of the proximal pole of the scaphoid by the dorsal lip of the distal radius as the injury shifts from a dorsal perilunate to a palmar lunate pattern.
