The long-term effect on the tibiofemoral compartment of the knee joint of comminuted fractures of the patella.

We have studied the long-term effect of comminuted fractures of the patella with particular reference to the development of degenerative changes in the tibiofemoral joint. Degenerative changes which could convincingly be ascribed to the original injury were found in only one case, but we stress that the numbers are small, and that carefull search sould be made at operation for damage to the femoral condyles.

Lateral epicondyle osteotomy for panoramic exposure of comminuted radial head fractures- a surgical technique guide.

AIM: Comminuted radial head fractures pose technical challenges regarding visualisation and access for anatomical reduction and fixation. The traditional lateral 'window' approaches (Kocher, Kaplan and EDC split) can, at times, offer limited views and a more extensive exposure can be beneficial in comminuted patterns (Mason II/III). We present a lateral epicondyle osteotomy as an alternative for unobstructed views of the radial head to utilise for open reduction and internal fixation (ORIF). METHODS: The patient is positioned supine with the operative arm on an arm board. A standard lateral s

"Sandwich" Structure in Reconstruction of Traumatic Comminuted Anterior Skull Base Fractures: Surgical Technique and Outcomes.

BACKGROUND: Comminuted fractures of the anterior skull base can easily cause dural damage, leading to cerebrospinal fluid leakage and retrograde infection. Skull base reconstruction is essential. This study aimed to present a novel and simple technique for repairing skull base defects using a self-made fascia-bone fragments-fascia "sandwich" structure made by fascia, fracture fragments, and sutures. METHOD: From 2018 to 2023, we performed self-made sandwich structures for skull reconstruction in 10 patients with anterior skull base comminuted fractures. After debridement, the intracranial bon

The "Bag of Bones" Treatment of Comminuted Intra-articular Distal Humerus Fractures in the Elderly.

BACKGROUND: Extensively comminuted intra-articular distal humerus fractures in the elderly present a challenging therapeutic dilemma. The purpose of this study was to investigate the results of nonoperative treatment of these fractures in a select subset of patients. METHODS: Patients treated with nonoperative management for a comminuted intra-articular distal humerus fracture between 2007 and 2018 were reviewed. Patients were administered 3 elbow-specific functional outcomes instruments. RESULTS: A total of 8 patients (2 men, 6 women) were treated with brief immobilization followed by early

Reconstruction of unfixable comminuted posterior wall acetabular fractures with autologous bone graft: A systematic review.

BACKGROUND: Posterior wall acetabular fractures, often caused by high-energy trauma, are complex injuries that pose significant surgical challenges, especially when comminuted. Traditional fixation techniques have shown variable outcomes, with severe comminution sometimes rendering fragment fixation impossible. The aim of this study was to evaluate the clinical and radiological outcomes of autologous bone grafting for reconstructing severely comminuted unfixable posterior wall acetabular fractures. MATERIALS AND METHODS: A systematic review was conducted in accordance with the PRISMA guidelin

Open Reduction Internal Fixation of Simple Versus Comminuted Radial Head Fractures: Comparison of Clinical Outcomes.

PURPOSE: Current teaching suggests that modified Mason type III and IV fractures of the radial head involving more than 3 fragments should be treated with radial head arthroplasty. The purpose of this study was to compare the outcome of simple (2 or fewer intra-articular pieces) versus comminuted (3 or more intra-articular pieces) radial head fractures treated with open reduction internal fixation (ORIF). METHODS: This was a retrospective review of 35 patients with modified Mason type III and IV fractures treated with ORIF. For the purpose of our study, simple fractures were defined as having

Intraoperative Parameters of Comminuted Proximal Humerus Fractures: A Comparison Between Deltoid-Split and Deltopectoral Approaches.

Background and objective Many controversies exist in the literature regarding proximal humerus fractures treated with various surgical procedures. The chosen approach decides the level of comfort with which the surgeon can perform a particular surgery in orthopedics and the amount of reduction a surgeon can bring to a fractured bone. The purpose of this study was to compare variables such as time taken for surgery, achievement of reduction, and intraoperative blood loss between the two most frequently employed surgical approaches for proximal humerus in comminuted fractures: the deltoid-split

Construct Choice for the Treatment of Displaced, Comminuted Olecranon Fractures: are Locked Plates Cost Effective?

BACKGROUND: Cost effective implant selection in orthopedic trauma is essential in the current era of managed healthcare delivery. Both locking and non-locking plates have been utilized in the treatment of displaced fractures of the olecranon. However, locking plates are often more costly and may not provide superior clinical outcomes. The primary aim of the present study is to assess the clinical and functional outcomes of olecranon fractures treated with locked and non-locking plate and screw constructs while providing insight into the cost of various implants. METHODS: We performed a retros

Flexor tendon complications in comminuted distal radius fractures treated with anatomic volar rim locking plates.

OBJECTIVE: Anatomic volar rim locking plates are designed with the aim of treating intraarticular distal radius fractures. When used to treat comminuted distal radius fractures, these plates can damage the flexor tendons. In this study, we sought to determine the radiological and functional results and rate of complications of these plates. METHODS: We retrospectively reviewed the records of 36 patients (28 males, 8 females; mean age: 46.4 years) with AO/OTA Type C2-C3 distal radius fractures treated with anatomic volar rim distal radius plates between January 2011 and December 2014. Surgical treatment of comminuted mandibular fractures using a low-profile locking mandibular reconstruction plate system.

OBJECTIVE: The treatment of comminuted mandibular fractures is challenging due to the severity of associated injuries and the need for a careful diagnosis with adequate treatment planning. Recently, open reduction and stable internal fixation (OR-IF) with a load-bearing reconstruction plate have been advocated for reliable clinical outcomes with minimal complications. This clinical prospective study evaluated OR-IF in the surgical management of comminuted mandibular fractures with a new low-profile, thin, mandibular locking reconstruction plate. MATERIALS AND METHODS: We prospectively assesse

Biomechanical evaluation of fixation of comminuted olecranon fractures: one-third tubular versus locking compression plating.

INTRODUCTION: New concepts in plate fixation have led to an evolution in plate design for olecranon fractures. The purpose of this study was to compare the stiffness and strength of locking compression plate (LCP) fixation to one-third tubular plate fixation in a cadaveric comminuted olecranon fracture model with a standardised osteotomy. MATERIALS AND METHODS: Five matched pairs of cadaveric elbows were randomly assigned for fixation by either a contoured LCP combined with an intramedullary screw and unicortical locking screws or a one-third tubular plate combined with bicortical screws. Comminuted radial head fractures treated with a modular metallic radial head arthroplasty. Study of outcomes.

BACKGROUND: Comminuted fractures of the radial head are challenging to treat with open reduction and internal fixation. Radial head arthroplasty is an alternative treatment with results that compare favorably with those reported after open reduction and internal fixation of similar fractures. The purpose of this study was to evaluate the two-year outcomes and the rate of recovery of a closely followed cohort of patients in whom an unreconstructible radial head fracture had been treated with a modular metallic prosthesis. METHODS: Twenty-six patients (seventeen female and nine male; mean age,

Treatment results for open comminuted distal humerus intra-articuler fractures with Ilizarov circular external fixator.

In open, intra-articular distal humerus fracture caused by gunshot injury, full functional recovery is difficult to obtain. Three basic treatment methods are available: minimal internal fixation, open reduction-internal fixation, and external fixation. In Gülhane Military Medical Academy Department of Orthopedics and Traumatology, 19 of 20 cases of gunshot injuries were treated with circular external fixator between the January 1995 and December 2000. Nine (45%) cases were type III-A, eight (40%) were type III-B, and three (15%] were type III-C. Eight (40%) cases were brought to the hospital 6

Indirect open reduction through intercartilaginous incision and intranasal Kirschner wire splinting of comminuted nasal fractures.

The majority of nasal fractures have been managed by using closed reduction and intranasal packing. In comminuted nasal fractures, open reduction and internal fixation may be indicated for accurate reduction and rigid fixation, but it is a very aggressive procedure. We developed a new technique for comminuted nasal fractures: indirect open reduction and intranasal Kirschner wire splinting. A periosteal elevator is used to elevate the mucoperiosteum posterior to the nasal bone through intercartilaginous incision and to reduce accurately the nasal bone, at the same time detecting the fracture li

Management of comminuted extra-articular distal femoral fracture: A finite element study.

In case of extra-articular fractures, internal fixation is usually performed using implants such as Intramedullary nails (IMN), medial plates (MP), and lateral plates (LP). Recently, an innovative "sandwich" technique has been introduced for treating acetabular defects. The efficacy of such a technique to treat comminuted extra-articular distal femoral fractures has never been investigated. Therefore, the current study aims to compare the biomechanical performances of three implants, that is, medial plate, lateral plate, and IM nails, with and without the "sandwich" bone grafts. A CAD model of
