Dependency in older people recently admitted to care homes.

OBJECTIVE: to investigate dependency and general health status of a cohort of older people admitted to residential or nursing homes for long-term care. METHOD: we assessed 308 people aged over 65 years within 2 weeks of admission for long-term care to one of 30 nursing or residential homes in north-west England. Dependency was assessed using the Barthel activities of daily living index and the Crichton Royal Behaviour Rating Scale. We collected information from the homes' records on diagnosed conditions and current medication. RESULTS: 50% of the cohort were in a 'low dependency' band (Barth

Characterisation of the treatment provided for children with unilateral hearing loss.

BACKGROUND: Children with permanent unilateral hearing loss (UHL) are an understudied population, with limited data to inform the guidelines on clinical management. There is a funding gap in healthcare provision for the children with UHL in the United Kingdom, where genetic screening, support services, and devices are not consistently provided or fully funded in all areas. They are a disparate population with regard to aetiology and their degree of hearing loss, and hence their device choice and use. Despite having one "good ear", some children with UHL can have similar outcomes, socially, beh

May Measurement Month 2017: an analysis of blood pressure screening results worldwide.

BACKGROUND: Increased blood pressure is the biggest contributor to the global burden of disease and mortality. Data suggest that less than half of the population with hypertension is aware of it. May Measurement Month was initiated to raise awareness of the importance of blood pressure and as a pragmatic interim solution to the shortfall in screening programmes. METHODS: This cross-sectional survey included volunteer adults (≥18 years) who ideally had not had their blood pressures measured in the past year. Each participant had their blood pressure measured three times and received a a questi

Coverage, pricing, and reimbursement of telemedicine: a qualitative study among implementers of the Akoma Pa mHealth-based telemedicine intervention in Ghana.

BACKGROUND: Healthcare systems are rapidly using digital health technologies, such as mHealth-based telemedicine, as digitalization in healthcare provision advances. However, there is limited knowledge of the pricing and reimbursement mechanisms for these health interventions, especially in the Global South. This study aimed to investigate healthcare professionals' perspectives on the coverage, pricing, and reimbursement of telemedicine services in Ghana. METHODS: This study used a qualitative exploratory design. Multistage sampling was employed. All 46 sampled Christian Health Association of

A systematic review and meta-analysis of Tobacco 21 policies and youth tobacco use: implications for future policy research.

BACKGROUND: Raising the sales age to 21 is among the most important steps to curb tobacco/nicotine use in youth in the past two decades. The study aims to systematically review and perform meta-analyses on the impact of Tobacco 21 (T21) policies on tobacco use among 11-to-20-year-olds in the United States, encompassing T21s at local, state, and national levels. METHODS: We screened 14 databases following PRISMA and PICOS criteria, applying standardized abstraction forms and quality assessment tools. Inverse-variance-weighted random-effects models estimated pooled, fully adjusted odds ratios (

From the Ground Up: Stakeholder Perspectives on Housing Interventions to Reduce Environmentally Mediated Infections.

BACKGROUND: Housing conditions are intrinsically linked to human health, with inadequate housing potentially increasing exposure to environmentally mediated pathogens. Housing interventions that aim to improve housing and reduce environmentally mediated infections, such as finished floors and housing upgrades for vector-borne diseases, remain relatively under-explored as health interventions. This study explored facilitators of and barriers to funding, implementing, and scaling up housing improvements as health interventions to reduce environmentally mediated infectious diseases. METHODS: Six

Emerging SARS-CoV-2 variants expand species tropism to murines.

BACKGROUND: Wildtype mice are not susceptible to SARS-CoV-2 infection. Emerging SARS-CoV-2 variants, including B.1.1.7, B.1.351, P.1, and P.3, contain mutations in spike that has been suggested to associate with an increased recognition of mouse ACE2, raising the postulation that these SARS-CoV-2 variants may have evolved to expand species tropism to wildtype mouse and potentially other murines. Our study evaluated this possibility with substantial public health importance. METHODS: We investigated the capacity of wildtype (WT) SARS-CoV-2 and SARS-CoV-2 variants in infecting mice (Mus musculu

The Promise of Integration of HIV Into Primary Care: Challenges and Opportunities.

INTRODUCTION: Remarkable progress has been made in response to the global HIV epidemic, yet critical gaps and inequities remain, combined with challenges stemming from the current threats to global funding, complacency and competing global health priorities. These constraints threaten to unravel the hard-won gains and to stall progress towards control of the HIV epidemic. In response to this rapidly changing landscape, the integration of HIV services into primary care has emerged as a potential solution to this crisis that would bring possible efficiencies and sustainability of the response.

A multi-stakeholder situation assessment of COVID-19 disease preparedness and mitigation measures in a large prison complex in Malawi.

PURPOSE: Prisons in the sub-Saharan African region face unprecedented challenges during the COVID-19 pandemic. In Malawi, the first prison system case of COVID-19 was notified in July 2020. While prison settings were included in the second domestic COVID-19 response plan within the Law Enforcement cluster (National COVID-19 preparedness and response plan, July-December 2020), they were initially not included in the K157bn (US$210m) COVID-19 fund. The purpose of the study was to assess prison preparedness, prevention and control of COVID-19 in Malawi.. DESIGN/METHODOLOGY/APPROACH: A multi-meth

Knowledge About HIV/AIDS and Its Transmission and Misconception Among Women in Bangladesh.

BACKGROUND: Asian and pacific region countries are high risk countries for human immunodeficiency virus/ acquired immune deficiency syndrome (HIV/AIDS). Although the prevalence of HIV/AIDS is low in Bangladesh but women in Bangladesh have been identified as susceptible due to associated socioeconomic exposures. There are various misconceptions associated with HIV/AIDS transmission among the women in low- and middle-income countries including Bangladesh, which lead to a negative attitude towards the HIV/AIDS-infected. The purpose of this study was to assess the overall knowledge, transmission,

Efficacy and safety of albumin combined with endovascular therapy in the treatment of patients with acute ischemic stroke: a randomized clinical trial.

The safety and efficacy of albumin combined with endovascular therapy for participants with acute ischemic stroke is unknown. This randomized, double blinded, multicenter study was conducted in China and completed in 2025. Eligible participants were randomly assigned in a 1:1 ratio to albumin group (0.5 g/kg; maximum dose 150 mL; intravenously infusion once daily for 4 days) or placebo group (equivalent volume of placebo). The primary endpoint was the change in infarct volume from baseline to day 5. A total of 134 patients were randomized (66 in albumin group and 68 in placebo group) and 134 p

Privatisation by stealth, design or default? The future of New Zealand's health system.

New Zealand's health system is under sustained pressure, with worsening access to primary and specialist care, widening inequities and declining public confidence. In this context, there is an emerging but largely unarticulated shift towards increased reliance on private provision and private financing. This editorial examines whether this represents privatisation by design, default or drift, and considers the likely consequences. Evidence from New Zealand and comparable health systems suggests that increased privatisation is associated with higher costs, reduced equity and no consistent impro

An audit of referrals for acute abdominal CT scans.

INTRODUCTION: The number of CTs performed worldwide has increased significantly over the past 20 years, with abdominal CTs being the most common. This trend has raised concerns about patient safety due to lifetime cumulative exposure to cancer-associated ionising radiation. The aim of this study was to evaluate the clinical justification for performing acute abdominal CTs in Danish patients admitted to a surgical department. METHODS: This quality assurance study was conducted as a clinical audit at the North Denmark Regional Hospital. A total of 200 patients who had undergone acute abdominal

Understanding the needs and challenges of unpaid carers caring for someone with drug and alcohol dependency: findings from a national qualitative evaluation.

PURPOSE: The cost-of-living crisis witnessed in the UK, in addition to the repercussions of the COVID-19 pandemic, has exacerbated growing concerns about its disproportionate impact on caregivers. All caregivers face unprecedented challenges, but this is further intensified when caring for someone with drug and alcohol dependency which this paper argues are direct social harms. METHODS: This paper presents findings from an external evaluation of three projects from a wider national programme funded by NHS England and Improvement [NHSE/I]. Identified by NHSE/I as a vulnerable community, interv

Implementing outcomes-based risk-sharing agreements: an integrative review of applications in blood cancer in the UK and beyond.

INTRODUCTION: Outcomes-based risk-sharing agreements (OBRSA) have been increasingly used worldwide to manage uncertainty in value assessments. This review aimed to summarize motivations, barriers, and facilitators to implementing OBRSAs with a specific focus on therapies for hematological cancer. AREAS COVERED: An integrative review was conducted based on a scoping of existing reviews on the topic and reports published by UK NICE. Findings from 16 articles and 10 reports were summarized and categorized into three themes: applications in blood cancer drugs, motivations for adoption, and barrie

Developments in understanding early onset Alzheimer's disease.

On September 25 and 26, 2021, the Alzheimer's Association hosted the first meeting focused on people with early-onset Alzheimer's disease (EOAD)-sometimes referred to as younger onset Alzheimer's disease (AD). Though a diagnosis of AD can be devastating at any age, those with a younger onset-defined as symptoms developing prior to 65 years of age-face unique challenges. EOAD occurs when people are in the prime of their lives, often with multiple responsibilities including careers, community activities, and raising children and caring for older family members. These challenges warrant special c

Healthcare experiences of patients with Down syndrome who are Black, African American, of African descent, or of mixed race.

Scant research has explored the healthcare experiences of people with Down syndrome (DS) in the United States who are Black, African American, of African descent, or of mixed race. The purpose of this study was to identify and describe the barriers and facilitators that such patients and their caregivers face when accessing healthcare. We gathered data in three ways: focus groups with caregivers, a national survey completed by caregivers, and in-depth interviews with primary care providers. Many caregivers and primary care physicians felt that patients with DS who are Black, African American,

Data Sharing Goals for Nonprofit Funders of Clinical Trials.

Sharing clinical trial data can provide value to research participants and communities by accelerating the development of new knowledge and therapies as investigators merge data sets to conduct new analyses, reproduce published findings to raise standards for original research, and learn from the work of others to generate new research questions. Nonprofit funders, including disease advocacy and patient-focused organizations, play a pivotal role in the promotion and implementation of data sharing policies. Funders are uniquely positioned to promote and support a culture of data sharing by serv

A clinical review of human disease due to Paragonimus kellicotti in North America.

Human autochthonous infections with the trematode Paragonimus kellicotti are increasingly being reported in North America, but the true prevalence and geographic distribution are unknown. Ingestion of raw crayfish is reported in most human cases, typically in the context of alcohol intoxication. Risk of infection varies depending on what part of the crayfish is ingested as metacercariae of P. kellicotti (>50% mature to adults) localize to the heart and pericardium of the crayfish. Reported human cases have manifested primarily in otherwise healthy young adults presenting with severe eosinophil
