Galactorrhea complicating wound healing following reduction mammaplasty occurs rarely; only isolated cases have been reported in recent years. We report the case of a 25-year-old woman who presented with delayed healing and dehiscence of surgical wounds 3 weeks following vertical scar reduction mammaplasty. In two of eight premenopausal women with somatotropic adenomas, galactorrhea was the earliest clinical feature, associated in one patient with amenorrhea. Eleven eumenorrhoic patients presenting galactorrhea were subjected to a three-month Bromoergocryptin therapy (2.5 mg/os/die). Prolactin Circadian Rhythm, performed on all patients, showed four hyper and seven normoprolactinemic cases. A young woman, 14 months post partum, committed suicide when she noticed milk discharging from her nipples and thought she was pregnant. At autopsy the uterus was found to be non-gravid and the pituitary gland enlarged. Histological examination revealed lymphocytic adenohypophysitis, a rarely identified condition usually associated with hypopituitarism. Galactorrhea is a known complication of breast surgery, particularly reduction mammoplasty. However, in augmentation mammoplasty, it is a rare event. There are only a few case reports concerning galactorrhea after augmentation mammoplasty. A case of galactorrhea immediately following a reduction mammaplasty is presented. Prolactin levels were elevated postoperatively and could be reduced to normal by a prolactin inhibitor. No other disease or disorder could be found. Based on a review of the literature, it seems that various factors have to contribute to galactorrhea, including stress-related prolactin secretion, rebound phenomenon after discontinuation of birth control pills, and hypersensitivity to prolactin receptors. Their coincidental occurrence, resulting in abundant milk production after surgery, is discussed.

A 17-year-old girl suffered a traumatic acute subdural hematoma and rib fractures after a motor vehicle accident. Prolonged coma resulted, and significant galactorrhea began 4 weeks after the accident. The galactorrhea was treated successfully by the administration of 2-bromoergocryptine. The pathophysiology of this condition is discussed.

Further substantiation of the interaction between psychological disease and the onset of galactorrhea-amenorrhea is presented in women who underwent intensive psychometric and neuroendocrine evaluation prior to the use of the dopamine against 2-Br-alpha-ergocryptine (CB-154). Normalization of a previously distorted psychometric test profile in each woman was accompanied by a profound decline in prolactin and an unexpected alteration in the peripheral concentrations of norepinephrine and dopamine-beta-hydroxylase. Cesation of galactorrhea and resumption of menses were observed in each instance.
