Herpes zoster oticus (Ramsay Hunt syndrome) in children: case report and literature review.

INTRODUCTION: Approximately 10% of patients with peripheral facial nerve palsy are children. Facial nerve palsy is usually idiopathic. An un … tibulocochlear dysfunction. It occurs by reactivation of latent varicella-zoster virus (VZV) in the geniculate ganglion, affecting the seventh and eighth cranial nerves. The diagnosis is based on history and physical findings. Treatment of Ramsay Hunt syndrome uses a combination of high-dose corticosteroids and acyclovir. Although the prognosis is better in children …

[An adult case of peripheral facial nerve palsy following acute cerebellitis associated with high antibody titers against varicella-zoster virus].

Here we report a case of acute cerebellitis, in which the patient developed right peripheral facial palsy during the recovery phase of cerebellar ataxia. A 67-year-old man developed truncal and limb ataxia following a fever, general fatigue and anorexia. He was diagnosed to have acute cerebellitis. While the ataxia symptoms were improving without any treatment, right peripheral facial nerve palsy developed and an MRI revealed an enhancement of the right facial nerve proximal to the geniculate ganglion. After treatment with acyclovir and corticosteroids, his facial nerve palsy and ataxia both g…

Perineural Invasion of the Intratemporal Facial Nerve: How Far Proximally Do We Chase the Positive Margin?

OBJECTIVE: To determine recurrence patterns in patients with head and neck cancers requiring facial nerve sacrifice and to determine the opt … lly along the facial nerve is an exceedingly rare event and the necessity of biopsy proximal to the geniculate ganglion, and in some cases even to the second genu, is called into question.

Transcanal endoscopic approach to lesions of the suprageniculate ganglion fossa.

OBJECTIVE: The aim of this paper would be to describe the first case series of exclusive transcanal endoscopic approach to treat lesions wit … ed in the suprageniculate fossa in some patients. We introduce a minimally invasive approach to the geniculate ganglion region in order to allow complete removal of suprageniculate disesases with low complication rates using a minimally invasive surgical route.

Retinal Input Is Required for the Maintenance of Neuronal Laminae in the Ventrolateral Geniculate Nucleus.

Retinal ganglion cell (RGC) axons provide direct input into several brain regions, including the dorsal lateral geniculate nucleus (dLGN), which is important for image-forming vision, and the ventrolateral geniculate nucleus (vLGN), which is associated with nonimage-forming vision. Through both activity- and morphogen-dependent mechanisms, retinal inputs play important roles in the development of dLGN, including the refinement of retinal projections, morphological development of thalamocortical relay cells (TRCs), timing of corticogeniculate innervation, and recruitment and distribution of inh

Salivary Duct Carcinoma Spread to the Internal Auditory Canal Along the Facial Nerve: A Case Report.

Salivary duct carcinoma (SDC) is a rare high-grade parotid malignancy prone to perineural spread. However, perineural spread of SDC has rarely been reported. The case of a 46-year-old male with SDC spread along the facial nerve (FN) is presented here. Despite their involvement, the FN geniculate ganglion and mastoid segment were ignored in the initial parotidectomy procedure. SDC progressed during the postoperative follow-up period, as clinicians were unaware of perineural spread. Finally, facial magnetic resonance imaging (MRI) and curved planar reformat (CPR)-computed tomography (CT) reveale…

Differential Impact of Retinal Lesions on Visual Responses of LGN X and Y Cells.

Damage to retinal cells from disease or injury causes vision loss and remodeling of downstream visual information processing circuits. As retinal cell replacement therapies and prosthetics become increasingly viable, we must understand the postretinal consequences of retinal cell loss to optimally recover visual perception. Here, we asked whether loss of retinal ganglion cells (RGCs) differentially impacts postsynaptic neurons in the visual thalamus-the dorsal lateral geniculate nucleus (LGN)-of ferrets, highly visual carnivores. We hypothesized that RGC loss might impact X more than Y LGN neu

Parallel pathways carrying direction-and orientation-selective retinal signals to layer 4 of the mouse visual cortex.

Parallel visual pathways from the retina to the primary visual cortex (V1) via the lateral geniculate nucleus are common to many mammalian species, including mice, carnivores, and primates. However, it remains unclear which visual features present in both retina and V1 may be inherited from parallel pathways versus extracted by V1 circuits in the mouse. Here, using calcium imaging and rabies circuit tracing, we explore the relationships between tuning of layer 4 (L4) V1 neurons and their retinal ganglion cell (RGC) inputs. We find that subpopulations of L4 V1 neurons differ in their tuning for

Effects of convergent strabismus on the development of physiologically identified retinogeniculate axons in cats.

We have studied the effects of surgically induced convergent strabismus (esotropia) on the morphological development of retinogeniculate X and Y axon arbors in cats. Single axons were recorded in the lateral geniculate nucleus or in the optic tract adjacent to the nucleus, classified physiologically, and injected intracellularly with horseradish peroxidase. The arbors of recovered axons were compared with X and Y axon arbors from normally reared adult cats. Our data demonstrate that while X axon arbors are relatively normal, the arbors of Y axons are profoundly affected by rearing with strabis

Retinotopic order appears before ocular separation in developing visual pathways.

In mammals, the major subcortical visual structures receive projections from both eyes, with the uncrossed projection being smaller than the crossed. Each projection is arranged as a separate orderly map of one hemiretina. Although these hemiretinal maps are separate in the nuclei, they are aligned so that the representations of points in the visual field are in register, thus there is a continuity of visual field representation between them. During the early development of the binocular pathways, terminals from the two eyes overlap almost entirely. As development proceeds, terminals arising f

Transneuronal Degeneration in the Brain During Glaucoma.

The death of retinal ganglion cells (RGCs) is a key factor in the pathophysiology of all types of glaucoma, but the mechanism of pathogenesis of glaucoma remains unclear. RGCs are a group of central nervous system (CNS) neurons whose soma are in the inner retina. The axons of RGCs form the optic nerve and converge at the optic chiasma; from there, they project to the visual cortex via the lateral geniculate nucleus (LGN). In recent years, there has been increasing interest in the dysfunction and death of CNS and retinal neurons caused by transneuronal degeneration of RGCs, and the view that gl

[Ramsay-Hunt syndrome].

Ramsay-Hunt syndrome is a peripheral facial nerve palsy accompanied by an erythematous vesicular rash on the ear or in the mouth; it is caused by varicella zoster virus that affects the geniculate ganglion. The zoster oticus is the second most common cause of atraumatic peripheral facial paralysis. We present a review of zoster oticus identified in our hospital among 2001-2005. We show various atypical cases with multiple cranial nerve involvement; cerebellum and spinal cord was affected in one patient. 3/10 cases were complicated with pneumonia. So, we think that some grade of immunodeficienc…

Facial nerve paralysis induced by herpes simplex virus in mice: an animal model of acute and transient facial paralysis.

We have been the first to succeed in producing an acute and transient facial paralysis simulating Bell's palsy, by inoculating herpes simple … cted facial nerve and nuclei. Herpes simplex virus antigens were also detected in the facial nerve, geniculate ganglion, and facial nerve nucleus. The pathophysiologic mechanisms of the facial paralysis are discussed in light of the histopathologic findings, in association with the causation of Bell's palsy.

Intraoperative evoked electromyography in Bell's palsy.

The technique of intraoperative evoked electromyography is described in detail. Direct intraoperative stimulation of the exposed facial nerve identified the sites of nerve impulse conduction blocks in 16 of 18 patients who needed decompression for Bell' s palsy. In order to determine the site of impulse conduction block the test must be performed before 100 per cent nerve degeneration occurs. The lesions extended only a few millimeters and were found to be proximal to the geniculate ganglion in 15 (94 per cent) of these patients. Schirmer's test correctly identified the sites of the lesions in…

Cell type-specific expression of FoxP2 in the ferret and mouse retina.

Although the anatomical and physiological properties of subtypes of retinal ganglion cells (RGCs) have been extensively investigated, their molecular properties are still unclear. Here, we examined the expression patterns of FoxP2 in the retina of ferrets and mice. We found that FoxP2 was expressed in small subsets of neurons in the adult ferret retina. FoxP2-positive neurons in the ganglion cell layer were divided into two groups. Large FoxP2-positive neurons expressed Brn3a and were retrogradely labeled with cholera toxin subunit B injected into the optic nerve, indicating that they are RGCs
