Recalcitrant scoliosis in Proteus syndrome.

Proteus syndrome is a rare congenital condition with various clinical features such as hemihypertrophy, macrodactyly, subcutaneous masses, brain-like hyperplasia of the soles and/or palms, epidermal naevi and scoliosis with other mesodermal malformations. Multifocal overgrowth can affect various tissues causing severe functional and cosmetic disability, but intellectual and language development are mostly normal. Orthopaedic problems include macrodactyly, hindfoot deformity, limb length inequality, genu valgum and scoliosis. Usually, scoliosis does not respond to bracing, and surgical interven…

Musculoskeletal manifestations of Hurler syndrome: long-term follow-up after bone marrow transplantation.

Bone marrow transplantation (BMT) is effective in ameliorating many of the clinical manifestations of Hurler syndrome. However, long-term data on the natural history of the musculoskeletal disorders of Hurler syndrome after BMT are limited. The authors report the orthopaedic outcomes in seven patients with Hurler syndrome who were successfully engrafted between 1990 and 1999, and have been followed for a mean of 7.6 years since transplantation. Medical records, clinical examinations, and imaging studies were reviewed to assess the development and management of hip dysplasia, genu valgum, spine…

[A surgical treatment of early partial closure of the epiphysial plate].

OBJECTIVE: To introduce the technique of epiphysiolysis and epiphysis grafting in the treatment of early partial closure of the epiphysial plate in children, and evaluate its effect. METHODS: Retrospective study was performed in 10 cases of cubitus varus and valgus, or genu varum and valgum due to early partial closure of the epiphysial plate, 6 cases received simple cuneiform osteotomy, and 4 cases received epiphysioloysis and epiphysis grafting. RESULTS: Clinical outcoming after 6-month to 5-year's follow-up showed 1 case deformity of recurrence and 2 cases of non-isometric limb after simp

Lateral patellar instability: treatment with a combined open-arthroscopic approach.

Lateral patellar maltracking, subluxation, and dislocation could arguably be grouped together as varying degrees of lateral patellar instability. Besides common anatomical etiological factors, such as genu valgum, patella alta, tibial rotation, and others, the author has found that many of these patients exhibit patellar hypermobility, a seldom mentioned physical finding. Lateral retinacular release fails to address the hypermobility, whereas traditional, more extensive open operations often carry significant postoperative morbidity and disability. This article suggests a surgical approach tha…

Cranio-metaphyseal dysplasia.

Cranio-metaphyseal dysplasia in two brothers, aged fourteen and twelve, is reported. Both brothers presented with deafness, repeated episodes of cold and cough and mouth breathing. Striking craniofacial configuration consisted of hypertelorism, prominent glabella and zygomatic arches, mandibular prognathism and overgrowth of middle third of face. Both patients had genu valgum deformity. Low intelligence and poor scholastic performance present in both brothers were attributed to deafness. Radiographic features consisted of obtuse mandibular angle, defective dentition, sclerotic frontal sinuses,…

Dysgerminoma--gonadoblastoma and familial 46XY pure gonadal dysgenesis: case report and review of the genetics and pathophysiology of gonadal dysgenesis and H-Y antigen.

46XY gonadal dysgenesis is a rare condition of intersexuality first described in detail by Swyer in 1955. It is characterized by a phenotypic female who is tall, eunuchoid with normal female external genitalia, hypoplastic uterus and Fallopian tubes, streak ovaries and primary amenorrhoea. The patients exhibit none of the associated congenital defects such as webbing of the neck and genu valgum seen in Turner's syndrome. Although rare, this clinical entity is important to recognize because of the high incidence of gonadoblastomas and subsequent development of dysgerminoma. The condition may be…

Cyclic variations in multiplanar knee laxity influence landing biomechanics.

PURPOSE: Females vary substantially in their multiplanar cyclic knee laxity changes across the menstrual cycle. The biomechanical implications of these cyclic changes are relatively unknown. Our purpose was to first cluster females based on their cyclic changes in anterior knee laxity (AKL), genu recurvatum (GR), varus-valgus (VV), and internal-external (IER) rotation knee laxity across the menstrual cycle. We then compared changes in landing biomechanics from days of minimum to maximum laxity between female clusters and a group of males. METHODS: A total of 49 males and 71 females were measu

Distal Femoral Varus Osteotomy for the Management of Valgus Deformity of the Knee.

Distal femoral varus osteotomy is a well-described treatment option for patients with valgus malalignment associated with a variety of underlying conditions. This procedure may be the definitive treatment option in active patients with isolated lateral compartment osteoarthritis or posttraumatic arthritis. It may be a useful concomitant procedure in young patients with lateral meniscus deficiency, focal chondral defects, chronic medial collateral ligament insufficiency, and/or patellofemoral instability. Distal femoral varus osteotomy can be performed with medial closing wedge or lateral openi

[Experience with temporary epiphyseodesis after Blount (author's transl)].

The article reports on the results of follow-up esaminations of 57 children in whom temporary epiphysiodesis according to Blount had been performed on account of differences in leg length and because of angular deformities. Numerous complications were revealed during the evaluation of the results, which had to be subdivided into pure clasp complications such as fracture of the clasps, and secondary consequences. Formation of a genu recurvatum was particularly frequent, as well as the development of genu valgum and varum, ligamental insufficiencies, and ugly scars. Although in some cases the pr…

Keblish's lateral subvastus approach for total knee arthroplasty: A technical note.

Keblish's lateral surgical approach for total knee arthroplasty enables a direct release of the lateral structures in valgus deformities. In recent years, the development of quadriceps-sparing techniques has improved early functional recovery after arthroplasty, which has been well described using a medial approach. The authors present Keblish's lateral subvastus approach for genu valgum deformities, with key surgical considerations and preliminary outcomes. The novelty of this technique lies in its combination of a lateral subvastus approach with Keblish's Z-plasty, aiming to optimize quadric…

Surgical treatment of juvenile rheumatoid arthritis.

Juvenile rheumatoid arthritis is rare and only affects a small number of children but can lead to severe crippling of adolescents or adults unless they are treated early. In this review the problems encountered in the surgical treatment, particularly those caused by skeletal stunting are discussed. The results of synovectomy of the knee and elbow are valuable procedures in suitably selected patients. Stapling of the knee in young children with genu valgum gave good correction in the majority. Soft tissue operation on the hip and knee in young children with severe deformities have given encoura…

Mucopolysaccharidosis type I disguised as rickets.

A 16-year-old boy with widening of the large joints of the extremities and bilateral genu valgum had been extensively treated with oral vitamin D, with little clinical benefit. A diagnosis of vitamin D-resistant rickets was considered initially but a thorough clinical examination and skeletal survey was suggestive of mucopolysaccharidosis. The diagnosis was confirmed biochemically and subtype classification pointed toward the type I variety of the storage disorder. Absence of mental retardation is very unusual in mucopolysaccharidosis type I, which itself is an uncommon clinical entity. This p…

Common paediatric conditions of the lower limb.

Growing children are susceptible to a number of disorders to their lower extremities of varying degrees of severity. The diagnosis and management of these conditions can be challenging. With musculoskeletal symptoms being one of the leading reasons for visits to general practitioners, a working knowledge of the basics of these disorders can help in the appropriate diagnosis, treatment, counselling, and specialist referral. This review covers common disorders affecting the hip, the knee and the foot. The aim is to assist general practitioners in recognising developmental norms and differentiati

Combined Distal Femoral Osteotomy and Medial Patellofemoral Ligament Reconstruction for Patellar Instability and Genu Valgus: A Case Report and Literature Review.

BACKGROUND: Medial patellofemoral ligament (MPFL) reconstruction alone is not effective for patellar instability associated with anatomic abnormalities of lower limbs. In this article, we report a case of complex lower limb malformations, including genu valgus, lower limb shortening, and increased femoral anteversion angle. In addition to MPFL reconstruction, we performed a rare osteotomy named combined distal femoral osteotomy (CDFO), which combined the characteristics of lateral opening wedge distal femoral osteotomy (LOWDFO) and derotational distal femoral osteotomy (DDFO). Potential influence of factors for genu valgus with hereditary multiple exostoses.

Genu valgus is one of the most common limb deformities in hereditary multiple exostoses (HME). However, it is easily concealed and may account for subsequent osteoarthritis of the knee. The knees of 56 patients (33 men and 23 women) with HME were investigated bilaterally. Knee valgus was described by the mechanical axis deviation (MAD), mechanical lateral distal femoral angle (LDFA), and medial proximal tibial angle (MPTA). We investigated sex, age, BMI, total number of palpable osteochondromas, number of radiographic osteochondromas around the knee, forearm deformities, morphology and distrib
