Gout in primary care.

Gout affects around 1% of people in the UK, most aged over 45 years. The condition, which is five times more common in men than women, usually presents as a painful inflammatory monoarthritis, which can be diagnosed clinically and successfully treated with a high dose of an NSAID. Problems arise when the features are atypical or when high doses of an NSAID are inappropriate. Also; there is no robust evidence to guide when to start prophylactic therapy or what advice to give on lifestyle changes. Here we review current practice and offer advice on managing patients with this increasingly common…

[Sonographic monitoring of gout].

OBJECTIVE: To assess the role of grayscale and power Doppler sonography in short-term monitoring of acute gouty synovitis. METHODS: Clinical and sonographic assessments were performed in a patient with gout. Clinical and sonographic evaluations were performed within the first day of the acute onset of synovitis of the first metatarsophalangeal joint of the left foot and 7 and 14 days after the baseline assessments. The patient was treated only with colchicine. RESULTS: At baseline, both grayscale and power Doppler sonography revealed clearly evident findings of acute synovitis (joint cavity …

Self-treatment for gout.

After describing some of the symptoms of gout and considering some causes, such as an excess of ethanol, the source of the pain in the infected joint is discussed. This is known to be from urate crystals formed in the synovial fluid inside the joint. It is suggested herein that the pain is due to grinding from the crystals through the surface film of the joint, and possibly into the bone itself, which is relatively soft. The pain then stems in part from the resulting inflammation. The key hypothesis is that these urate crystals dissolve on warming. Hence, by warming the joint concerned in hot …

[Therapy of hyperuricemia and gout].

Therapy of hyperuricemia and gout has to depend on pathogenesis and stage of the disease. Dietary regimen are in the forefront in treatment of asymptomatic hyperuricemia. Uric acid lowering drugs can only be supported in repeated serum-measures from 9 mg/dl up. The therapy of an acute attack of gout primarily is done with non-steroidal antiinflammatory drugs, in rare cases with colchicine or corticoids. Gouty arthritis in intermission, independent of the extent of hyperuricemia, as well as chronic gout are indications for an uric acid lowering pharmacotherapy, usually for life. A special thera…

[Gout as a cause of uveitis].

The paper presents the case of a serious uveitis not yielding to a conventional therapy. After an insight differential diagnostics it was found that the gout made the disease etiology. After including a specific therapy an improvement in the local status was achieved.

[Radiologic findings in gout].

Radiological gout manifestations are detectable in the early stage, but also especially in the chronic stage of gout. Whereas in the early stage only soft tissue mutations (bursitis inflammation) and light calcium deposits are usually discernible, chronic gout leads to asymmetrical, diverse forms of osseous destruction, favouring smaller joints, but also affecting larger ones, which are caused by the intra-articular and extra-articular deposit of tophus material, corresponding to the progression and degree of severity of the illness. Radiologically-detectable changes in other organs, such as t…

[MR imaging in gout].

The appearance of gouty tophus in magnetic resonance imaging (MRI) is characteristic. On T1- and T2-weighted SE images, the signal intensity of tophaceous lesions is similar to that of muscles. According to the histology, T2-weighted SE images demonstrate extremely hyperintense signals, which reflect the high protein content in the amorphous center of the tophus. The microscopic urate crystals deposited there have no MRI signal and are of no further diagnostic impact. Vascularized granulation tissue surrounding the tophus center enhance after intervenous application of contrast agents (Gadolin…

Radiologic features of gout.

Gout is a common rheumatologic disease characterized by the deposition of monosodium urate crystals in tissue from supersaturated extracellular fluid. The deposition of crystals in the joints and periarticular soft tissue can lead to arthritis and bone destruction. The radiologic features of gout include swelling of soft tissues, tophi, normal mineralization, preservation of joint space until the later stages of disease, "punched-out" erosions with overhanging edge of cortex and sclerotic borders, and an asymmetric polyarticular distribution. The lower-extremity joints are most often affected,…

Gout: 'a disease of plenty'.

Gout is a syndrome caused by an inflammatory response to the formation of monosodium urate monohydrate crystals which develop secondary to hyperuricaemia. Acute and chronic forms occur. Hyperuricaemia may be due to environmental and/or genetic factors. It most commonly affects middle-aged males. This article discusses the management of both acute and chronic gout.

[Diet therapy in gout].

In gout, an effective diet is important to avoid or to lower complications and expenses of drug treatment. Most important dietary measures are reduction of purine uptake and alcohol consumption and normalization of body weight. Purine content of food can be altered quantitatively and qualitatively by storage and cooking with consequences for digestion and absorption of purines. The modern concept of diet in gout takes into consideration the purine content of food per weight, per portion and per energy.
