The iC-GPC Assay (iCubate, Huntsville, AL) is a qualitative multiplex test for the detection of five of the most common Gram-positive bacteria (Staphylococcus aureus, Staphylococcus epidermidis, Streptococcus pneumoniae, Enterococcus faecalis, and Enterococcus faecium) responsible for bacterial bloodstream infections, performed directly from positive blood cultures. The assay also detects the presence of the mecA, vanA, and vanB resistance determinants. This study comparatively evaluated the performance of the iC-GPC Assay against the Verigene Gram-positive blood culture (BC-GP) assay (Luminex

Aztreonam was used for the treatment of gram-negative bacteremia in 101 patients. In 34 instances a second antibiotic was prescribed for the treatment of suspected or documented gram-positive or anaerobic infection. The sources of bacteremia were the urinary tract (50 patients), an intraabdominal site (17), the respiratory tract (8), an intravascular site (9), and an unknown site (17). The clinical response rate was 92% (91 of 99 patients). The bacteriologic response rate was 97% (98 of 101 patients). In six of seven patients, Pseudomonas aeruginosa bacteremia was cured. Twelve patients develo

Previous studies have demonstrated that athletic mouthguards worn by ice hockey and football players harbor large numbers of bacteria, yeasts, and molds, some of which are either opportunistic or frank pathogens. This article details the clinical history of two junior high school football players. The first player had cellulitis of the leg after a non-break injury. The same unusual bacterium was isolated from both the athletic mouthguard and abscess cultures from the wound. The second patient suffered an attack of exercise-induced asthma so severe that his inhaler could not control the symptom

BACKGROUND: Skin and soft tissue infections remain among the most frequently encountered infections in surgery, and their severity ranges from mild cellulitis to severe, necrotizing infections with high incidences of morbidity and mortality. Most commonly, these disorders result from skin lesions in a susceptible host, but sometimes develop following hematogenous spread from a previously unknown focus.
METHODS: The current literature was reviewed to provide an up-to-date summary of the microbiology, diagnosis, pathogenesis, and therapy of complicated skin and soft tissue infections. Special c

Certain group VIIIB transition metal compounds were found to inhibit cell division in Escherichia coli, causing marked filamentous growth. Gram-negative bacilli were the most sensitive to this effect, whereas gram-positive bacilli responded only at near-toxic levels of the metal. None of the cocci tested showed any apparent effect. Cytokinesis (cross-septation) can be initiated by removal or decrease of platinum, but not by treatment with pantoyl lactone, divalent cations, or a temperature of 42 C.

INTRODUCTION: Staphylococcus epidermidis is currently the most frequent pathogen of opportunistic and nosocomial infections worldwide. Most cases of Staphylococcus epidermidis infections are associated with indwelling medical devices and/or immunocompromised conditions. Community-acquired urinary tract infections are rare, particularly among pediatric populations, and clinicians often do not consider Staphylococcus epidermidis as a uropathogen.

To identify the molecular characteristics of Gram-positive cocci isolated from blood cultures and clinical outcome among critically ill patients. This retrospective study was conducted in the general intensive care unit of the University General Hospital of Patras, Greece, during a 5-year period (2012-2016). All adult patients with a Gram-positive BSI were included. PCR was applied to identify mecA gene (staphylococci); vanA, vanB, and vanC genes (enterococci). Linezolid-resistant S. epidermidis, MRSA, and VRE were further typed by multilocus sequence typing. Mutations in region V of 23S rDNA

BACKGROUND Little is known of neutrophil-to-lymphocyte ratio (NLR) variations in septic shock. Hence, the predictive value of procalcitonin (PCT) and NLR variations for septic shock in bloodstream infection were explored. MATERIAL AND METHODS We analyzed 146 patients with bloodstream infection admitted to the Intensive Care Unit (ICU) of the First Affiliated Hospital of Anhui Medical University from October 2016 to May 2020. PCT and NLR were evaluated at 0 and 48 h after admission, and their variations (ΔPCT and DNLR) were calculated. The patients were divided into a shock group (n=80) and a n

We studied the activity of cefotaxime both microbiologically and clinically. 138 blood cultures positive for gram-positive cocci were evaluated (90 strains of Staphylococcus aureus, 25 of Streptococcus faecalis, 13 of Streptococcus alpha and ten of Streptococcus mutans). Cefotaxime showed good activity against all strains with the exception of S. mutans, of which only 30% were sensitive. Ten cases of gram-positive infections were studied clinically: six sepsis cases and one endocarditis case due to S. aureus, two sepsis cases caused by Streptococcus alpha and one Enterococcus endocarditis case

Antilysozyme activity (ALA) as well as antibiotic resistance were detected in 133 microbial cultures isolated from bioptic specimens of the mucous membrane of the ulcerous and periulcerous zones, taken from patients with gastric and duodenal ulcer. In 85.7-94.7% of cases Gram positive cocci and in 62.5% of cases Gram-positive bacilli showed no ALA. 50% of Gram negative bacteria cultures lacked ALA, while the remaining 50% exhibited this activity, on the average, 2.36 +/- 1.40 mkg/ml. Lysozyme activity was determined in 33.3% of the isolated staphylococci strains both with and without ALA. Stap

A new monocyclic beta-lactam antibiotic, SQ26,776, was evaluated in vitro against clinical isolates, and its activity was compared with that of other cephalosporins, penicillins, and aminoglycosides. This compound was very active against the majority of the aerobic gram-negative bacilli tested, including 47 multiresistant strains. It had no activity against the gram-positive cocci. It also showed activity comparable to that of moxalactam, ceftazidime, ceftriaxone, thienamycin, and tobramycin. It was superior to mezlocillin. The in vitro activity suggests that SQ26,776 should prove useful in se

Once traditionally considered a neonatal pathogen, Group B Streptococcus (GBS) is a pathogen of growing importance in adults and those with underlying comorbidities. This case series discusses five cases of community-acquired GBS meningitis in adults who presented with history of fever, altered mental status, and other neurological symptoms. Gram stain revealed pus cells and gram-positive cocci and culture was positive in four out of five cases and one isolate was CAMP-negative.In culture negative case PCR clinched the diagnosis. This case series highlights the importance of considering GBS as

The susceptibilities of clinical isolates to fluoroquinolones (FQs) and other antimicrobial agents were surveyed to obtain an accurate understanding of the trends in incidence and antimicrobial resistance. The samples were collected from across Japan, biennially, between 1994 and 2002 and a defined level of resistance to FQ determined. Streptococcus pneumoniae and Haemophilus influenzae exhibited stable and high rates of susceptibility to FQ over the period examined. For methicillin-resistant Staphylococcus aureus the rate of resistance to FQ was 80%-90%, markedly higher than that of methicill

BACKGROUND: Podoconiosis, a non-infectious disease originating from long-term exposure of bare feet to irritant red clay soil is a lifelong, disabling disease with no specific diagnostic tool, classified into 5 stages based on the severity of leg swelling (lymphoedema). Secondary bacterial infections have been suggested to cause acute dermatolymphangioadenitis (ADLA) attacks and drive disease progression. Although the North West Region of Cameroon has a proven history of podoconiosis endemicity, the bacterial composition of lymphoedema due to this condition has not been studied. Thus, this stu

INTRODUCTION: Worldwide, about one-tenth of end-stage renal disease (ESRD) patients are on peritoneal dialysis (PD). Peritonitis is a major cause of PD failure and change of therapy to haemodialysis. An update on peritoneal dialysis-related infections has recommended the use of a first generation cephalosporin or vancomycin as an empirical therapy for Gram-positive organisms. Pediococcus spp. is a Gram-positive environmental cocci that have been increasingly reported from various nosocomial infections but very rarely from peritoneal dialysis infections. It is intrinsically resistant to Vancomy

This study aimed to evaluate a new protocol of the bile solubility test performed directly on the blood from positive blood culture bottles to identify Streptococcus pneumoniae rapidly. Seventy-five positive blood cultures (PBC) showing Gram-positive cocci in pairs or chains on Gram stain, including 32 S. pneumoniae isolates and three reference American Type Culture Collection (ATCC) isolates were included to evaluate the performance of a modified bile solubility test (MBST). One milliliter of blood from the PBC bottle was mixed with 0.5 mL of 10% desoxycholate or a saline solution. Both suspe

Aspiration of exudate through an open perforation was performed in 183 patients with chronic otitis media. The pus was cultured aerobically and anaerobically. Aerobes only were isolated from 71 patients (39%); 20 patients (11%) had only anaerobes; and 91 patients (50%) had both aerobes and anaerobes. Only 1 specimen had no growth. There were 259 aerobic isolates. Pseudomonas aeruginosa was recovered from 68 patients. Other aerobes commonly recovered included Staphylococcus aureus and Klebsiella pneumoniae. There were 178 anaerobic isolates. Only anaerobic gram-positive cocci were isolated in 2

Herein, we present the case of a 64-year-old woman diagnosed with left calculous pyelonephritis. Gram-positive cocci in clusters were isolated from two sets of her blood culture. These cocci were later identified as Staphylococcus saprophyticus. Although S. saprophyticus is a common causative pathogen of urinary tract infection (UTI) among young women, coagulase-negative staphylococci including S. saprophyticus isolated from UTI patients' blood samples were often considered a result of contamination because of their rareness. Considering the high percentage of methicillin-resistant strains of
