PURPOSE: To evaluate the risk factors and renal prognosis of acute kidney injury (AKI) in patients with hemolysis, elevated liver enzymes, and low platelet count (HELLP) syndrome.
METHODS: Women with HELLP syndrome over a 15-year period at Peking Union Medical College Hospital, China, were retrospectively studied.

We describe a case of a woman who presented with HELLP syndrome in two subsequent pregnancies (age 36 and 40), at gestational age of 22 weeks in both pregnancies with a rapid onset and progression. A total of 20 women with HELLP syndrome (haemolysis, elevated liver enzymes and low platelets), and 20 gestational age-matched healthy controls participated in this study which was conducted at Assiut University Maternity Hospital, Egypt. Dual hepatic perfusion was assessed by evaluation of hepatic artery blood flow and portal vein blood flow. The total liver blood flow was taken as the sum of flow volumes in the common hepatic artery and portal vein as assessed by Doppler ultrasound. The total liver blood flow decreased significantly in women with HELLP syndrome, compared with controls.

The objective of this case report is to highlight the lack of screening tests that is capable of predicting HELLP syndrome before its occurrence. The patient developed severe pre-eclamptic toxaemia at 34+ weeks gestation. The foetus was growth retarded. The patient received anti-hypertensive therapy and was delivered by an emergency caesarean section. The patient developed post partum HELLP syndrome that required supportive treatment and renal haemodialysis; nevertheless, the patient fully recovered.

We describe a case of a severe, immediate postpartum HELLP syndrome (hemolysis, elevated liver enzyme levels, low platelet count) with reversible posterior leukoencephalopathy (RPLS) characterized by generalized seizure, altered mental status, and visual loss in a 37-year-old primigravid woman. Magnetic resonance brain imaging showed diffuse subcortical edema, which resolved completely after 2 weeks along with complete symptom regression. The pathogenesis of RPLS is discussed and the importance of a prompt diagnosis is emphasized, as is the crucial role of rapid blood pressure reduction.
