Hemi-hamate arthroplasties for proximal interphalangeal joint fracture dislocations: long-term follow-up in acute, subacute and chronic cases.

INTRODUCTION: Hemihamate arthroplasty (HHA) method is one of the recommended treatment methods for unstable proximal interphalangeal joint (PIPJ) fracture dislocations The primary goal of this method is to achieve a stable and well-aligned joint by reconstructing the palmar lip. The aim of this study is to present mid to long term results of patients treated with HHA method in the acute, subacute and chronically. MATERIALS AND METHODS: 22 patients was operated with this technique by single surgeon between 2013 and 2025. Patients were classified as acute (< 3 weeks), subacute (3-6 weeks), and

Long-Term Clinical and Radiological Results of Hemi-Hamate Arthroplasty for Proximal Interphalangeal Fracture Dislocation.

PURPOSE: This study aimed to assess the long-term clinical and radiologic results after hemi-hamate arthroplasty in patients with acute or chronic irreparable fractures of the base of the middle phalanx. METHODS: Between 2010 and 2014, hemi-hamate arthroplasties were performed on 12 patients with a mean age of 29.5 (SD 4.9) years. The patients included acute cases (treated within 6 weeks of the fracture) and chronic cases (treated after 6 weeks). We assessed the cases at a mean follow-up of 10.7 years. We recorded visual analog scale (VAS) scores, Quick Disabilities of the Arm, Shoulder, and

Management of Proximal Interphalangeal Joint Fracture Dislocations.

Fracture dislocations of the proximal interphalangeal (PIP) joint of the finger are often caused by axial load applied to a flexed joint. The most common injury pattern is a dorsal fracture dislocation with a volar lip fracture of the middle phalanx. Damage to the soft-tissue stabilizers of the PIP joint contributes to the deformity seen with these fracture patterns. Unfortunately, these injuries are commonly written off and left untreated. A late-presenting PIP joint fracture dislocation has a poor chance of regaining normal range of motion. The provider must be suspicious of these injuries.

Hemi-hamate osteochondral transplantation in proximal interphalangeal dorsal fracture dislocations: a minimum 4 year follow-up in eight patients.

Fracture dislocations of the PIP joint are challenging to treat. In hemi-hamate arthroplasty, the palmar lip joint surface is reconstructed using an osteochondral graft from the hamate and the immediate stability permits early movement. In the long term, collapse of non-vascularized osteochondral grafts might lead to degenerative arthritis. We examined the radiographic result after a minimum of 4 years with special reference to the development of osteoarthritis and its relation to clinical symptoms in eight patients, mean age 49 (25-66) years. After a mean of 60 (48-69) months, the arc of moti

Proximal row carpectomy: long-term results.

OBJECTIVES: To evaluate long-term clinical outcome of proximal carpectomy. Our assumption was that this intervention should result in long-term benefit, making a wrist painless, mobile, and functional, compatible with social and professional life, whatever the initial etiology of the degenerative wrist. METHODS: We report a continuous single centre retrospective series of 24 patients. Objective (mobility, strength, radiographic evaluation) and subjective (pain, subjective wrist value, functional scores) functions were assessed by an independent observer. Surgery was performed mainly for Kienb

[Ulnar carpometacarpal joint fracture luxation of fourth and fifth ray].

ABSTRACT: Carpometacarpal fracture dislocations of the ulnar rays are rare but complex injuries of the hand and frequently pose a significant diagnostic and therapeutic challenge. ABSTRACT: Epidemiologically, affected patients are predominantly young males, with a high proportion of injuries resulting from punching mechanisms. Owing to the often unspecific clinical presentation and difficult visualization in conventional radiographs, there is a substantial risk of delayed diagnosis and treatment, which appears to be associated with inferior clinical outcomes. Early CT scans in cases of suspec

Diagnostic Musculoskeletal Ultrasound in the Evaluation of the Carpal Tunnel and Median Nerve.

The carpal tunnel is a fibro-osseous canal at the wrist that is formed by the retinaculum and bony structures. The superficial border is composed of the flexor retinaculum, the floor of the tunnel is formed by the carpal bones, the medial (ulnar) border by the hamate, and the lateral (radial) border by the trapezium. Contents that originate in the forearm and run distally through the carpal tunnel include the finger and thumb flexors and the median nerve. Carpal tunnel syndrome, when the median nerve becomes compressed or entrapped, occurs due to various issues that will be discussed in this p

Technique for Reconstruction of Midcarpal Instability Associated with Lunotriquetral Coalition.

Carpal coalition is a rare congenital presentation of 2 or more fused carpal bones due to a failure of apoptotic segmentation during development. The most common subtype is lunotriquetral coalition (LTC). Most cases are asymptomatic and found incidentally on imaging; however, a few symptomatic cases requiring treatment have been reported. Surgical intervention of arthrodesis and proximal row carpectomy in adults have been reported where conservative management of splinting, physiotherapy, anti-inflammatory medication, or steroid injections have failed. We report a unique case of Minnaar type 2

Hook of hamate pull test.

Hook of hamate fractures are often missed despite a distinct history. The purpose of this report was to describe a specific physical examination test that is diagnostic for a hook of hamate fracture, the hook of hamate pull test. It is highly sensitive and was positive in a consecutive series of 5 patients treated for a hook of hamate fracture. These patients also had a distinctive history of playing racket- or club-type sports and presented with longstanding wrist or palm pain.

Treatment of unstable proximal interphalangeal joint fractures with hemi-hamate osteochondral autografts.

UNLABELLED: The primary aim of this study was to assess the clinical and radiological results after hemi-hamate resurfacing arthroplasty in patients with acute or chronic unstable fractures of the base of the middle phalanx and to describe technical features that can facilitate the surgical procedure. Hemi-hamate arthroplasties were done in 19 patients (mean age 39 years) with an isolated fracture at the base of the middle phalanx that involved more than 40% of the articular surface. We assessed ten chronic cases (treated >6 weeks after fracture) and nine acute ones (<6 weeks) at a mean of 24

Modified Hemihamate Arthroplasty With Oblique Osteotomy of the Middle Phalanx Base to Reconstruct Proper Contour of the Articular Surface.

Treatments of unstable dorsal fracture dislocations of the proximal interphalangeal (PIP) joint are very challenging. The authors performed modified hemihamate arthroplasty for these injuries in 13 patients (13 fingers). Our surgical technique was unique in the point that we put volarly oblique osteotomy in the coronal plane at the recipient site to create an adequate reconstruction of the volar lip of the articular surface of the middle phalanx. Affected fingers are index in 2, middle in 4, ring in 3, and little in 3, and the mean duration of follow-up was 14 months. Hemihamate arthroplasty w

The safest direction of percutaneous pinning for achieving firm fixing of the fifth carpometacarpal joint.

INTRODUCTION: Traditional treatment of unstable fifth carpometacarpal joint injuries is closed reduction and pinning. The purpose of this study was to determine the safest corridor for pinning of the fifth carpometacarpal joint to prevent iatrogenic injury to the ulnar nerve and tendons. MATERIALS AND METHODS: In the first phase of study, three fresh cadavers were dissected and the safest directions of Kirschner wire (k-wire) insertion in the coronal and sagittal planes were determined for k-wire entrance 2 cm distal to the base of the fifth metacarpal. The second phase objective was to evalu

Chronic asymptomatic contralateral wrist scapholunate dissociation.

BACKGROUND: The authors report a series of six patients who presented with scapholunate dissociation, with no significant contralateral antecedent trauma and no rheumatoid arthritis or congenital ligamentous laxity. Such patients may present with pain or a snapping/popping sensation in one wrist, undergo radiographs of both wrists, and are discovered to have scapholunate gapping bilaterally. The literature contains few reports of this condition, and this series of six is a relatively large one for this infrequently reported condition. METHODS: The six charts were reviewed retrospectively; eac

Lunate morphology as a risk factor of idiopathic ulnar impaction syndrome.

AIMS: Positive ulnar variance is an established risk factor for idiopathic ulnar impaction syndrome (UIS). However, not all patients with positive ulnar variance develop symptomatic UIS and other factors, including the morphology of the lunate, may be involved. The aim of this study was to clarify the relationship between lunate morphology and idiopathic UIS. PATIENTS AND METHODS: A cohort of 95 patients with idiopathic UIS (UIS group) was compared with 95 asymptomatic controls with positive ulnar variance. The shape of the lunate was measured using the capitate-triquetrum distance (CTD), uln
