[Nerve transplantation and neurolysis of the brachial plexus after post-traumatic lesions].

Autologous transplantation in brachial plexus injuries has become significantly more efficacious through increased knowledge of the microtopographic anatomy of the fascicle structures and the refinement of microneurosurgical techniques. Recovery of shoulder and upper arm functions can be achieved with autologous transplantation in the majority of patients under optimized conditions. However, attempts to restore forearm, hand and finger functions in this way have been disappointing so far. Therapeutic success is primarily governed by early decision making, selection of adequate surgical strateg

[Renal transplantation - new developments].

Renal transplantation has become an established option for renal replacement therapy in many patients with end stage renal disease. Living donation is a possibility for timely transplantation, hampered in 20 % of all possible donors and recipients byincompatible blood groups. AB0-incompatible renal transplantation overcomes this hurdle with acceptable allograft survival compared to conventional living-donor renal transplantation. During the last 10 years, the number of patients awaiting renal transplantation older than 65 years has nearly doubled. The decision to transplant those patients and

The First Hand Allotransplantation in Taiwan: A Report at 9 Months.

PURPOSE: Hand transplantations have been initiated and have been encouraged by promising results for more than 1 decade. The aim of this study was to present the first case of hand transplantation performed in Taiwan. MATERIALS AND METHODS: On September 3, 2014, we transplanted the left distal forearm and hand of a brain-dead managed 37 years to a man aged 45 years who had traumatic amputation of the distal third of his right forearm 30 years ago. The total ischemic time during the transplantation was 6 hours and 45 minutes. Immunosuppression included anti-thymocyte globulins, and methylpredn…

Corneal transplantation after failed grafts: Options and outcomes.

Corneal transplantation is the most commonly performed human tissue transplantation procedure worldwide. Because of the large number of transplants, corneal graft failure has become one of the most common indications for corneal transplantation. The relatively recently developed lamellar transplant techniques have brought about specific potential complications leading to graft failure that may require different approaches to repeat transplantation other than penetrating keratoplasty. On the other hand, these new lamellar techniques also provide novel ways of rescuing failed penetrating grafts,

Coccidioidal pneumonia, arthritis, and soft-tissue infection after kidney transplantation.

A 60-year-old male kidney transplant recipient presented with pneumonia 4 months after transplantation. Coccidioidomycosis was suspected and empirically treated with fluconazole. He was maintained on suppressive fluconazole without problems. Three weeks after discontinuing secondary prophylaxis, the patient experienced coccidioidal arthritis and an infection of the soft tissue of the hand that required debridement. Transplant recipients may have quiescent disseminated coccidioidomycosis that is reactivated by immunosuppression after withdrawal of suppressive antifungal therapy.

Liver Transplantation and Budd-Chiari Syndrome: When the Cause Becomes the Solution.

Budd-Chiari syndrome consists a rare medical entity which has an estimated incidence of 0.1 to 10 people per million every year. It is defined by the obstruction of the flow in the inferior vena cava or the hepatic veins. Various classifications have been proposed. So, it can be acute or chronic and primary or secondary. Iatrogenic, a subtype of secondary Budd-Chiari syndrome, is caused by various medical interventions, including liver transplantation. On the other hand, liver transplantation is the ultimate therapeutic management of Budd-Chiari syndrome. Finally, a medical paradox and a vicio

Challenges in Promoting Mitochondrial Transplantation Therapy.

Mitochondrial transplantation therapy is an innovative strategy for the treatment of mitochondrial dysfunction. The approach has been reported to be useful in the treatment of cardiac ischemic reperfusion injuries in human clinical trials and has also been shown to be useful in animal studies as a method for treating mitochondrial dysfunction in various tissues, including the heart, liver, lungs, and brain. On the other hand, there is no methodology for using preserved mitochondria. Research into the pharmaceutical formulation of mitochondria to promote mitochondrial transplantation therapy as

Mesenchymal stem cells alleviate experimental rheumatoid arthritis through microRNA-regulated IκB expression.

Previous studies have demonstrated that mesenchymal stem cell (MSC) transplantation reduces the severity of collagen-induced arthritis (CIA) in mice, which is a model for rheumatoid arthritis (RA) in humans. However, the underlying molecular mechanisms remain ill-defined. Here, we showed that MSC transplantation reduced the activities of NF-κB signaling and decreased microRNA-548e (miR-548e) levels in the joint tissue in CIA-mice, seemingly through activation of transforming growth factor β receptor signaling. Bioinformatics analyses revealed that miR-548e inhibited protein translation of the

Successful treatment of unresectable advanced hepatoblastoma: living liver transplantation after surgical removal of lung metastasis.

Hepatoblastoma is a rare malignant tumor of the liver in children. Intensive combination chemotherapy has increased the number of surgically resectable cases and improved prognosis markedly. However, unresectable cases and cases with residual metastasis, including lung metastases, have a poor prognosis. In these refractory cases, treatment strategy has not been established. On the other hand, living liver transplantation has been shown to be effective in cases of advanced hepatoblastoma, but its effectiveness in cases with residual distant metastases after chemotherapy remains unclear. We repo

Importance of informatics and database management in transplant coordination.

Advanced use of informatics within modern health care has become essential. Transplantation and transplant coordination, a high technically advanced and very specific niche within health care, is strongly depending on time management and exclusion of possible pitfalls within an acute organization at both the donor and the recipient sites. Based on the increased donor and transplant activity, we stratified two goals. The first goal was to improve working methods at the donor site. The second goal was to reduce administrative tasks and increase quality follow-up at the recipient side. For the do

Magnetic Resonance Imaging of Liver Transplant.

MR imaging increasingly has been adopted for follow-up imaging post-liver transplantation and for diagnosis of its complications. These include vascular and biliary complications as well as post-transplant malignancies. Interpretation of postoperative MR imaging should take into account the surgical technique and expected post-transplant changes. Contrast-enhanced MR imaging has high sensitivity for identification of vascular complications. MR cholangiopancreatography on the other hand is the most accurate noninvasive method for evaluation of biliary complications.

[Second toe transfer in congenital hand differences].

In congenital hand differences, microsurgical toe transfer involves the creation of one or several rays in a child that never possessed them before. Second toe transplantation was uniformly used in this series that include 65 consecutive cases. Etiologies were represented by symbrachydactyly, constriction band syndrome or central cleft hand. Second toe transfer was performed with or without the MTP joint and followed by proximal ray resection of the donor toe. In the vast majority of theses cases, transplantation was performed in adactylic or monodactylic hand, the ultimate functional goal was

Anesthetic management in upper extremity transplantation: the Pittsburgh experience.

BACKGROUND: Hand/forearm/arm transplants are vascularized composite allografts, which, unlike solid organs, are composed of multiple tissues including skin, muscle, tendons, vessels, nerves, lymph nodes, bone, and bone marrow. Over the past decade, 26 upper extremity transplantations were performed in the United States. The University of Pittsburgh Medical Center has the largest single center experience with 8 hand/forearm transplantations performed in 5 recipients between January 2008 and September 2010. Anesthetic management in the emerging field of upper extremity transplants must address p

Difference in onset between cytomegalovirus and idiopathic interstitial pneumonitis following allogeneic bone marrow transplantation for leukemia.

Interstitial pneumonitis is one of the major causes of morbidity and mortality following bone marrow transplantation (BMT). Based on the Japanese BMT Registry, however, since 1983 the incidence of interstitial pneumonitis at one year has decreased to 32% by means of several efforts, such as fractionated low-dose rate total body irradiation, selection of platelet donor from cytomegalovirus (CMV) seronegative donor, and prophylactic administration of anti-CMV high titer globulin. Cytomegalovirus (58%) was the most frequently causative organism within 100 days after bone marrow transplantation. O

Pretransplantation Imaging Workup of the Liver Donor and Recipient.

Liver transplantation requires thorough preoperative imaging. Contrast enhanced computed tomography (CT) and MR imaging are the main modalities utilized for this purpose. Radiation-reducing protocols, advanced imaging techniques for evaluation of parenchymal fat and iron content, and 3-dimenionsal and volumetric reformations are routinely used. The helpful diagnostic radiologist has a complete understanding of indications for transplant, the indications for bridging and/or down-staging therapies for hepatocellular carcinoma, and the components of the transplant operation. Reconstruction of the burned palm: full-thickness versus split-thickness skin grafts--long-term follow-up.

The long-term results of full-thickness (N = 11) and split-thickness (N = 14) skin grafts for reconstitution of the palmar surface following release of palmar burn scar contractures in pediatric patients are compared. Patients treated with full-thickness skin grafts required 1.2 +/- 0.4 operations (mean +/- SD). Patients treated with split-thickness skin grafts required 1.3 +/- 0.6 operations (mean +/- SD). No significant difference in the number of operative procedures was noted. No functional difference existed between the two groups. The use of split-thickness skin grafts provided comparabl

Predisposing factors for late mortality in heart transplant patients.

BACKGROUND: Because of the growing prevalence of terminal heart failure on the one hand and organ shortage on the other hand, an optimal care of heart transplant recipients based on the knowledge of potential risk factors not only early, but also in a long-term course after heart transplantation is of great importance. Therefore, the aim of the present study was to identify predisposing factors for late mortality in this patient collective. METHODS: Data from long-term heart transplant patients collected during follow-up visits in the current center were retrospectively analyzed. Clinical, la

One-stage reconstruction of complicated thumb injury with combination of microsurgical transplantations.

Complicated thumb loss of the hand still remains a great challenge to hand and microsurgeons. In this article, we report our technique and outcomes in 10 cases using one-stage microsurgical procedures. In each case, three tissue transplants in combination with a sequential vascular anastomoses was performed, i.e. the second toe for the thumb, the extensor digitorum brevis for thenar opponent muscle, and the anterolateral thigh flap for the first web space, and adjacent soft tissue defects. All the transplants survived eventually. After an average of 6 years follow-up, the results were very ins

Location of Dupuytren's disease on the radial aspect of the hand.

The fibrous elements of the radial side of the hand, i.e., index, first web space, and thumb, have not previously been fully described. However, involvement of this area may be significant, as seen in patients with a malignant form of Dupuytren's disease known as Dupuytren's diathesis, and adequate surgical removal depends on knowledge of the anatomy. Using cadavers, four major fibrous elements are defined: (1) the fibrous skeleton of the thenar eminence; (2) the radial longitudinally-oriented fibers of the palmar aponeurosis; (3) the distal transverse commissural ligament of the first web; an
