Implicit bias in the patient descriptor "homeless" and its association with emergency department opioid administration and disposition.

BACKGROUND: Biased language in provider documentation of marginalized patient populations has been shown to negatively influence patient management. There has been debate over the use of "homeless" as a descriptor of people experiencing homelessness (PEH), as it is a potentially biased term with negative connotations. This study explores the relationship between the use of the word "homeless" in Emergency Department (ED) provider documentation and admission rates, as well as intravenous (IV) vs. oral (PO) opioid administration rates. METHODS: This single-center retrospective study analyzed el

[Long and short stays under ASPPI: Comparison of clinical and non-clinical data from 250 initial certificates].

BACKGROUND: On July 5, 2011, France introduced a law permitting the involuntary admission of patients considered to be in "imminent danger" into psychiatric care without the consent of the family. This is known as "admission en soins psychiatriques pour péril imminent"(ASPPI). ASPPI authorizes all physicians to hospitalize a patient without his or her consent nor the consent of a third party. This differs from previous measures as only one certificate is needed. The law also requires involuntarily admitted patients to present themselves before a judge specialized in Liberties and Detentions (j

Characteristics of progressive multifocal leukoencephalopathy clarified through internet-assisted laboratory surveillance in Japan.

BACKGROUND: Progressive multifocal leukoencephalopathy (PML), a rare but fatal demyelinating disease caused by JC virus (JCV), occurs mainly in immunocompromised patients. As PML develops in individuals with various underlying disorders sporadically and infrequently, a nationwide survey of PML is difficult. This study was conducted to elucidate the characteristics of PML in Japan through an internet-assisted laboratory surveillance program. METHODS: A diagnostic support system for PML was established using a real-time PCR assay of JCV DNA in cerebrospinal fluid (CSF), and requests for testing

Trace of the tracer: Techno-solutionism, surveillance and the ambivalent experiences of digital health in Türkiye's COVID-19 response.

This study examines how digital health technologies were implemented in pandemic response, based on interviews and focus groups with frontline health workers in Türkiye. While the COVID-19 pandemic accelerated the adoption of digital tools as essential solutions for crisis management, the findings reveal a more ambivalent reality.Digital systems improved coordination, replaced paper-based chaotic workflows, and enabled real-time data sharing. However, their effectiveness was constrained by infrastructural shortcomings, uneven digital skills, and misalignment with field conditions. Digitalisati

Medical history, body size, and cigarette smoking in relation to fatal prostate cancer.

OBJECTIVES: Prostate cancer has few known risk factors. As part of a population-based case-control study conducted in four health maintenance organizations, the authors examined the associations between fatal prostate cancer and several medical and behavioral characteristics. METHODS: Cases were 768 health plan members who died of prostate adenocarcinoma during the period 1997-2001. We randomly selected controls (929) from the health plan membership and matched them to cases on health plan, age, race, and pattern of health plan membership. We examined medical records to obtain information on

Protocol of the baseline assessment for the Environments for Healthy Living (EHL) Wales cohort study.

BACKGROUND: Health is a result of influences operating at multiple levels. For example, inadequate housing, poor educational attainment, and reduced access to health care are clustered together, and are all associated with reduced health. Policies which try to change individual people's behaviour have limited effect when people have little control over their environment. However, structural environmental change and an understanding of the way that influences interact with each other, has the potential to facilitate healthy choices irrespective of personal resources. The aim of Environments for

Identifying and Addressing Housing Insecurity in Older Patients: Trends, Referrals, and Inequities in a California Medical System.

BACKGROUND AND OBJECTIVES: Housing insecurity is rising among older adults, especially for racially minoritized people. Few studies have evaluated whether healthcare institutions are meeting that challenge. Using data from a large California medical system, we examined how often older patients are (A) identified as housing insecure and then (B) referred to social services, as well as inequities in those rates. RESEARCH DESIGN AND METHODS: We analyzed electronic health records (2013-2022) for 119,127 older adults (55+) receiving primary or emergency care. We used a natural language processing

Seeing the unseen: how can we best identify transgender women within the Veterans Affairs healthcare system's electronic medical record?

BACKGROUND: One challenge in transgender research is reliably identifying patients through electronic medical records data, as there is no universal transgender International Classification of Diseases (ICD) code, but rather multiple ICD codes that can be used. AIM: To explore the sensitivity and specificity of 5 commonly used ICD codes to identify transgender patients overall and transgender women specifically (assigned male sex at birth) by using data from the Veterans Affairs (VA), the largest integrated health system in the United States. METHODS: Patients aged ≥18 years were identified

Usability testing finds problems for novice users of pediatric portals.

OBJECTIVE: Patient portals may improve pediatric chronic disease outcomes, but few have been rigorously evaluated for usability by parents. Using scenario-based testing with think-aloud protocols, we evaluated the usability of portals for parents of children with cystic fibrosis, diabetes or arthritis. DESIGN Sixteen parents used a prototype and test data to complete 14 tasks followed by a validated satisfaction questionnaire. Three iterations of the prototype were used. MEASUREMENTS: During the usability testing, we measured the time it took participants to complete or give up on each task.

Drug disorders and cardiovascular disease: the impact on annual hospital length of stay for the Medicare population.

We studied 3,942,868 Medicare patients (comprised of elderly and disabled) discharged with cardiovascular disease (CVD) during 1987, of which 41,095 (1%) had a drug disorder. Among this small subgroup, the percent of those overlapping with an alcohol and/or mental disorder is 33% for the elderly and 47% for the disabled. The presence of a drug disorder discharge diagnosis is associated with an excess of 329,650 days of hospital care and +174,498,071 in hospital charges as illustrated by a 51% increase in average annual days in the hospital for the elderly, and a similar 61% increase for the di

A comparison of web sites used to manage and present home blood pressure readings.

Home blood pressure (HBP) monitoring is now recommended as a routine component of blood pressure management in patients with known or suspected hypertension. Over the last few years, a large number of Web sites, commonly termed Personal Health Records, have been developed so that patients can manage and present HBP readings. The objective of this report is to describe and compare these Web sites. A list of 33 desirable Web site features, organized into 4 categories, was developed. Between June and August of 2009, a total of 60 Web sites was identified, of which 20 were free or free to try. Eac
