As early as the 1930s the term Master Hearing Aid (MHA) described a device used in the fitting of hearing aids. In their original form, the MHA was a desktop system that allowed for simulated or actual adjustment of hearing aid components that resulted in a changed hearing aid response. Over the years the MHA saw many embodiments and contributed to a number of rationales for the fitting of hearing aids. During these same years, the MHA was viewed by many as an inappropriate means of demonstrating hearing aids; the audio quality of the desktop systems was often superior to the hearing aids them…

Sudden onset sensorineural hearing loss is a medical emergency that continues to be poorly understood despite being recognized in the literature since 1944 (De Kleyn, 1944). A commonly used criterion to qualify for this diagnosis is a sensorineural hearing loss over three contiguous pure-tone frequencies of 30 dB or more that develops within 72 hours. The vast majority of cases are unilateral and the estimated annual incidence is 20 per 100 000 persons (Nosrati-Zarenoe et al, 2007). A cause for the hearing loss is only identified in up to 10% of cases but 50% of patients will improve spontaneo…

Sudden sensorineural hearing loss is usually unilateral and can be associated with tinnitus and vertigo. In most cases the cause is not identified, although various infective, vascular, and immune causes have been proposed. A careful examination is needed to exclude life threatening or treatable causes such as vascular events and malignant diseases, and patients should be referred urgently for further assessment. About half of patients completely recover, usually in about 2 weeks. Many treatments are used, including corticosteroids, antiviral drugs, and vasoactive and oxygen-based treatments. …

In this article, we review the common entities causing the hearing loss and divide these pathologies into conductive hearing loss, sensorineural hearing loss, and mixed hearing loss. With this format, congenital pathologies are addressed first followed by the acquired entities. We review the common pathologies and the imaging findings affecting the hearing pathways from outside to inside, starting at the external auditory canal and moving inward toward the midbrain.

The management of hearing loss in adults depends of etiology and its severity. It can be as simple as treating an external otitis, removing an impacted cerumen or a more complex one such as a surgery for otosclerosis. The hearing loss is managed mainly by new advances in hearing aids technology and implantable hearing devices which include BAHA, middle ear implant and cochlear implants. The research is focused on developing new molecules for intracochlear drug therapy to treat noise induced hearing loss, drug ototoxicity as well as hearing loss related to cochlear implant insertion trauma. Ant…

An HIV-positive man with hepatitis B co-infection, naïve to highly active antiretroviral therapy, with a CD4 of 594 copies/mL and HIV-1 viral load of 140,070 copies, presented with right-sided facial weakness and hearing loss. He had been treated for secondary syphilis three months earlier when his rapid plasma reagin (RPR) result was 1:16, this had fallen to neat. At presentation, his RPR had risen to 1:16 again. A magnetic resonance imaging scan showed enhancement of the internal auditory canal and right cochlea. His cerebrospinal fluid examination was normal. He was treated with acyclovir a…

A wide range of pathology involving the external, middle, and inner ear contribute to conductive and sensorineural hearing loss. Diagnostic imaging plays a critical role in the evaluation and management of hearing loss. High resolution computed tomography (CT) of the temporal bone and magnetic resonance imaging (MRI) are the preferred imaging modalities to evaluate the ear structures for causes of hearing loss, with the specific type of hearing loss and location of defect dictating which type of imaging is preferred. In general, the external auditory canal, middle ear space, mastoid, petrous a…

UNLABELLED: Aging is a natural consequence of a society developing process. The city of São Paulo has almost one million people who are above sixty years of age. Age-related hearing loss equals the total hearing loss resulted from cell degeneration caused by noise exposure, ototoxic agents and the loss caused by disorders and medical treatments.
AIM: To study age-related hearing degeneration by means of higher thresholds and hearing sensitivity measures.
MATERIALS AND METHODS: Cross-sectional contemporary cohort study in which we assessed 211 elderly patients with mean age of 75.24 years, of…

Age-related hearing loss or presbycusis is hearing loss that progressively worsens with age. With the expected increase in the number of elderly in the United States, the problem is anticipated to be increasingly common, and the impact widely felt in coming years. This article discusses the causes and mechanisms of this type of hearing loss and current research that may lead to new treatments.

Congenital hearing loss is estimated to affect one in every 1,000 newborns. Causes of hearing loss can be conductive, sensorineural, mixed, or central. Known risk factors for congenital hearing loss include cytomegalovirus infection and premature birth necessitating a stay in the neonatal intensive care unit. However, up to 42 percent of profoundly hearing-impaired children will be missed using only risk-based screening. Universal newborn hearing screening is a way to identify hearing-impaired newborns with or without risk factors. Newborns with positive screening tests should be referred for …

BACKGROUND: Myringoplasty is one of the various surgical techniques for the management of chronic supurative otitis media of tubotympanic type (CSOM-TT). The presence of a perforation of tympanic membrane with intermittent discharge and hearing loss of conductive nature are the indications of myringoplasty. It is a beneficial procedure done for closing tympanic membrane perforation and improving hearing.
OBJECTIVE: The aim of this study was to assess hearing improvement after myringoplasty within ten weeks following surgery.
MATERIAL AND METHODS: The study population consisted of 50 patients…

The present study was carried on forty-five patients of confirmed hypothyroidism. Complete clinical examination and laboratory investigations were done regarding audiological and vestibular system. It was found that hypothyroidism affects the ear at multiple sites producing various types of hearing impairment viz. conductive, sensorineural and mixed. Vestibular system was found to be affected only minimally. The patients were then given levothyroxine and follow-up was done when they were euthyroid, which revealed statistically significant improvement in hearing threshold in 30% ears, in which …

Hearing loss caused by exposure to recreational and occupational noise results in devastating disability that is virtually 100 percent preventable. Noise-induced hearing loss is the second most common form of sensorineural hearing deficit, after presbycusis (age-related hearing loss). Shearing forces caused by any sound have an impact on the stereocilia of the hair cells of the basilar membrane of the cochlea; when excessive, these forces can cause cell death. Avoiding noise exposure stops further progression of the damage. Noise-induced hearing loss can be prevented by avoiding excessive nois…

A group of 199 children and adolescents (153 boys, 46 girls) with autistic disorder was audiologically evaluated. Mild to moderate hearing loss was diagnosed in 7.9% and unilateral hearing loss in 1.6% of those who could be tested appropriately. Pronounced to profound bilateral hearing loss or deafness was diagnosed in 3.5% of all cases, representing a prevalence considerably above that in the general population and comparable to the prevalence found in populations with mental retardation. Hearing deficits in autism occurred at similar rates at all levels of intellectual functioning, so it doe…
