Heart Rate Imputation Using Accelerometers for Wearable Devices.

Heart rate (HR) estimation from photoplethysmography (PPG) signals is a vital component of many health monitoring systems. However, PPG signals are susceptible to motion artifacts, which can lead to inaccurate HR measurements. Moreover, PPG-based HR estimation often encounters catastrophic failures in the presence of excessive motion artifacts where reliable HR reading from PPG is not possible. In this study, we introduce a novel approach to reducing catastrophic failures by imputing missing HR using tri-axial accelerometer data in consumer-grade smart watches. Our proposed method employs a li

Contactless continuous heart rate monitoring system using ballistocardiography.

Cardiovascular disease is the number one cause of death in the world and is a serious problem. In the case of cardiopulmonary arrest due to myocardial infarction, the survival rate is as low as 13.3% one month after resuscitation, which birthed the need for continuous heart monitoring. In this study, we develop a Ballistocardiogram (BCG) measurement system using a load cell installed on a chair and a heart rate estimation algorithm that is robust to waveform changes, with the aim of constructing a non-contact heart rate acquisition system. The proposed system was evaluated by utilizing data ob

Temporal Evolution of Intrapartum Fetal Heart Rate Features.

Our research goal is to improve the intrapartum identification of tracings associated with severe acidosis at birth and subsequent hypoxic-ischemic encephalopathy so that timely interventions could avoid such complications without causing excessive unnecessary interventions in births with normal outcomes. The present study examines the evolution of fetal heart rate (FHR) features over the course of labor. We analyzed FHR signals collected in the last 6 hours before delivery in 21,853 births with normal neonatal outcomes and in 163 that developed hypoxic-ischemic encephalopathy (HIE) from 15 ho

Multichannel Esophageal Heart Rate Monitoring of Preterm Infants.

OBJECTIVE: Autonomic dysregulation in preterm infants requires continuous monitoring of vital signs such as heart rate over days to months. Unfortunately, common surface electrodes are prone to electrocardiography (ECG) signal artifacts and cause serious skin irritations during long-term use. In contrast, esophageal ECG is known to be very sensitive due to the proximity of electrodes and heart and insensitive to external influences. This study addresses if multichannel esophageal ECG qualifies for heart rate monitoring in preterm infants. METHODS: We recorded esophageal leads with a multi-ele

Appropriate heart rate during exercise in Fontan patients.

OBJECTIVE: To evaluate heart rate against workload and oxygen consumption during exercise in Fontan patients. METHOD: Fontan patients (n = 27) and healthy controls (n = 25) underwent cardiopulmonary exercise testing with linear increase of load. Heart rate and oxygen uptake were measured during tests. Heart rate recovery was recorded for 10 minutes. RESULTS: Heart rate at midpoint (140 ± 14 versus 153 ± 11, p < 0.001) and at maximal effort (171 ± 14 versus 191 ± 10 beats per minute, p < 0.001) of test was lower for patients than controls. Heart rate recovery was similar between groups. Heart

Lacunar stroke and heart rate variability during sleep.

OBJECTIVE: Autonomic instability during sleep can influence the risk of cerebrovascular diseases. In this study, we performed a polygraphy to evaluate, heart rate variability (HRV) in a group of patients with lacunar stroke that is a condition at high risk of recurrence. METHODS: Twenty-one lacunar stroke patients were enrolled one month after stroke occurrence. Sleep quality was assessed with the Pittsburgh Sleep Quality Index (PSQI). The presence of common sleep disorders and autonomic changes during the night was investigated by a polygraphy monitoring. Results were compared with those obt

Zipf's Law of Vasovagal Heart Rate Variability Sequences.

Cardiovascular self-organized criticality (SOC) has recently been demonstrated by studying vasovagal sequences. These sequences combine bradycardia and a decrease in blood pressure. Observing enough of these sparse events is a barrier that prevents a better understanding of cardiovascular SOC. Our primary aim was to verify whether SOC could be studied by solely observing bradycardias and by showing their distribution according to Zipf's law. We studied patients with vasovagal syncope. Twenty-four of them had a positive outcome to the head-up tilt table test, while matched patients had a negati

Correction of the QRS duration for heart rate.

OBJECTIVE: To determine the clinical value of correcting the QRS duration for heart rate. BACKGROUND: We recently observed [1] that the QRS duration shortens during spontaneous increases in heart rate. In the current study, we analyzed ECG and pharmacokinetic data of 21 subjects who received quinidine in a recent study [2]. They experienced the expected post-quinidine increase in heart rate, allowing us to determine if quinidine's well-known QRS prolongation might be attenuated due to the concomitant rate increase. METHODS: In a crossover-designed study, after baseline ECG recording, the sub

Heart rate, startle response, and intrusive trauma memories.

The current study adopted the trauma film paradigm to examine potential moderators affecting heart rate (HR) as an indicator of peritraumatic psychological states and as a predictor of intrusive memories. We replicated previous findings that perifilm HR decreases predicted the development of intrusive images and further showed this effect to be specific to images rather than thoughts, and to detail rather than gist recognition memory. Moreover, a group of individuals showing both an atypical sudden reduction in HR after a startle stimulus and higher trait dissociation was identified. Only amon

Heart rate affects endothelial function in essential hypertension.

Increased heart rate (HR) is a risk factor for cardiovascular morbidity and mortality in the general population and in some clinical conditions. Endothelial dysfunction is an adverse prognostic factor for cardiovascular events. The aim of the study was to evaluate the effect of HR on central hemodynamic parameters and endothelial function in hypertension. We evaluated forearm blood flow (FBF) response to intra-arterial infusion of acetylcholine (ACh) and sodium nitroprusside (SNP) in 30 patients with HR ≤60 min(-1) and 30 with HR ≥80 min(-1). The FBF was measured by strain-gauge plethysmograph

Adaptive multiscale complexity analysis of fetal heart rate.

Per partum fetal asphyxia is a major cause of neonatal morbidity and mortality. Fetal heart rate monitoring plays an important role in early detection of acidosis, an indicator for asphyxia. This problem is addressed in this paper by introducing a novel complexity analysis of fetal heart rate data, based on producing a collection of piecewise linear approximations of varying dimensions from which a measure of complexity is extracted. This procedure specifically accounts for the highly non-stationary context of labor by being adaptive and multiscale. Using a reference dataset, made of real per

Diurnal heart rate patterns in inappropriate sinus tachycardia.

BACKGROUND: Inappropriate sinus tachycardia (IAST) is a supraventricular tachycardia originating from the sinus node. Proposed etiologies for this symptom complex include autonomic dysfunction, abnormal automaticity, or hypersensitivity of the sinus node. METHODS: Patients with IAST were identified by symptomatic tachycardia, with P-wave morphology consistent with origination from a sinus location. A matched set of control subjects was included. Hourly heart rate (HR) was measured as the average HR during each one hour period on an ambulatory 24-hour Holter monitor. Patients were further divi

Effect of methamphetamine dependence on heart rate variability.

Methamphetamine (METH) is an increasing popular and highly addictive stimulant associated with autonomic nervous system (ANS) dysfunction, cardiovascular pathology and neurotoxicity. Heart rate variability (HRV) has been used to assess autonomic function and predict mortality in cardiac disorders and drug intoxication, but has not been characterized in METH use. We recorded HRV in a sample of currently abstinent individuals with a history of METH dependence compared to age- and gender-matched drug-free comparison subjects. HRV was assessed using time domain, frequency domain, and non-linear en

Analysis of heart rate variability and clinical implications.

In the last decades analysis of heart rate variability has emerged as a new useful tool for assessing the changes in autonomic tone that influences cardiovascular function. High variability of oscillations and the integrated physiologic activities of heart and cardiovascular system isassociated with physiologic functioning. Irregularity of the length of consecutive heart cycles, designated as heart rate variability, reflects the ability of cardiovascular system to adapt to different situations in everyday life. Reduction of these fluctuations of the heart periods has been shown to be associate

Heart rate and posttraumatic stress in injured children.

BACKGROUND: Elevated, acute heart rate has been related to later posttraumatic stress disorder (PTSD) development in injured adults, but this has not been examined in children and adolescents. Better understanding of the relationship between acute physiological arousal and later child PTSD could help elucidate the etiology of posttrauma responses in children and might identify useful markers for PTSD risk. OBJECTIVE: To evaluate the relationship between heart rate assessed in the emergency department (ED) during normal clinical care and later PTSD outcome in traumatically injured children. D
