Bridge to heart transplantation with left ventricular assist device versus inotropic agents in status 1 patients.

OBJECTIVE: Left ventricular assist devices (LVADs) are commonly used for critically ill patients awaiting heart transplantation, although their effect on long-term outcomes, relative to inotropic support alone, is still debated. METHOD: Data from Status 1 patients who underwent heart transplantation at our institution between 1990 and 2005 were reviewed (n = 180). They were divided into two groups: those who underwent LVAD implantation as a bridge to transplant (n = 31) and those treated with inotropic agents without the support of LVAD (n = 149). They were compared in terms of demographics a

TNFalpha in patients with end-stage heart failure on medical therapy or supported by a left ventricular assist device.

BACKGROUND: In the heart elevated levels of TNFalpha can cause lethal heart failure, like Dilated Cardiomyopathy (DCM). The level of TNFalpha production is in part determined by promoter gene polymorphisms. We investigated whether the TNFalpha promoter gene polymorphism is in this way involved in the outcome of end-stage heart failure and predicts whether patients require left ventricular assist device (LVAD) support or can be kept on medical therapy (MT)while awaiting heart transplantation (HTx). As most patients in this study received a heart transplant, the role of the TNFalpha polymorphism

Altered myocardial Ca2+ cycling after left ventricular assist device support in the failing human heart.

OBJECTIVES: The objective of the present study was to determine whether improved contractility after left ventricular assist device (LVAD) support reflects altered myocyte calcium cycling and changes in calcium-handling proteins. BACKGROUND: Previous reports demonstrate that LVAD support induces sustained unloading of the heart with regression of pathologic hypertrophy and improvements in contractile performance. METHODS: In the human myocardium of subjects with heart failure (HF), with non-failing hearts (NF), and with LVAD-supported failing hearts (HF-LVAD), intracellular calcium ([Ca(2+)]

Totally implantable total artificial heart and ventricular assist device with multipurpose miniature electromechanical energy system.

A multipurpose miniature electromechanical energy system has been developed to yield a compact, efficient, durable, and biocompatible total artificial heart (TAH) and ventricular assist device (VAD). Associated controller-driver electronics were recently miniaturized and converted into hybrid circuits. The hybrid controller consists of a microprocessor and controller, motor driver, Hall sensor, and commutation circuit hybrids. The sizing study demonstrated that all these components can be incorporated in the pumping unit of the TAH and VAD, particularly in the centerpiece of the TAH and the mo

Difficult cases in heart failure: Left ventricular assist device implantation for the treatment of recurrent ventricular tachycardia in end stage heart failure.

The authors describe the surgical implantation of a left ventricular assist device (LVAD) in a patient with ischemic cardiomyopathy and recurring episodes of ventricular tachycardia with associated sudden death, as a therapeutic intervention for the recurrent ventricular arrhythmias. The clinical inference of this report demonstrates that these devices are useful as a bridge to heart transplantation, not only improving the symptoms of heart failure but also suppressing malignant ventricular arrhythmias. Left ventricular assist devices versus medical management in ambulatory heart failure patients: An analysis of INTERMACS Profiles 4 and 5 to 7 from the ROADMAP study.

BACKGROUND: The ROADMAP study showed survival with improved functional status was better with left ventricular assist device (LVAD) therapy compared with optimal medical management (OMM) in ambulatory, non-inotrope-dependent (INTERMACS [IM] Profile 4 to 7) patients. To study more balanced cohorts and better define which patients may benefit from implantation of an LVAD, we re-evaluated the patients enrolled in ROADMAP when stratified by INTERMACS profile (Profile 4 and Profiles 5 to 7). METHODS: The primary end-point (survival on original therapy with improvement in 6-minute walk distance ≥75

Myocardial gene expression of regulators of myocyte apoptosis and myocyte calcium homeostasis during hemodynamic unloading by ventricular assist devices in patients with end-stage heart failure.

BACKGROUND: In patients with end-stage heart failure, characterized by an increased susceptibility to cardiomyocyte apoptosis and a labile cardiomyocyte calcium homeostasis, a ventricular assist device (VAD) is implanted for bridging to cardiac transplantation and results in myocardial unloading. Although phenotype changes in the failing heart are assumed to result from hemodynamic overload, the reversibility of these changes under unloading is unknown. METHODS AND RESULTS: By use of quantitative reverse-transcription polymerase chain reaction, mRNA expression analyses were performed on left

Right Heart Reserve Function Assessed With Fluid Loading Predicts Late Right Heart Failure After Left Ventricular Assist Device Implantation.

BACKGROUND: A left ventricular assist device (LVAD) is an effective therapeutic option for advanced heart failure. Late right heart failure (LRHF) is a complication after LVAD implantation that is associated with increasing morbidity and mortality; however, the assessment of right heart function, including right heart reserve function after LVAD implantation, has not been established. We focused on a fluid-loading test with right heart catheterization to evaluate right heart preload reserve function and investigate its impact on LRHF. METHODS: Patients aged > 18 years who received a continuou

Genetic Testing Practices and Pathological Assessments in Patients With End-stage Heart Failure Undergoing Heart Transplantation and Left Ventricular Assist Device Use.

BACKGROUND: Genetic cardiomyopathies (CMs) are increasingly recognized as causes of end-stage heart failure (ESHF). Identification of a genetic etiology in ESHF has important prognostic and family implications. However, genetic testing practices are understudied in patients with ESHF. METHODS: This single-center, retrospective study included consecutive patients with ESHF who underwent heart transplantation (HT) or left ventricular assist device (LVAD) implantation between 2018 and 2023. Data, including genetic testing and pathology reports, were collected from the electronic medical records.

Alterations in the sarcopenia index are associated with inflammation, gut, and oral microbiota among heart failure, left ventricular assist device, and heart transplant patients.

BACKGROUND: Sarcopenia, characterized by loss of muscle mass and function, is prevalent in heart failure (HF) and predicts poor outcomes. We investigated alterations in sarcopenia index (SI), a surrogate for skeletal muscle mass, in HF, left ventricular assist device (LVAD), and heart transplant (HT), and assessed its relationship with inflammation and digestive tract (gut and oral) microbiota. METHODS: We enrolled 460 HF, LVAD, and HT patients. Repeated measures pre/post-procedures were obtained prospectively in a subset of LVAD and HT patients. Impact of bridge-to-bridge strategies from paracorporeal to implantable left ventricular assist devices on the pre-heart transplant outcome: A single-center analysis of 134 cases.

BACKGROUND: In Japan, patients with heart failure who have a paracorporeal left ventricular assist device (pLVAD) and cannot be weaned from the VAD may undergo conversion to implantable continuous-flow LVAD (iLVAD) via a bridge-to-bridge (BTB) strategy for bridge-to-transplantation (BTT). This study aimed to evaluate the real-world clinical status of BTB strategies. METHODS: Among 134 patients who underwent iLVAD implantation for BTT, 34 patients underwent conversion from pLVAD to iLVAD (BTB group) and 100 patients underwent iLVAD implantation primarily (primary iLVAD group). The clinical cha

National trends and outcomes in device-related thromboembolic complications and malfunction among heart transplant candidates supported with continuous-flow left ventricular assist devices in the United States.

BACKGROUND: This study evaluated current trends in incidence and outcomes of left ventricular assist device (LVAD)-related thromboembolic (LVAD-TE) and LVAD malfunction (LVAD-M) complications among heart transplant (HT) candidates supported with continuous-flow LVADs. LVAD-TE and LVAD-M are potentially catastrophic complications that may require status upgrade on the HT waiting list. An increased incidence of device thrombosis has been recently observed; however, whether similar trends of LVAD-TE and LVAD-M are observed on the HT waiting list and their effect on outcomes is unknown. METHODS:

Short-Term Care Burden After Left Ventricular Assist Device Implantation and Heart Transplant in the United States.

BACKGROUND: Heart transplant (HT) and left ventricular assist devices (LVADs) are treatment options for advanced heart failure refractory to standard therapy. Historically, LVADs have been used as either destination therapy or a bridge to transplant. However, recent changes to the organ allocation system have deprioritized patients on LVADs as transplant recipients, leading to divisive views on the role of an LVAD. The comparative short-term care burden with each modality remains unclear. METHODS: We aimed to describe and compare characteristics, outcomes, and cost burden during index hospita

Hybrid durable biventricular assist device implantation with Berlin Heart EXCOR pulsatile RVAD and a continuous flow LVAD.

BACKGROUND: Biventricular failure or right ventricular failure (RVF) after continuous-flow left ventricular assist device (cf-LVAD) implantation is associated with high mortality. Durable right ventricular assist device (RVAD) options are limited, particularly for patients with critical cardiogenic shock or late-onset RVF. Use of right-sided cf-LVAD is costly, complex, and off-label. This study explores a hybrid VAD approach, combining a cf-LVAD and a Berlin Heart EXCOR pulsatile RVAD. METHODS: This multicenter retrospective analysis included patients who received a cf-LVAD with an EXCOR RVAD

Successful heart transplant after 10 years of left ventricular assist device support in a highly sensitized patient.

Long-term left ventricular assist devices (LVAD) are an effective bridge-to-transplant strategy in end-stage heart failure. Prolonged device exposure may induce humoral activation, leading to anti-human leukocyte antigen (HLA) and non-HLA alloimmunization, complicating transplant access. We report a 43-year-old woman supported by a continuous-flow HeartWare ventricular assist device (HVAD) for 10 years, with severe anti-HLA hyperimmunization. She successfully underwent heart transplantation with stable hemodynamics, satisfactory early graft function, and no acute rejection. The postoperative c
