Cytochemistry and immunochemistry in hematologic diagnoses.

With new understanding in the biology of hematologic diseases and the rapid progress in their treatment, it is essential to diagnose and classify the various hematologic diseases with great precision. In recent years, the cell type-specific markers have been used successfully in cell identification for more precise diagnosis and classification of neoplastic diseases. The cytochemical method for cell identification is simple and is most useful for disorders of granulocytes, monocytes/histiocytes, hairy cells, and mast cells. The immunochemical methods are particularly useful in identifying the

Pemigatinib for the treatment of myeloid/lymphoid neoplasms with FGFR1 rearrangement.

INTRODUCTION: Myeloid/lymphoid neoplasms with fibroblast growth factor receptor-1 (FGFR1) rearrangements (MLNFGFR1) are rare entities with aggressive features and poor prognosis. Presentation is heterogeneous, ranging from myeloproliferative neoplasms (with or without eosinophilia) to T-cell lymphoma and acute leukemia. Historical treatments have been guided by the presenting phenotype with induction chemotherapy frequently used. Pemigatinib is a FGFR1-3 tyrosine kinase inhibitor that has demonstrated high complete hematologic and cytogenetic response rates in MLNFGFR1. AREAS COVERED: We disc

Aberrant histone modifications induced by mutant ASXL1 in myeloid neoplasms.

An epigenetic modulator Additional sex combs-like 1 (ASXL1) is recurrently mutated in myeloid neoplasms such as myelodysplastic syndromes (MDS), acute myeloid leukemia (AML) and myeloproliferative neoplasms (MPNs). ASXL1 mutations are also frequently detected in clonal hematopoiesis with indeterminate potential (CHIP), which is the clonal expansion of premalignant hematopoietic cells without any evidence of hematological malignancies. Thus, understanding the roles of ASXL1 in hematopoiesis and myeloid neoplasms is a clinically crucial issue. ASXL1 mutations in hematological neoplasms are typic

COVID-19 in vaccinated adult patients with hematological malignancies: preliminary results from EPICOVIDEHA.

In a Plenary Paper, Mittelman and colleagues assess the relative clinical efficacy of mRNA vaccination on COVID-19 disease incidence and outcomes in patients with hematologic malignancies compared with healthy matched controls. This population-based study from Israel links prior observations of poor serologic responses to vaccination to higher risk for breakthrough infection, hospitalization, and death in patients with blood cancer, especially those on active antineoplastic therapy. In an accompanying Letter to Blood, Pagano et al provide supportive data using a multination survey approach to

Nonmelanoma Skin Cancer With Aggressive Subclinical Extension in Immunosuppressed Patients.

IMPORTANCE: Immunosuppression (IS), such as in solid-organ transplant recipients (SOTRs) and patients with human immunodeficiency virus (HIV) or hematologic malignant neoplasms, increases the risk of developing nonmelanoma skin cancers (NMSCs). However, it is unknown whether IS patients are at increased risk of developing NMSCs with aggressive subclinical extensions (NMSC-ASE), which may extend aggressively far beyond conventional surgical margins. OBJECTIVE: To study clinical characteristics of NMSC-ASE among immunocompetent (IC) and various subgroups of IS patients and to suggest a predicti

Fine-needle aspiration of secondary neoplasms involving the salivary glands. A report of 36 cases.

Metastases or secondary deposits account for 16% of the malignant neoplasms involving the major salivary glands. A correct diagnosis of a secondary neoplasm is important to avoid unnecessary radical surgery and to guide further therapy. Fine-needle aspiration biopsy (FNAB) is an excellent noninvasive diagnostic tool for evaluating salivary gland lesions. We reviewed 36 secondary malignant salivary gland neoplasms evaluated by FNAB. Ancillary studies were performed in selected cases. Follow-up included clinical correlation and review of histologic material. For 4 adenocarcinomas, 4 squamous cel

Role of p53 in regulation of hematopoiesis in health and disease.

PURPOSE OF REVIEW: Human aging is associated with an exponential increase in the occurrence of clonal hematopoiesis of indeterminate potential (CHIP). CHIP is associated with increased risks of de novo and therapy-related hematologic neoplasms and serves as a reservoir for leukemic relapse. Somatic mutations in the TP53 gene, which encodes the tumor suppressor protein p53, rank in the top five among genes that were mutated in CHIP. TP53 mutations in CHIP are associated with an increased incidence of myeloid neoplasms such as myelodysplastic syndromes (MDS) and acute myeloid leukemia (AML). Thi

Kaposi's sarcoma: relationship with hematologic, lymphoid, and thymic neoplasia.

Four cases of Kaposi's sarcoma (KS) are described; one each was associated with Hodgkin's disease, a thymoma, an atypical lymphoid proliferation resembling Castleman's disease, and a cutaneous reticulosis. The association of KS with lymphoproliferative, hematologic, and other neoplasms is thoroughly reviewed, with special attention to tissue diagnosis, relative order of development, and relationship to immunosuppressive treatment. Review of 65 accepted cases, including ours, showed that 85% of patients with KS and a known order of development appeared with KS either simultaneously or after the

Thrombosis in Pregnant Women with Hematological Malignancies: A Case-Based Review.

Cancer and pregnancy induce a procoagulant environment which may lead to maternal and fetal complications, such as venous thromboembolism, fetal growth restriction, and fetal loss. The incidence of hematological malignancies diagnosed during pregnancy is rising, and thrombotic events in such malignancies are not rare. Management of thrombosis during pregnancy poses a therapeutic challenge, that is further exacerbated by the impact of cancer. The available data on managing pregnant women with hematological malignancies are limited to those with myeloproliferative neoplasms, mainly essential thr

Role of the gut microbiota in hematologic cancer.

Hematologic neoplasms represent 6.5% of all cancers worldwide. They are characterized by the uncontrolled growth of hematopoietic and lymphoid cells and a decreased immune system efficacy. Pathological conditions in hematologic cancer could disrupt the balance of the gut microbiota, potentially promoting the proliferation of opportunistic pathogens. In this review, we highlight studies that analyzed and described the role of gut microbiota in different types of hematologic diseases. For instance, myeloma is often associated with Pseudomonas aeruginosa and Clostridium leptum, while in leukemias

Auer rods and faggot cells: A review of the history, significance and mimics of two morphological curiosities of enduring relevance.

Myeloid differentiation in blasts is distinguished by the presence of one or more needle-shaped crystalline structures called Auer rods. Auer rods manifest either alone or as faggot cells (containing bundles of Auer rods) in various types of acute myeloid leukemia (AML), myelodysplastic syndromes (MDS) and myelodysplastic/myeloproliferative neoplasms (MDS/MPN). Their presence largely portends a better prognosis in AML (as markers of maturation/differentiation) and upstages cases of MDS and MDS/MPN. Observation of these rods in residual blasts in treated cases of AML indicates an absence of rem

Clinical Significance of TP53-Mutant Clonal Hematopoiesis Across Diseases.

UNLABELLED: Clonal hematopoiesis of indeterminate potential (CHIP) has broad clinical relevance, and TP53 plays various roles within cells. However, the gene-specific and cross-disease significance of CHIP with TP53 mutations (TP53-CHIP) remains unclear. In this study, we evaluated TP53-CHIP using targeted sequencing data of 140,597 individuals without hematologic neoplasms in BioBank Japan. We identified 1,157 individuals with TP53-CHIP and clarified the characteristics of mutations and carriers. TP53-CHIP was associated with poor overall survival, especially because of lymphoid neoplasms and

Differences in the diagnosis and treatment of hematologic malignancies attributable to health insurance coverage.

Medical care should be equally provided to the public regardless of their financial capability. In the real world, expenditures directly out from the patient sector decide the medical journey, even in a country with national health insurance. The aim of this study was to investigate whether there are differences in the diagnostic and treatment processes in hematologic malignancies based on patient characteristics, such as health insurance status.Through the review of 5614 "CBCs with differential count" results with abnormal cells from 358 patients from January 2010 to June 2017, 238 patients w

Pulmonary disease in patients with hematologic malignancies.

Patients with hematologic neoplasms frequently experience pulmonary disease. The possibility of a malignant involvement of the lung parenchyma is a well recognized and not unusual event, secondary spread due to lymphoproliferative disorders being the most common situation. Furthermore, the development and the advances in treatment options such as hematopoietic stem cell transplantation, radiation therapy and/or combined drug regimen use have significantly widened the spectrum of non-neoplastic pulmonary complications that can crop up in these patients. Infections, drug/radiation-induced toxici
