PURPOSE: The transanal hemorrhoidal de-arterialization (THD) procedure is an effective treatment of hemorrhoidal disease. The ligation of hemorrhoidal arteries ("de-arterialization") can provide a significant reduction of arterial blood flow to the hemorrhoidal tissues. Tetanus after hemorrhoidal banding is an extremely rare but serious complication of the procedure. We describe the second reported case of this complication and review the literature concerning tetanus after different gastrointestinal procedures. Although a rare complication, practicing physicians need to be aware of the clinical presentation of this deadly disease when encountered in at-risk patient populations. Such cases also reemphasize the importance of primary tetanus immunization and follow-up boosters for all vulnerable patients.

A new approach for the pathogenesis and treatment of recurrent bacteriuria is presented. Ten patients with recurrent bacteriuria were studied. Episodes started 3-8 years prior to presentation. Different antimicrobial therapy and surgical procedures had no influence on the recurrence of infection. Three cases of rectal bleeding within a series of 120 transrectal prostatic biopsies are reported. The precipitating factor was the presence of hemorrhoids which were unsuspected prior to prostatic biopsy. Rectal bleeding in these patients was arrested by ballon tamponade with a 24 French Foley catheter.
