Hepatitis C: education and counseling issues.

This paper examines issues and dilemmas faced by counselors and other health care providers in addressing the needs of active or recovering alcohol and drug users who test positive for HCV. The ambiguities and changing treatment prospects for these patients require education and counseling interventions, elements of which are described in the paper. Motivational enhancement issues are particularly important, to facilitate the patient's commitment to maintain a healthy life style while effective treatments are being developed. The high proportion of recovering staff in addiction treatment setti

[Cirrhosis due to hepatitis C virus].

Occurrence of cirrhosis is a milestone in the protracted course of chronic hepatitis C. Indeed, liver decompensation and hepatocellular carcinoma occur only in cirrhotic patients, resulting in morbidity and mortality. Accordingly, being certain of cirrhosis diagnosis is needed. Clinical management of patients with compensated cirrhosis type C is mainly based on unspecific measures, such as stopping of regular drinking, preventive treatment of variceal haemorrhage, regular screening and early treatment of hepatocellular carcinoma. Interferon treatment is rarely effective from a virological poin…

Neurological manifestations of chronic hepatitis C.

Hepatitis C virus (HCV) infection is often associated with abnormal immunological responses. We describe four patients with vasculitic neurological signs and symptoms following HCV infection. A 56-year-old woman with HCV infection developed peripheral neuropathy characterized by asymmetric distal painful hypesthesia, dysesthesia and moderate motor weakness of the lower limbs. Serological examinations revealed cryoglobulinemia and low levels of complement C4. A biopsy of the sural nerve revealed vasculitic neuropathy. HCV infection associated immunomediated vasculitis was diagnosed. While stero…

[The European consensus on hepatitis C].

The European Association for the Study of the Liver (EASL) International Consensus on Hepatitis C made by 72 experts in hepatology, epidemiology and virology at the EASL Consensus Conference, Paris, February, 1999 and confirmed by the 34th EASL Annual Meeting, Naples, 8-12 april 1999, is widely reviewed. Some newest and more effective strategies for treatment of the chronic viral hepatitis C were discussed during the Naples EASL meeting. Higher doses interferons (interferon-alpha or consensus interferon) plus ribavirin (or combination ribavirin and amantadine) for a longer period--12 months, i…

[Extrahepatic manifestations of hepatitis C infection].

The hepatitis C virus is associated with various extrahepatic manifestations. It is unequivocal that hepatitis C is associated with mixed cryoglobulinemia, membranoproliferative glomerulonephritis, and in Southern Europe to some extend also with porphyria cutanea tarda. The rare combination of hepatitis C and panarteriitis nodosa is still questionable. The Sicca syndrome also seems to be associated with hepatitis C virus, but this is not the typical Sjögren syndrome. It is still unclear whether the rare combination of hepatitis C with aplastic enemia has pathogenetic aspects. Although an epide…

Hepatitis C virus and renal transplantation.

During the past 12 months additional evidence has emerged from several studies, indicating that hepatitis C virus infection is the most important liver disease after renal transplantation. A new, severe and rare entity called fibrosing cholestatic hepatitis can lead to early liver failure, although the most important complications appeared in the long-run. Encouraging results with ribavirin have been described. Although glomerular lesions and more severe infections can appear in hepatitis C virus patients, graft and patient survival rates in most series are similar to those in hepatitis-C-nega…

[Epidermolysis bullosa acquisita and hepatitis C].

INTRODUCTION: Epidermolysis bullosa acquisita is a bullous dermatosis. Its etiology remains unknown and the efficacy of its treatment is low. OBSERVATION: We report the first association between epidermolysis bullosa acquisita, chronic hepatitis C and cryoglobulinemia, healing with interferon alpha and ribavirine. DISCUSSION: We suggest a role for hepatitis C virus in the pathogenesis of epidermolysis bullosa acquisita. We suppose a synthesis of autoimmune antibodies in a dysimmune environment. Interferon alpha and ribavirine might be a new therapeutic avenue but further studies are necessar…

Hepatitis C virus infection and cryoglobulinaemia.

Mixed cryoglobulins (CG) are serum proteins that precipitate at low temperature and are commonly classified into two types according to the presence (type II) or not (type III) of monoclonal immunoglobulins. Mixed CG are observed in a wide variety of diseases. Some mixed cryoglobulinaemia occurs without evidence of an underlying disease and is considered as essential mixed cryoglobulinaemia (EMC). Many studies have underlined the possible involvement of liver diseases in the pathogenesis of cryoglobulinaemia and particularly viral hepatitis. Recently, it has been shown that 50 to 80% of patien

Hepatitis C in asymptomatic blood donors.

Among 248 asymptomatic blood donors positive for antibody to hepatitis C virus (anti-HCV) enrolled in a long-term prospective study, 86% had chronic HCV infection and 14% appeared to have recovered as assessed by serial determinations of serum alanine aminotransferase (ALT) levels and HCV RNA by polymerase chain reaction. Established parenteral risk factors for HCV transmission were identified in 75% of donors. In addition, there was a strong independent association between HCV positivity and cocaine snorting, suggesting that shared snorting devices may be a covert route of parenteral transmis…

Therapy of hepatitis C: other options.

Because current standard therapy of chronic hepatitis C with alpha interferon is less than ideal, numerous other approaches have been studied. Iron in the liver, particularly that found in vascular endothelial cells of portal tracts, has been associated with decreased responsiveness to alpha interferon therapy. Iron reduction alone, generally achieved by therapeutic phlebotomy, regularly has been associated with biochemical improvement (decrease in serum alanine aminotransferase), but not with virological improvement. Iron reduction has been reported to increase the therapeutic response to alp…

[Hepatitis C-associated vasculitic mononeuritis multiplex].

HISTORY AND CLINICAL FINDINGS: A 56-year-old woman with hepatitis C had symptoms of a polyneuropathy with asymmetrical distal pareses and painful sensory disturbances in the limbs. INVESTIGATION: In addition to positive serology for hepatitis C cryoglobulins were demonstrated and complement C4 was reduced. Biopsy of the sural nerve showed a vasculitic neuropathy. TREATMENT AND COURSE: The multiple neuropathy (cryoglobulinaemia neuropathy), associated with hepatitis C, was treated with corticosteroids (starting with 60 mg/d prednisolone) and the neurological abnormalities regressed well. Hepatitis C virus-associated mixed cryoglobulinaemia.

Symptomatic mixed cryoglobulinaemia may dominate the course of hepatitis C virus infection. While conventional treatment of mixed cryoglobulinaemia relied on immunosuppression, the discovery of the close association to HCV infection has prompted treatment studies with interferon-alpha. Although symptomatic improvement is achieved in most patients during therapy, the rate of sustained response is low.

Nosocomial transmission of hepatitis C virus.

The nosocomial transmission of hepatitis C involving three subjects participating in a research project in a Swedish hospital is described. One person contracted acute hepatitis C and another participant had an asymptomatic seroconversion. A temporal relationship between participation in the experiments and seroconversion for hepatitis C, and virology with phylogenetic analyses, suggest that they most probably contracted hepatitis C from a third subject who unknowingly had chronic hepatitis C. Faulty nursing and procedural routines were identified as the probable cause of the transmission. Our…

Serum cholesterol and chronic hepatitis C.

Total serum cholesterol levels have been studied in 100 patients with histological diagnoses of chronic hepatitis B and 100 wit chronic Hepatitis C, all without cirrhosis, and two age- and sex-matched control groups (B and C). Mean serum cholesterol levels of the groups were compared also in relation to sex, liver function, duration of the disease, alcohol intake, mass index, liver enzymes, presence of liver steatosis and severity of the liver disease on the basis of the histological activity index. The percentages of patients with serum cholesterol level < 150 mg/dl and > 240 mg/dl were also …

Hepatitis C and porphyria cutanea tarda.

Porphyria cutanea tarda (PCT) is a clinical manifestation of decreased uroporphyrinogen decarboxylase (UPD) activity. Multiple endogenous and exogenous factors have been implicated in inducing PCT in genetically predisposed patients. The most recent is the RNA virus hepatitis C (HCV), which is transmitted via blood exposure. The mechanism of action in HCV-induced PCT is unknown but produces the same clinical, laboratory, and histopathologic changes seen in other forms of sporadic PCT. Therefore, patients presenting with PCT clinically should be tested serologically for antibodies against HCV a…

[Familial transmission of hepatitis C virus].

OBJECTIVE: To establish the prevalence of antibodies to hepatitis C virus (HCV) among relatives and other persons cohabiting with HCV carriers, as an indication of the risk of intradomestic transmission. EXPERIMENTAL DESIGN: A transversal study of series of clinical cases. A questionnaire was applied to the index cases (IC) and contact cases (CC), differentiating sexual contacts (SC) from non sexual contacts (NSC). PATIENTS: One hundred and sixty four IC and 533 CC were included. Information on parenteral risk factors, type of sexual relationship, period of exposure, and other epidemiologica…

Histopathologic findings in chronic hepatitis C.

Evaluation of liver biopsies in hepatitis C is aimed at confirming the clinical and serologic diagnosis, grading of necroinflammatory activity, staging of consecutive fibrosis, ruling out or confirming liver diseases of different etiology, and assessment of therapeutic effects. Usually, the course of chronic hepatitis C virus (HCV) infection is slow, with mild inflammatory changes. Nevertheless, even in mild asymptomatic chronic hepatitis C episodes of higher inflammatory activity associated with extensive piecemeal necrosis and porto-central bridging, necrosis can accelerate the course of the…

The natural history of hepatitis C.

The natural history of hepatitis C is complex and still poorly known. Hepatitis C virus (HCV) replication can be detected very soon after exposure and, at least in the transfusional setting, it persists indefinitely in up to 90% of the cases. While liver damage during the acute phase of hepatitis is almost invariably mild (fulminant cases are exceptions), chronic sequelae of HCV infection may be severe in the long run. Chronic hepatitis C, in fact, is a long-lasting indolent process which leads to cirrhosis in approximately 20% of all infected patients. Hepatocellular carcinoma is a well-recog…
