High-frequency oscillatory ventilation during physiological-based cord clamping attenuates inflammation in preterm lambs.

BACKGROUND: Adequate lung aeration at birth is essential for a successful transition to extrauterine life. Premature infants are especially vulnerable to injury caused by mechanical ventilation, particularly when exposed to excessive tidal volumes during initial respiratory support. OBJECTIVE: To evaluate whether high-frequency oscillatory ventilation (HFOV) initiated at birth facilitates lung aeration and reduces lung inflammation and injury, compared with conventional mechanical ventilation (CMV) in preterm lambs. DESIGN: Preterm lambs (126±1 days' gestation, term ~148 days) were instrumen

Skin-to-Skin Therapy on High-Frequency Jet Ventilation: A Trauma-Informed Best Practice.

OBJECTIVE: To mitigate trauma for infants on high-frequency jet ventilation by decreasing exposure to noise and facilitating skin-to-skin therapy. DESIGN: Key drivers were identified, and we designed and implemented equipment and processes through a series of interventions. A mixed methods evaluation was used. Retrospective chart reviews assessed safety (unplanned extubation) and stability parameters. Semi-structured interviews were conducted to understand parent and staff experiences. RESULTS: Stability parameters demonstrated safe skin-to-skin therapy. Data from the interviews showed that

Anesthetic management of a neonate with congenital diaphragmatic hernia under high-frequency oscillatory ventilation.

Perioperative management of a neonate with congenital diaphragmatic hernia (CDH) is challenging because of pulmonary hypoplasia, pulmonary hypertension, and respiratory insufficiency. In this report, we present our intra-operative experience in a 4-days old and 3070 grams CDH neonate. He was admitted to neonatal intensive care unit and intubated due to severe respiratory insufficiency. He showed signs of severe pulmonary hypoplasia and his echocardiography revealed a cardiac dextroversion. The patient was relatively stabilized after four days under combined high-frequency oscillatory ventilati

Bronchoesophageal Fistula Stenting Using High-Frequency Jet Ventilation and Underwater Seal Gastrostomy Tube Drainage.

Managing a patient scheduled for bronchoesophageal fistula repair is challenging for the anesthetist. If appropriate ventilation strategy is not employed, serious complications such as hypoxemia, gastric distension, and pulmonary aspiration can occur. We present the case of a 62-year-old man with a bronchoesophageal fistula in the left main stem bronchus requiring the insertion of a Y-shaped tracheobronchial stent through a rigid bronchoscope, under general anesthesia. We successfully managed this intervention and herein report this case to demonstrate the effectiveness of underwater seal gast

Impact of conventional breath inspiratory time during high-frequency jet ventilation in preterm lambs.

BACKGROUND: Conventional mechanical ventilator (CMV) breaths during high-frequency jet ventilation (HFJV) are advocated to recruit and stabilize alveoli. OBJECTIVES: To establish if CMV breath duration delivered during HFJV influences gas exchange, lung mechanics and lung injury. METHODS: Preterm lambs at 128 days gestational age were studied. HFJV (7 Hz, PEEP 8 cm H(2)O, PIP(HFJV) 40 cm H(2)O, FiO(2) 0.4) with superimposed CMV breaths (PIP(CMV) 25 cm H(2)O, rate 5 breaths/min) was commenced after delivery and continued for 2 h. CMV breath inspiratory time (t(I)) was either 0.5 s (HFJV+CMV(0

High frequency jet ventilation through the laryngeal mask airway in a critically obstructed airway.

A 72 year-old man with an anticipated difficult airway secondary to a large, obstructing supraglottic tumour was scheduled for a panendoscopy. An elective pre-induction transtracheal catheter was placed to allow oxygenation if airway maintenance proved difficult. Following induction of anaesthesia, tracheal intubation was impossible and ventilation via both a facepiece and a laryngeal mask airway became progressively more difficult. An emergency tracheostomy was attempted whilst oxygenation was maintained with transtracheal jet ventilation, but the position of the transtracheal catheter made t

Nitric oxide inhalation and high frequency oscillatory ventilation for hypoxemic respiratory failure in infants.

BACKGROUND: High frequency oscillatory ventilation has proved valuable in recruiting and sustaining lung volume; the combined treatment may augment nitric oxide delivery to target vessels. NO therapy lowers pulmonary resistance and improves oxygenation. OBJECTIVE: To retrospectively review data on changes in oxygenation-indicated by arterial/alveolar PO2 ratio, oxygenation index, and outcome--in a cohort of 10 infants with hypoxemic respiratory failure in whom nitric oxide inhalation was instituted in a compassionate-use protocol after deteriorated oxygenation. METHODS: NO inhalation was adm

[Application of high frequency jet ventilation during microsurgery of the larynx under suspending laryngoscopy].

OBJECTIVES: This study describes the results obtained with high frequency jet ventilation (HFJV) in 19 patients who underwent laryngeal microsurgery under suspending laryngoscopy. MATERIAL AND METHODS: After premedication and anesthetic induction with propofol (Diprivan R, ICI-Farma), we performed an endotracheal intubation (28F tube) under succinyl-choline facilitation. Conventional ventilation was started at a rate of 14-16 cpm with a minute volume of 8-10 l, and a FiO2 of 0.4 (air-oxygen). The endotracheal tube was replaced by an insufflation catheter of 2 mm internal diameter and HFJV was

Laryngeal mask airway and high-frequency jet ventilation for the resection of a high-grade upper tracheal stenosis.

The surgical resection of a high-grade tracheal stenosis presents a special case of a difficult airway. A 20-year-old male was treated for a 45-mm long tracheal stenosis with 60% reduction of the patent airway area beginning 25 mm below the glottis. To avoid manipulations of the affected segment before surgical exposure of the trachea was established, strictly supraglottic ventilation via a laryngeal mask airway was performed. During removal of the stenosis and creation of the anastomosis, transglottic high-frequency jet ventilation (HFJV) is a convenient technique that enables optimal access

Effects on Lung Function of Small-Volume Conventional Ventilation and High-Frequency Oscillatory Ventilation in a Model of Meconium Aspiration Syndrome.

For treatment of severe neonatal meconium aspiration syndrome (MAS), lung-protective mechanical ventilation is essential. This study compared short-term effects of small-volume conventional mechanical ventilation and high-frequency oscillatory ventilation on lung function in experimentally-induced MAS. In conventionally-ventilated rabbits, MAS was induced by intratracheal instillation of meconium suspension (4 ml/kg, 25 mg/ml). Then, animals were ventilated conventionally with small-volume (f-50/min; VT-6 ml/kg) or with high frequency ventilation (f-10/s) for 4 h, with the evaluation of blood
